[Acute abdomen secondary to pyonephrosis. Perinephritic abscess with fistula to peritoneal cavity].

OBJECTIVE: To describe a case of acute abdomen arising from an underlying urological condition. METHODS/RESULTS: Herein we describe a patient with acute abdomen arising from a pyonephrotic kidney with fistulization to the peritoneal cavity. The clinical manifestations disappeared following nephrectomy by the anterior approach and drainage of the intraperitoneal cavity. CONCLUSION: Although infrequent, it should be taken into account that peritoneal abscess and/or pyonephrosis can cause acute abdomen when they fistulize to the peritoneal cavity.

Cocaine inhibits production of murine hepatitis virus by peritoneal macrophages in vitro.

Our previous studies have demonstrated a number of effects of cocaine on macrophage (M phi) functions. The present studies were initiated to ascertain the effects of cocaine on virus production in vitro. C57BL/6 mice were injected intraperitoneally with thioglycollate broth, and the peritoneal M phi collected 4 days later were cultured in 96-well microtiter plates as continuous monolayers. Various concentrations of cocaine were incubated with M phi for up to 5 days. Mouse hepatitis virus (MHV) was added to these cultures, which was followed by a methyl cellulose overlay. Cocaine caused a dose-

Analysis of risk factors for omental herniation associated with removal of peritoneal catheters.

BACKGROUND: Peritoneal catheters may be used routinely in children undergoing cardiac surgery. Removal of the catheter is often complicated by omental herniation, which can cause intraperitoneal bleeding, peritonitis, and bowel obstruction. Instillation of saline into the catheter before removal is used in some institutions as a preventive measure, but the practice has never been investigated. OBJECTIVE: To determine the effectiveness of instilling saline into peritoneal catheters before their removal in reducing the occurrence of omental herniation and to determine risk factors for omental h

Intussusception: trapped peritoneal fluid detected with US--relationship to reducibility and ischemia.

PURPOSE: To determine the frequency and meaning of fluid inside the intussusception at ultrasound (US) and its relationship to irreducibility and ischemia. MATERIALS AND METHODS: US enabled the diagnosis of intussusception in 145 cases. Shape and axial diameters of the area of fluid were determined. US-guided hydrostatic reduction was attempted in 144 cases. RESULTS: Fluid was present in the intussusception in 20 cases (14%) and appeared on axial images as an anechoic crescent between both serosal layers of the enfolded and everted intussusceptum. No cystic structural anomaly was detected at

Hydralazine inhibits human peritoneal mesothelial cell proliferation and collagen synthesis.

The integrity of the mesothelial layer is essential for both defence and solute transport in continuous ambulatory peritoneal dialysis (CAPD). The human peritoneal mesothelial cell (HPMC) culture has been shown to be a very useful tool to study the peritoneal mesothelial stem cell behaviour. We investigated whether hydralazine, an antihypertensive agent frequently used, might affect HPMC growth and collagen synthesis. HPMCs were cultured from specimens of human omentum by enzymatic disaggregation of omentum. HPMC growth was evaluated by modified methyltetrazolium (MTT) assay. Cell viability wa

Direct activation of human peritoneal mesothelial cells by heat-killed microorganisms.

OBJECTIVE: The aim of the study was to determine if human peritoneal mesothelial cells (HPMCs) can be activated directly by bacterial products contained in preparations of heat-killed Escherichia coli and staphylococci. SUMMARY BACKGROUND DATA: It has been shown recently that cytokine-activated HPMCs produce the inflammatory mediators, interleukin-1, interleukin-6, interleukin-8, and macrophage chemotactic protein-1. Studies concerning the effects of bacterial products on HPMCs are scarce and have not yielded conclusive results. METHODS: Growth-arrested HPMC monolayers were prepared from cel

Losses, maintenance and recruitment of patients in a peritoneal dialysis home training program.

The St. Peter Hospital Kidney Dialysis Center began a peritoneal dialysis home training program in 1988, and demonstrated steady growth until the end of its sixth year at which time the census plummeted. Peritoneal dialysis staff sought to determine a cause for the reduction in census by evaluating the reasons for drop-out. They concluded that drop-out rates were not unexpected or unusual and that maintenance and referrals are influenced by: (a) patient predialysis education and (b) continual conversation and collaboration between the nephrologists and peritoneal dialysis home training staff.

