Protocol for a biomarker discovery study to identify correlates of risk for future tuberculosis disease progression in South African children (INTREPID).

INTRODUCTION: Young children and children living with HIV are at high risk of progressing to tuberculosis (TB) disease following Mycobacterium tuberculosis (Mtb) exposure and infection, and also of developing severe forms of disease and TB-related mortality. Identifying children who have very early (sub-clinical) TB disease, prior to progression to clinically apparent TB, would mean that TB preventive treatment (TPT) could be more efficiently targeted to this group. Identifying biomarker changes on drug therapy in children with Mtb infection or very early disease could pave the way for the dev

PROspective Prostate Cancer Infrastructure: study protocol for the ProPCI 'trials within cohorts' study.

INTRODUCTION: The diagnostic and therapeutic landscape for high-risk localised prostate cancer and synchronous metastatic hormone-sensitive prostate cancer (mHSPC) is rapidly evolving, driven by advances in imaging, risk stratification and systemic therapies, including the advent of precision medicine. High-quality real-world data integrating clinical, imaging, molecular, quality-of-life information and outcome data remain scarce. The PROspective Prostate Cancer Infrastructure (ProPCI) is a nationwide, multicentre observational cohort designed to collect comprehensive longitudinal data and bio

Multimodality detection of early relapse in radically treated high-risk HPV-negative head and neck squamous cell carcinoma: study protocol for a prospective multicentre UK cohort study (HERD).

INTRODUCTION: Head and neck squamous cell carcinoma (HNSCC) is the seventh most common cancer globally, with over 12 000 new cases annually in the UK. Despite aggressive radical treatment, approximately 50% of patients with locally advanced (stage III/IV), human papilloma virus (HPV)-negative disease relapse within 2 years, often in the first year. Although current post-treatment follow-up schedules are recommended by clinical guidelines, imaging surveillance is led by symptoms after the initial 3 months and reliable biomarkers for early relapse are lacking. Consequently, recurrences are frequ

Effect of intermittent theta burst stimulation combined with acoustic startle priming motor training on upper limb motor function and neural plasticity in stroke individuals: study protocol for a randomised controlled proof-of-concept trial.

INTRODUCTION: Stroke is a major cause of acquired disability globally, yet the neural mechanisms driving motor recovery post-stroke remain elusive. Recent research has underscored the growing significance of subcortical pathways in neural plasticity and motor control. Among these, the cortico-reticulospinal tract (CRST) has gained attention in rehabilitation due to its unique ascending and descending structural features as well as its cellular properties which position it as an excellent candidate to compensate for inadequate motor control post-stroke. However, the optimal strategies to harnes

High-dose high-intensity Queen Square upper-limb rehabilitation for people with chronic stroke (INTENSIVE): protocol for a single-centre, randomised controlled trial.

INTRODUCTION: There is currently insufficient high-quality evidence to make general recommendations about high-dose high-intensity upper-limb rehabilitation programmes. Here we describe a randomised controlled trial that will determine the efficacy of two forms of high-dose, high-intensity upper-limb rehabilitation provided in a rehabilitation unit setting. METHODS AND ANALYSIS: Patients with moderate upper-limb impairment (n=105, at least 6 months after stroke) will be randomised to either (1) high-dose high-intensity conventional upper-limb rehabilitation, (2) high-dose high-intensity virtu

Non-invasive device to alleviate symptoms in people living with Parkinson's: study protocol for a multicentre phase II double-blind randomised controlled trial.

INTRODUCTION: Existing interventions for people with Parkinson's disease (PwP) often fall short in addressing gait disturbances and falls, impacting their quality of life. The CUE1 non-invasive medical device, along with its updated version, CUE1+, offers vibrotactile stimulation with cueing. The device shows promise in alleviating motor symptoms and reducing falls based on early user testing and a 9-week pilot study. This study aims to assess the usability, safety, tolerability and effectiveness of CUE1+ in improving Parkinson's symptoms compared with a sham device over a 12-week period. MET

Safety of early mobilisation in the intensive care unit: a prospective and multicentre cohort study protocol.

