A Novel Nomogram for Predicting Warfarin-Related Bleeding: A Retrospective Cohort Study.

Warfarin is a widely used anticoagulant, and bleeding complications are the main reason why patients discontinue the drug. Currently, there is no nomogram model for warfarin-associated bleeding risk. The aim of this study was to develop a risk-prediction nomogram model for warfarin-related major and clinically relevant non-major (CRNM) bleeding. A total of 280 heart disease outpatients taking warfarin were enrolled, 42 of whom experienced major or CRNM bleeding at the one-year follow-up. The Least Absolute Shrinkage and Selection Operator regression model was employed to identify potential pre

Machine learning study using 2020 SDHS data to determine poverty determinants in Somalia.

Extensive research has been conducted on poverty in developing countries using conventional regression analysis, which has limited prediction capability. This study aims to address this gap by applying advanced machine learning (ML) methods to predict poverty in Somalia. Utilizing data from the first-ever 2020 Somalia Demographic and Health Survey (SDHS), a cross-sectional study design is considered. ML methods, including random forest (RF), decision tree (DT), support vector machine (SVM), and logistic regression, are tested and applied using R software version 4.1.2, while conventional metho

Clinical Evaluation of the HER2 Extracellular Domain in Breast Cancer Patients by Herceptin-Encapsulated Gold Nanocluster Probe-Based Electrochemiluminescence Immunoassay.

The extracellular domain (ECD) of human epidermal growth factor receptor 2 (HER2) serves as a promising biomarker for the early diagnosis and treatment of breast cancer (BC). However, due to the heterogeneity of tumors, assessing HER2 status through a core needle biopsy presents significant challenges. In this study, we propose a facile and high-performance electrochemiluminescence immunoassay (ECLIA) platform utilizing a herceptin-encapsulated gold nanoclusters (HER-AuNCs)/(diisopropylamino)ethanol (DIPEA-OH) ECL system for the clinical evaluation of HER2 ECD in BC patients. The two-in-one HE

Inpatient opioid withdrawal: a qualitative study of the patient perspective.

Opioid withdrawal is common among hospitalized patients. Those with substance use disorders exhibit higher rates of patient-directed discharge. The literature lacks information regarding the patient perspective on opioid withdrawal in the hospital setting. In this study, we aimed to capture the patient-reported experience of opioid withdrawal during hospitalization and its impact on the desire to continue treatment for opioid use disorder after discharge. We performed a single-center qualitative study involving semi-structured interviews of hospitalized patients with opioid use disorder (OUD)

Kinematic Assessment of Upper Limb Movements Using the ArmeoPower Robotic Exoskeleton.

After a neurological injury, neurorehabilitation aims to restore sensorimotor function of patients. Technological assessments can provide high-quality data on a patient's performance and support clinical decision making towards the most appropriate therapy. In this study, the ArmeoPower, a robotic exoskeleton for the upper extremities, was used to assess 12 neurological patients and 31 non-disabled participants performing various standardized single joint and frontal plane game-like exercises. From the collected data, kinematic metrics (End-Point Error, Hand-Path Ratio, reaction time, stabilit

Open Reduction Internal Fixation vs Primary Arthrodesis for Lisfranc Fracture-Dislocations: A Cost Analysis.

BACKGROUND: Lisfranc fracture-dislocation is an uncommon but serious injury that currently lacks universal consensus on optimal operative treatment. Two common fixation methods are open reduction and internal fixation (ORIF) and primary arthrodesis (PA). The objective of this study is to analyze the cost difference between ORIF and PA of Lisfranc injuries, along with the contribution of medical services to overall costs. METHODS: This was a retrospective cost analysis of the MarketScan database from 2010 to 2020. MarketScan is an insurance and commercial claims database that integrates deiden

Methylenetetrahydrofolate reductase (MTHFR) C677T and A1298C polymorphisms in Turkish postmenopausal women with osteoporosis.

