BACKGROUND: Patients with lower extremity arterial disease (LEAD) undergoing hemodialysis for end-stage renal disease (ESRD) often experience reduced limb muscle strength and impaired circulation. Conventional exercise is often unfeasible due to pain or immobility. Electrical muscle stimulation (EMS) provides a non-weight-bearing alternative for improving muscular and vascular function. This pilot study evaluated the effects of EMS on lower limb muscle strength and peripheral blood flow in older adult patients with diabetes with LEAD undergoing hemodialysis.
METHODS: Six patients (mean age: 7

Non-traumatic osteonecrosis of the femoral head (NONFH) is a disabling disease mainly caused by glucocorticoid use and alcohol abuse, characterized by local impairment of blood supply and limited bone repair; however, the composition and regulatory mechanisms of specialized endothelial cells (ECs) in its microenvironment remain unclear. In this study, we used single-cell RNA sequencing (scRNA-seq) to identify four EC subsets, including type L, type H, type R, and arterial ECs. Type H and type R ECs were primarily enriched in cell adhesion and angiogenesis pathways and represented key subsets t

Background: A small proportion of patients in need of transcatheter aortic valve replacement (TAVR) are not suitable for the transfemoral approach due to peripheral artery disease. Alternative TAVR approaches are associated with short- and long-term hazards. A novel technique of caval-aortic (transcaval) access for TAVR has been utilized as an alternative access technique. Aim: To compare safety and efficacy of transcaval access as compared to other alternative access (axillary or apical) for TAVR. Methods: A single-center, retrospective analysis of consecutive patients undergoing alternative

Left ventricular thrombus (LVT) is a well-known complication of myocardial infarction (MI) leading to significant morbidity and mortality. LVT can also lead to systemic thromboembolic events causing threatening limb ischemia. We report a rare case of critical bilateral limb ischemia that resulted from peripheral embolization of LVT post MI, which was managed successfully by emergent surgical intervention and anticoagulation. A 74-year-old male with a medical history of hypertension, diabetes, hyperlipidemia and coronary artery disease status post stenting of the left anterior descending and le

BACKGROUND: Altered blood flow occurs in patients with low extremity peripheral artery disease (LE-PAD). LE-PAD is mostly associated with coronary artery disease (CAD). Adenosine is an endogenous nucleoside that affects both coronary and limb artery blood flow, mostly via the adenosine A2A receptor (A2AR). We evaluated A2AR expression and function in peripheral blood mononuclear cells (PBMCs) and the femoral artery tissues of patients with LE-PAD.
METHODS: Artery tissues and PBMCs were sampled in 24 patients with intermittent claudication, and compared with PBMCs in 24 healthy subjects. Expre

Antiplatelet therapy reduces the risks for cardiovascular morbidity and mortality in patients with atherosclerotic disease, and it is also beneficial in managing peripheral arterial disease (PAD). These agents work through various therapeutic pathways to achieve antithrombotic effects. Although single- or two-drug regimens have been deployed to prevent vascular events, approximately 10% of the patients with acute coronary syndrome remain at risk for recurrent thrombotic events and may need a more aggressive preventative strategy. Vorapaxar offers a unique mechanism for platelet inhibition via

Cytopenias are frequently observed in patients with cirrhosis and are associated with increased morbidity. In particular, thrombocytopenia can impact routine care of patients with cirrhosis by potentially postponing or interfering with diagnostic and therapeutic procedures including liver biopsy and medically indicated or elective surgery. The pathogenesis of cytopenias in cirrhosis remains largely unknown. Historically, the concept of hypersplenism has long been associated with the cirrhosis-related hematological disorders but was never proven. On the other hand, intestinal bacterial overgrow

BACKGROUND: The prevalence of occlusive peripheral vascular disease in the superficial femoral artery approaches 20% in patients over the age of 60. Symptomatic peripheral arterial disease (intermittent claudication) is present in 3% to 7% of patients over the age of 60. The attraction of minimally invasive, percutaneous endovascular angioplasty (PTA) for this large number of patients has resulted in multiple trials of new PTA +/- stenting devices. The purpose of this report is to determine whether trials should include controlled patients who have optimal medical management including supervis

