Diclofenac sodium suppositories 150-200 mg day-1 were compared with placebo in a double-blind study during the first 3 days after uvulopalatopharyngoplasty in 40 patients with habitual snoring or obstructive sleep apnoea syndrome. Consumption of rescue analgesics (paracetamol suppositories) and pain assessed by a visual analogue scale were significantly less in the diclofenac group. Bleeding time (modified Ivy's test) and reported side effects did not differ between the two groups.

We report a method of measuring the static pressure-volume properties of the upper airway in unanesthetised humans. Although uvulopalatopharyngoplasty relieves obstructive sleep apnea in the majority of patients, the factors that determine a successful response are not well defined. Nasopharyngoscopy with videorecording was used as a visual feedback therapy tool to establish and promote consistent closure of the velopharyngeal ports during connected speech after pharyngeal flap surgery. Of the three adults in this report, two achieved consistent closure during connected discourse and have been discharged from therapy; the third demonstrated improved velopharyngeal closure but elected not to continue therapy. 32 patients with habitual snoring and 2 patients with obstructive sleep apnea syndrome underwent nasal and/or palatopharyngeal surgery. 12 patients with combined nasal septoplasty, submucous resection of the inferior turbinate bones and palatopharyngoplasty gained total relief from snoring. Reconstruction in head and neck surgery has been greatly advanced with the use of the pectoralis major and trapezius myocutaneous flaps. Most surgical defects can be repaired with one of these flaps alone, or in conjunction with cutaneous flaps. This paper reports the results of reconstructions of difficult wounds in the head and neck area with musculocutaneous flaps in 20 patients. Twelve patients had reconstructions following cranial and orbital resections, and eight patients had reconstructions of the pharyngoesophagus. There were four wound complications, all of which healed without further surgical procedures. A barium pharyngogram was incorporated into the multiview videofluoroscopic evaluation of the velopharyngeal portal in 195 subjects in order to assess its usefulness in demonstrating competence or incompetence. Reflux of barium was always associated with a velopharyngeal gap and/or hypernasality; however, the absence of reflux was not similarly diagnostic. The pharyngogram proved to be useful in confirming the presence of a palatal or pharyngeal-flap fistula. While it is helpful in assessing velopharyngeal closure in selected cases, it should not be used in the absence of other information.

A comparison is made of the preoperative and postoperative speech evaluations of 15 selected subjects who had pharyngeal flap operations combined with palatal pushback. Postoperatively, 13 of the 15 patients (86 percent) showed no abnormal nasal emission and no evidence of significant hypernasality during word production. In this preliminary study, the use of polyacrylamide gel electrophoresis as an aid in the characterization of Corynebacterium diphtheriae was evaluated and a standardized method was developed.