Cokeromyces recurvatus isolated from pleural and peritoneal fluid: case report.

The zygomycetous mold Cokeromyces recurvatus was isolated from the pleural and peritoneal fluids of a 64-year-old man with a history of peptic ulcer disease and alcohol abuse. The patient presented to the hospital with severe abdominal pain and a ruptured duodenal ulcer. This is one of the few times that C.recurvatus has been isolated from a human source, and the organism may have contributed to the death of this patient.

Entrapment of the appendix and the fallopian tube in peritoneal dialysis catheters in two children.

Continuous ambulatory peritoneal dialysis (CAPD) has revolutionized the management of renal failure in children. However, CAPD catheter malfunction is not uncommon, the main complications being catheter-related infection and catheter-related obstruction. Herein the authors report both of the above complications, attributable to unique and previously unreported causes. One patient had appendicitis and peritonitis secondary to catheter entrapment of the appendix; the other had an obstructed catheter secondary to entrapment of the right fallopian tube within its lumen. To avoid potentially seriou

[Extrapulmonary tuberculosis: 2 cases with hepatic, splenic, peritoneal and cerebral involvement].

Two cases of unusual extrapulmonary tuberculosis are presented. One patient was suffering of a pulmonary tuberculosis involving the brain, liver, spleen and peritoneum, with headaches, ascites, weight loss and night sweats. The other patient had lymph nodes and nodular liver tuberculosis and complained of fever, right upper quadrant pain, anorexia and weight loss. This tuberculosis form is extremely rare; only 23 cases were reported between 1950 and 1990. Furthermore, a drug-induced hepatitis developed in a liver already damaged by the tuberculosis and a chronic active C hepatitis. These two c

Intestinal peritoneal sling as an adjunct to radical pelvic operation and pelvic irradiation.

BACKGROUND: Pelvic peritoneal surfaces are often denuded extensively during radical pelvic operations, providing raw areas for small bowel adherence and potential obstruction. This hazard is compounded in patients who receive whole pelvic adjuvant irradiation. Omental and synthetic slings or redundant sigmoid colon have been incompletely effective in excluding small bowel from the pelvic area. Furthermore, these exclusion procedures have been associated with significant complications. STUDY DESIGN: Anterior parietal peritoneal flaps were created in ten patients who were undergoing radical hys

Changes in biocompatibility of dialysis fluid during its dwell in the peritoneal cavity.

OBJECTIVE: To evaluate the changes in biocompatibility of peritoneal dialysis solutions during intraperitoneal dwell. DESIGN: We studied the effect of the drained dialysates at time 0 and after 30, 60, 120, 240, and 360 minutes of intraperitoneal dwell on the growth of peritoneal mesothelial cells and fibroblasts and the synthesis of proteins by these cells. On one day the patients were dialyzed with glucose-based Dianeal and on alternate days with an amino acid-containing solution based on Travasol. PATIENTS: Dialysates were collected from 4 patients during continuous ambulatory peritoneal

Effect of glucose on TGF-beta 1 expression in peritoneal mesothelial cells.

The peritoneum's response to peritoneal dialysis is a complex and poorly understood process that can result in fibrosis. The transforming growth factor beta(TGF-beta 1) plays a central role in regulating tissue repair and remodeling after injury. To determine the role of mesothelial cells during peritoneal dialysis in the alteration of the peritoneum, we studied the effect of glucose on TGF-beta 1 expression by mesothelial cells. Human mesothelial cells were cultured in serum-free DMEM with different glucose concentrations for 48 hours. TGF-beta 1 messenger (mRNA) was measured by slot-blot hyb

Effect of intraperitoneally administered agents on human peritoneal mesothelial cell growth.

During continuous ambulatory peritoneal dialysis, the peritoneal mesothelial cell layer is under continuous sloughing and regeneration processes. Agents unfavorable for mesothelial cell growth may be harmful to the peritoneal membrane. We investigated whether frequent intraperitoneally instilled agents affect mesothelial cell growth. Peritoneal mesothelial cells were cultured from the human omentum. The proliferation was assessed by using a modified methyltetrazolium assay and confirmed by Coulter cell counting. The results showed that a high-glucose medium and heparin inhibited mesothelial ce