Interactions between complementary receptors on leukocytes and endothelial cells play a central role in regulating extravasation from the blood and thereby affect both normal and pathologic inflammatory responses. CD44 on lymphocytes that has been "activated" to bind its principal ligand hyaluronate (HA) on endothelium can mediate the primary adhesion (rolling) of lymphocytes to vascular endothelial cells under conditions of physiologic shear stress, and this interaction is used for activated T cell extravasation into an inflamed site in vivo in mice (DeGrendele, H.C., P. INTRODUCTION: Wound complications and amputations are common after lower extremity burn injury in patients with diabetes mellitus (DM). Endovascular revascularization has the potential to promote wound healing and limb salvage in this patient population but has yet to be evaluated. This study describes a burn center's experience incorporating endovascular revascularization into the acute management of lower extremity burns.
METHODS: Patients admitted to a regional burn center from August 2021 to August 2023 with a lower extremity burn, DM, and an abnormal pulse or noninvasive vascular exam wh

BACKGROUND: Identifying patients with diabetes at increased risk of chronic kidney disease (CKD) is essential to prevent/slow the progression to end-stage renal disease (ESRD). CKD and diabetic peripheral neuropathy (DPN) share common mechanisms. Hence, we aimed to examine the relationship between foot insensitivity and CKD in patients with Type 2 diabetes.
METHODS: A prospective observational cohort study in adults with Type 2 diabetes. Patients with ESRD were excluded. Foot insensitivity was assessed using the 10-g monofilament test. Renal function was assessed using estimated glomerular fi

Abnormal bone metabolism is a recognized complication of end-stage renal disease, but fracture risk following renal transplantation has not been well quantified. We followed the 86 Olmsted County, Minnesota, residents who underwent initial renal transplantation in 1965-1995 for 911 person-years (median, 10.6 years per subject) in a retrospective cohort study. Fractures, and possible risk factors, were assessed through review of each subject's complete community medical records. Altogether, 117 fractures were observed during follow-up extending to 33 years. The cumulative incidence of any fract

Left ventricle dysfunction and comorbidities are responsible for a large number of complications after CABG. OPCAB could be an interesting alternative for very high-risk patients. Patients were included if EuroSCORE >9, or with at least two of the following criteria, severe LV dysfunction, recent myocardial infarction (MI), terminal renal failure, lung dysfunction, PVD, BMI>30. Patients were operated using the Octopus (Medtronic) system. One hundred and twenty patients, mean age 68+/-10 years, 72% male, were operated. Mean EuroSCORE was 10.2+/-5.3, LV function 36.79+/-11.3%, recent MI 57%, ren

AIMS: Coronary artery disease (CAD) is the leading cause of death in patients with Type 2 diabetes and is often asymptomatic. Silent myocardial ischaemia (SMI) is frequent in diabetic subjects and is responsible for a late diagnosis of CAD; its early detection is important. There are some data about the prevalence of SMI in Type 2 diabetic patients at high risk for cardiovascular disease, while no data are available in subjects at the onset of diabetes without other cardiovascular risk factors.
METHODS: We screened 274 consecutive patients (mean age 64.3 +/- 8.4 years, 66% male) at the time o

This report reviews the present status of cardiovascular surgery in West Africa and highlights some of the constraints of development in this field.Rheumatic heart disease is still endemic in the tropics, where it constitutes about 20 percent of all cases of cardiovascular disease (CVD) in Nigeria. Endomyocardial fibrosis is a disease of unknown etiology accounting for 10 to 20 percent of cases. Purulent pericarditis is a common complication of pyomyositis and osteomyelitis found in 5 percent of patients. Chronic constrictive pericarditis is a sequela of infective pericarditis found in 5 perce

Peripheral artery disease (PAD) commonly affects the lower extremity and occurs due to the narrowing or blockage of vessels carrying blood from the heart to the legs. This review aims to examine racial and ethnic disparities in the diagnosis and management of PAD among adults in the United States by summarizing differences in diagnostic evaluation, treatment utilization, and management outcomes. A search was done from inception to the 18th of November 2025 across PubMed and Google Scholar, and 1,372 articles were generated. Eight articles were included for the final analysis after applying the

PURPOSE: A hallmark of the IMPDH1-related rod-cone dystrophy is the highly variable expressivity. Disease onset, severity and progression are variant-dependent due to specific affected photoreceptor mechanisms, underscoring the importance of genotype-phenotype correlations. We provide a detailed clinical characterization of a patient with rod-cone dystrophy caused by a rare de novo missense IMPDH1 variant.

Rheumatoid vasculitis (RV) is a rare, severe extra-articular manifestation of rheumatoid arthritis (RA), typically occurring after long-standing erosive disease. Advances in RA therapy have reduced its incidence; however, RV can occasionally present early in high-risk patients. We report a man in his thirties from Central India with a 3-year history of inadequately treated seropositive RA, who presented with progressive polyarthritis, neuropathy, and distal ischemic changes. Over five months, he developed worsening joint pain and swelling, followed by tingling, numbness, and subsequently dry g
