Left Ventricular Failure Despite Patent Coronary Arteries in Acute Type A Aortic Dissection.

Severe myocardial failure after acute type A aortic dissection is usually attributed to structural coronary malperfusion. We report catastrophic global left ventricular failure despite preserved epicardial coronary anatomy, successfully bridged with durable mechanical circulatory support. A 57-year-old man presented with acute type A dissection and cardiogenic shock with extensive precordial ST-segment elevation. Emergency total arch replacement with a frozen elephant trunk was performed. Intraoperative inspection and postoperative coronary angiography confirmed patent coronary ostia and arter

Hyperspectral imaging in the emergency department to characterize lower leg edema.

Interactions between light and biological tissue could reveal disease-related changes that could be advantageous for rapid diagnosis when applied to whole limbs. Here, we tested the extent to which visible/near-infra-red hyperspectral imaging (HSI) deployed in the emergency department could classify cellulitis, non-cellulitis edema, and healthy lower-leg tissue across skin pigmentation types. We collected HSI data of the lower leg from 83 emergency-room patients and analyzed the calibrated normalized spectra (400-1000 nm) using a machine-learning classification algorithm, evaluated by cross-va

Adnexal torsion due to teratoma in a young adolescent.

BACKGROUND: Adnexal torsion is a gynaecological emergency in which twisting of the ovary and/or fallopian tube around the vascular pedicle leads to impaired perfusion and ischemia. Non-specific symptoms in children and adolescents can delay diagnosis. CASE PRESENTATION: We present the case of a teenage girl admitted to hospital with lower abdominal pain and a palpable mass. She had a history of chronic constipation and had not had a bowel movement for 5 days. Initial ultrasound evaluation of the mass by gynaecologists and radiologists was interpreted as being of extragenital origin. Her mild

Photoacoustic microscopy for studying mechano-transduction response in resistance vessels.

Cardiovascular diseases are on the rise, presenting a significant global health challenge. The development of methods enabling the detection of alterations in vascular networks is critical for the early diagnosis and treatment of cardiovascular diseases, including peripheral arterial disease, stroke, and hypertension. Here, we use photoacoustic microscopy (PAM), a non-invasive imaging technique, to monitor morphological changes within the skin vessels of chronically hypertensive mice deficient in the mechanosensitive channel Piezo1 in endothelial cells (Piezo1 EC-KO). We show that, compared to

B-flow imaging in abdominal ultrasound: a pictorial essay.

B-Flow technology represents a significant advancement in vascular ultrasound imaging, offering high-resolution visualization of blood flow without the limitations inherent in conventional Doppler techniques. By employing Coded Excitation and Tissue-Blood Equalization (TBE) technologies, B-Flow enhances the detection of low-velocity and microvascular blood flow while minimizing aliasing artifacts and angle dependency. This innovative technique proves especially useful in abdominal ultrasound, enabling the accurate assessment of vascularization in organs such as the liver, kidneys, pancreas, sp

Thirty years of intravenous thrombolysis for acute ischemic stroke: Historical perspectives, evidence from randomized trials, and emerging directions.

Intravenous thrombolysis (IVT) remains a cornerstone in the management of acute ischemic stroke (AIS). Since the landmark NINDS and ECASS III trials, numerous randomized controlled trials (RCTs) have expanded IVT indications beyond the initial 4.5-h window. Advanced imaging modalities, including perfusion-based and diffusion/fluid-attenuated inversion recovery (FLAIR) mismatch, have enabled individualized patient selection, leading to positive outcomes in trials such as WAKE-UP, EXTEND, and EXPECTS. Despite these advances, key challenges remain. Patients with large ischemic cores, those on dir

[Main Trunk Interposition for Adult Anomalous Origin of the Left Coronary Artery from the Pulmonary Artery Using a Great Saphenous Vein:Report of a Case].

A 54-year-old woman was incidentally diagnosed with anomalous origin of the left coronary artery from the pulmonary artery( ALCAPA) during a pre-treatment screening computed tomography( CT) scan for alopecia areata. She had no cardiac symptoms, but cardiac magnetic resonance imaging( MRI) revealed late gadolinium enhancement (LGE) in the mid-anterior wall of the left ventricle, indicating myocardial ischemia. Median sternotomy was performed, with the left main trunk (LMT) was transected from the main pulmonary artery. Initially, reconstruction with an expanded polytetrafluoroethylene(ePTFE) gr

Epicardial Adipose Tissue Thickness and Preserved Ejection Fraction Heart Failure.