Osteoporosis is a common age-related skeletal disease, characterized by changes in the microarchitectural structure of bone tissue and decreased bone mass, especially affecting postmenopausal women. Genetic and environmental factors affecting bone metabolism play a role in the development of osteoporosis. Methylenetetrahydrofolate reductase (MTHFR) is an important enzyme involved in the conversion of homocysteine to methionine. Genetic variations in the MTHFR gene lead to impaired function or inactivation of this enzyme. A decrease in MTHFR enzyme activity and an increase in homocysteine level

Genome-Wide Analysis of Exertional Rhabdomyolysis in Sickle Cell Trait Positive African Americans.

Sickle cell trait (SCT), although generally a benign carrier state of hemoglobin S (HbAS), is a risk factor for exertional rhabdomyolysis (ERM), a rare but potentially fatal consequence of highly intense physical exercise, particularly among active-duty military personnel and high-performance athletes. The association between SCT and ERM is poorly understood. The objective of this study was to elucidate the genetic basis of ERM in an SCT-positive African American cohort. SCT-positive African Americans with a personal history of ERM (cases, n = 30) and without history of ERM (controls, n = 53)

Zero-covid advocacy during the COVID-19 pandemic: a case study of views on Twitter/X.

During the COVID-19 pandemic, many advocacy groups and individuals criticized governments on social media for doing either too much or too little to mitigate the pandemic. In this article, we review advocacy for COVID-19 elimination or "zero-covid" on the social media platform X (Twitter). We present a thematic analysis of tweets by 20 influential co-signatories of the World Health Network letter on ten themes, covering six topics of science and mitigation (zero-covid, epidemiological data on variants, long-term post-acute sequelae (Long COVID), vaccines, schools and children, views on monkeyp

Craniomaxillofacial Bone Segmentation and Landmark Detection Using Semantic Segmentation Networks and an Unbiased Heatmap.

Craniomaxillofacial (CMF) surgery always relies on accurate preoperative planning to assist surgeons, and automatically generating bone structures and digitizing landmarks for CMF preoperative planning is crucial. Since the soft and hard tissues of the CMF regions possess complicated attachment, segmenting the CMF bones and detecting the CMF landmarks are challenging problems. In this study, we proposed a semantic segmentation network to segment the maxilla, mandible, zygoma, zygomatic arch, and frontal bones. Then, we obtained the minimum bounding box around the CMF bones. After cropping, we

The Use of Wearable Activity Monitors to Measure Upper Limb Physical Activity After Axillary Lymph Node Dissection and Sentinel Lymph Node Biopsy.

BACKGROUND: We suspect that morbidity from both sentinel lymph node biopsy (SLNB) and axillary lymph node dissection (ALND) has been inadequately evaluated to date. Current methodologies are subjective and susceptible to bias. Objective assessment using wearable activity monitors (WAMs) would allow quantitative analysis of recovery by measuring physical activity (PA) and could provide evidence for axillary de-escalation. PATIENTS AND METHODS: A prospective, single center, observational study was conducted from February 2020 to May 2022. Consecutive patients undergoing breast and/or reconstruc

Huntington Study Group's Neuropsychology Working Group: Implementing Non-Motor Diagnostic Criteria.

BACKGROUND: The clinical diagnosis of manifest Huntington's disease (HD) relies on a high level of clinical confidence (99% confidence) of HD-consistent motor signs. Longitudinal data have reliably identified cognitive and behavioral dysfunction predating clinical motor diagnosis by up to 15 years. Reliance on motor signs to establish a diagnosis of HD increases risk of early misdiagnosis or delayed diagnosis. Clinical neuropsychologists are uniquely positioned to advise on the clinical application of the Movement Disorder Society Task Force's recently proposed non-motor diagnostic criteria fo

Patient feedback and psychosocial outcomes of deep brain stimulation in people with obsessive-compulsive disorder.

Severe and refractory psychiatric patients can experience complex and profound changes in symptomology, functioning and well-being from deep brain stimulation (DBS) therapy. Currently, the efficacy of DBS is assessed by clinician rated scales of primary symptoms, yet this does not capture the multitude of DBS mediated changes or represent the patient perspective. We aimed to elucidate the patient perspective in psychiatric DBS application by investigating 1) symptomatic, and 2) psychosocial changes, 3) therapeutic expectations and satisfaction, 4) decision-making capacity, and 5) clinical care