INTRODUCTION: Intensive care unit-acquired weakness (ICU-AW) is a common complication in critically ill patients, with an incidence of approximately 50% in the ICU. It is closely associated with increased morbidity, physical deterioration and higher short-term and long-term mortality. Its main characteristics include generalised and symmetrical muscle weakness affecting both respiratory and limb muscles. Early mobilisation has been identified as a feasible intervention in critically ill patients to mitigate the effects of ICU-AW. However, the incidence and nature of potential adverse events as

APPROACH: Analysis of Proton versus Photon Radiotherapy in Oligodendroglioma and Assessment of Cognitive Health - study protocol paper for a phase III multicentre, open-label randomised controlled trial.

INTRODUCTION: Oligodendroglioma (ODG) is a rare type of brain tumour, typically diagnosed in younger adults and associated with prolonged survival following treatment. The current standard of care is maximal safe debulking surgery, radiotherapy (RT) and adjuvant procarbazine, lomustine and vincristine (PCV) chemotherapy. Patients may experience long-term treatment-related toxicities, with RT linked to impairments of neurocognitive function (NCF) and health-related quality of life (HRQoL). With proton beam therapy (PBT), radiation dose falls off sharply beyond the target with reduced normal bra

Development and validation of diagnostic and prognostic prediction tools for dental caries in young children through prospective and cross-sectional observational studies: a protocol.

INTRODUCTION: Dental caries is the most common oral disease worldwide, affecting up to 90% of children globally. It can lead to pain, infection and impaired quality of life. Early prevention is a key strategy for reducing the prevalence of dental caries in young children. Valid and reliable diagnostic or prognostic tools that enable accurate individualised prediction of current or future dental caries are essential for facilitating personalised caries prevention and early intervention. However, no efficacious tools currently exist in early childhood-the optimal period for disease prevention. W

Individualised dosimetry for holmium-166 RE in patients with unresectable hepatocellular carcinoma; a multi-centre, interventional, non-randomised, non-comparative, open label, phase II study: RHEPaiR.

INTRODUCTION: Radioembolisation (RE) is gaining traction as a robust treatment option for patients with hepatocellular cancer (HCC) across all cancer stages. RE allows the delivery of targeted high-dose radiation directly to tumours, with relative sparing of the surrounding liver tissue. Traditionally, radiation has been delivered using 90Yttrium ([90Y]Y)-labelled microspheres, either glass or resin. The success of RE is dependent on the dose delivered to the tumour. When using [90Y]Y microspheres, dose prediction is calculated through a 99mTechnitium ([99mTc]Tc)-macroaggregated albumin (MAA)

Stepped wedge cluster randomised controlled trial to assess the impact of a decision support tool for physical restraint use in intensive care units (ARBORea Study): a study protocol.

INTRODUCTION: Intensive care units (ICUs) manage patients with or likely to have one or more life-threatening acute organ failures that might require the use of invasive supportive therapies. The use of physical restraint is frequent, with rates up to 50%, and usually initiated to maintain patient safety especially if the patient is agitated. Physical restraints have been associated with delirium, post-traumatic stress disorder and physical injuries while restricting patients' individual freedom. Moreover, the incidence of invasive therapeutic devices' self-removal by patients might not be dec

Systems Policy Analysis for Antimicrobial Resistance Targeted Action (SPAARTA): A Research Protocol.

BACKGROUND: The majority of countries (88%) have an Antimicrobial Resistance (AMR) National Action Plan (NAP V.1.0), but many remain unimplemented, and lack funding for interventions. Intervention selection requires a systematic approach to explain and predict progress. Looking beyond AMR is important to ensure the capture of systemic factors at the country level, which can impede or accelerate success. AIM: To provide innovative policy analysis to allow country comparison and refine targeted action, while developing and implementing NAPs (V.2.0). METHODS: Mixed-method multi-country case stu

Assessing the global variation in patient characteristics, management and short-term outcomes of spontaneous intracranial haemorrhage worldwide: a protocol for a global observational prospective multicentre study (the PLOT-ICH study).