PURPOSE OF THE REVIEW: Preserved ejection fraction heart failure and obesity frequently coexist. Whether obesity plays a consistent role in the pathogenesis of preserved ejection fraction heart failure is unclear. Accumulation of visceral adiposity underlies the pathogenic aftermaths of obesity. However, visceral adiposity imaging is assessed by computed tomography or magnetic resonance and thus not routinely available. In contrast, epicardial adiposity thickness is assessed by echocardiography and thus routinely available. We review the rationale for assessing epicardial adiposity thickness i

Recognition of Significantly Delayed Spinal Cord Ischemia Following Thoracic Endovascular Aortic Repair: A Case Report and Review of the Literature.

Spinal cord ischemia (SCI) is an uncommon but serious complication of thoracic endovascular aortic repair (TEVAR). SCI after TEVAR is thought to result from decreased segmental blood supply to an important network of collateral blood flow in the spinal cord. Little is known about the prevalence and optimal treatment of SCI that occurs beyond the periprocedural period. We report a case of delayed SCI in a 67-year-old patient who underwent TEVAR. The patient presented almost two years after TEVAR with acute paraplegia preceded by pre-syncope. The delayed SCI was likely triggered by pre-syncope,

Automated, Transferable, and Ethanol-Free Radiosynthesis of [11C]Butanol.

Cerebral blood flow and blood-brain barrier permeability assessment are crucial hemodynamic parameters to measure under neurological conditions. In conjunction with positron emission tomography (PET), oxygen-15-labeled water has emerged as a gold standard for measuring cerebral perfusion; however, at higher flow rates, [15O]water extraction becomes nonlinear. In such a scenario, freely diffusible [11C]butanol can provide a truer estimate. Radiosyntheses of [11C]butanol reported to date are protracted, are not automated, or require ethanol in the final formulation. By using a flow-based, captiv

Utilisation of a balloon-mounted coronary stent in the treatment of a patient with petrous carotid artery dissection.

A man in his 50s presented with acute left-sided weakness, facial drooping and slurred speech, suggestive of a major stroke. Imaging revealed a complete blockage in the right internal carotid artery (ICA) due to dissection. Perfusion studies showed a significant area at risk of infarction. Attempted navigation of standard carotid stents failed due to the tortuosity of the ICA. A balloon-mounted coronary stent was successfully deployed in the petrous ICA segment, restoring blood flow. A follow-up MRI confirmed stent patency, and the patient achieved complete recovery with return to baseline fun

Interpretation and clinical utility of indocyanine-green fluorescence imaging (IFI) in robot-assisted anorectal-function saving operations (ASOs): A propensity-score matched analysis of 872 prospectively enroled patients undergoing IFI.

BACKGROUND: This study suggested a novel physiological evaluation of indocyanine-green fluorescence imaging (IFI), and its utility associated with anastomotic leakage/stricture (AL/AS) and prognosis. METHODS: This study focussed on the utility of IFI, comparing IFI + versus IFI- groups (n = 878 vs. 339), optimised by propensity-score matching. After intravenous injection of indocyanine green, maximal perfusion was separately assessed at the vasa recta (VR) and colonic wall (CW), by determining intensities at the VR (VRI) and CW (CWI) and respective time. RESULTS: Although IFI did not signifi

Occipital artery bypass importance in unsuitable superficial temporal artery: Two case reports.

BACKGROUND: Superficial temporal artery-middle cerebral artery (STA-MCA) bypass helps treat cerebral ischemia. However, the STA is not available for bypass in some conditions. Therefore, with some technical tips, the authors introduced a bypass technique using the occipital artery (OA). CASE SUMMARY: Two female patients complained of hemiparesis. Brain magnetic resonance imaging (MRI) indicated contralateral infarction from the MCA steno-occlusion. On Diamox single photon emission computed tomography or perfusion MRI, the contralateral front parietotemporal reserve was diminished. On transfem

Advanced Neuromonitoring Modalities on the Horizon: Detection and Management of Acute Brain Injury in Children.

Timely detection and monitoring of acute brain injury in children is essential to mitigate causes of injury and prevent secondary insults. Increasing survival in critically ill children has emphasized the importance of neuroprotective management strategies for long-term quality of life. In emergent and critical care settings, traditional neuroimaging modalities, such as computed tomography and magnetic resonance imaging (MRI), remain frontline diagnostic techniques to detect acute brain injury. Although detection of structural and anatomical abnormalities remains crucial, advanced MRI sequence

Hematopoietic stem-cell gene therapy is associated with restored white matter microvascular function in cerebral adrenoleukodystrophy.

Blood-brain barrier disruption marks the onset of cerebral adrenoleukodystrophy (CALD), a devastating cerebral demyelinating disease caused by loss of ABCD1 gene function. The underlying mechanism are not well understood, but evidence suggests that microvascular dysfunction is involved. We analyzed cerebral perfusion imaging in boys with CALD treated with autologous hematopoietic stem-cells transduced with the Lenti-D lentiviral vector that contains ABCD1 cDNA as part of a single group, open-label phase 2-3 safety and efficacy study (NCT01896102) and patients treated with allogeneic hematopoie