INTRODUCTION: Stroke is the second leading cause of death worldwide, with the greatest burden in low- and middle-income countries (LMICs). Haemorrhagic stroke or spontaneous intracranial haemorrhage (sICH), including intraparenchymal haemorrhage (IPH) and subarachnoid haemorrhage (SAH), has the highest mortality and morbidity. Local management practices for haemorrhagic stroke vary greatly between geographical regions. The Planetary Outcomes after Intracranial Haemorrhage study aims to provide a global snapshot of the patient characteristics, processes of care and short-term outcomes of patien

Impact of an educational physiotherapy-yoga intervention on perceived stress in women treated with brachytherapy for cervical cancer: a randomised controlled mixed study protocol (KYOCOL).

INTRODUCTION: Cervical cancer is a major global health issue. The standard treatment for locally advanced disease involves radiochemotherapy followed by uterovaginal brachytherapy (UBT). UBT requires several days of hospitalisation and strict bed rest. UBT often induces pain, anxiety, stress, distress and a decline in physical capacity during and after treatment. Previous research suggests that non-pharmacological interventions, such as yoga, may help alleviate these issues. However, few studies have specifically evaluated their effectiveness in reducing stress during UBT. Furthermore, patient

How can we make postnatal information resources more accessible to women experiencing challenges accessing healthcare? Report of a co-production project.

BACKGROUND: There is a serious and persistent gap in perinatal morbidity and mortality rates for women from minoritised groups and for those living in deprived areas in the UK. If symptoms can be recognised in time this enables them to be treated more effectively. There are numerous information resources of varying quality detailing symptoms of potential postnatal serious symptoms that inform women's decisions to seek care. However, cross cultural understanding, linguistic constraints and multiple challenges that act in a mutually reinforcing way can impact women's access to information in a f

Carbon Footprint of Laparoscopic Appendicectomy: A Prospective Observational Study at a Tertiary Care NHS Trust Hospital.

Introduction Climate change is a significant global challenge as a result of rising greenhouse gas emissions. The National Health Service (NHS), as one of the biggest contributors to the United Kingdom's emissions, has a net-zero carbon emissions target. Operating theatres have a large resource burden in hospitals, and the laparoscopic appendicectomy (LA) is one of the most commonly performed procedures. As of yet, the laparoscopic appendicectomy has not been effectively carbon footprinted. Aims The objective of this study is to calculate the carbon footprint of the laparoscopic appendicectomy

Evaluating DOAC dipstick testing in the management of acute stroke: protocol for a multicentre, prospective, observational registry study.

INTRODUCTION: Direct oral anticoagulants (DOACs) are preferred over vitamin K antagonists for stroke prophylaxis in non-valvular atrial fibrillation. Yet, DOAC use is regarded as a contraindication for intravenous thrombolysis in acute ischaemic stroke. The stratification of patients into 'on-therapy' and 'off-therapy' categories based on their plasma DOAC concentrations is particularly crucial in the acute phase of stroke when decisions for thrombolysis or anticoagulation reversal are time-sensitive. The novel point-of-care DOAC dipstick assay (DOASENSE) rapidly assesses urine for clinically

Impact of an acute 1-month cannabidiol treatment on pain and inflammation after a long bone fracture: a triple-blind randomised, placebo-controlled, clinical trial protocol.

INTRODUCTION: Acute pain levels following orthopaedic injury (eg, fracture) are a predictor of the onset of chronic pain, which affects nearly 50% of fracture patients and impairs functional recovery. Among current pharmacological treatments for acute pain, non-steroidal anti-inflammatory drugs have been associated with delayed bone healing, while opioids inhibit effective bone remodelling, increase the risk of pseudarthrosis and carry a high risk of addiction. In light of this, the development of new pain treatments is essential. Cannabidiol (CBD), a non-addictive and non-psychotropic cannabi
