A Cross-Sectional Latent Class Analysis of 3 Distinct Satisfaction Subgroups Among Non-Local Patients in VIP Outpatient.

To meet the personalized needs of patients, VIP (Very Important Person) outpatient clinics, operating as a private practice, are an important option for non-local patients who often lack familiarity with medical procedures and seek efficient care within a limited timeframe. This study aimed to address 2 core questions: what are the distinct satisfaction classes among non-local patients, and what factors determine membership in these patterns? Accordingly, it sought to identify these latent subgroups and analyze their determinants. A cross-sectional study was conducted among 1961 non-local pati

Implementing Respiratory Illness Care Pathways in General Hospitals: A Cluster Randomized Trial.

BACKGROUND AND OBJECTIVES: A prior study at a freestanding children's hospital tested simultaneous implementation of multiple clinical pathways and demonstrated improved guideline adherence and patient health outcomes. This high-efficiency approach has not been studied in general hospitals, which face unique implementation barriers. Our objective was to determine the effectiveness of a multi-condition pathway intervention in general hospitals. METHODS: A pragmatic, cluster-randomized trial (1:1 allocation to intervention versus waitlist control) conducted from July 2022 to June 2024 in genera

Intersecting Risk Factors Associated With High Syphilis Seroprevalence Among a Street-Involved Population in Canada.

BACKGROUND: Syphilis has reemerged as a global public health concern. In Ontario, Canada's most populous province, a 340% increase in infectious syphilis cases was observed between 2013 and 2023. This surge was accompanied by a demographic shift, with women emerging as the fastest-growing at-risk group. We examined intersecting risk factors associated with syphilis seropositivity among a street-involved population. METHODS: Data were collected from the Syphilis Point of Care Rapid Test and Immediate Treatment Evaluation (SPRITE) study-an outreach model of care implemented by 8 public health u

Rapid identification and management of stress-induced cardiomyopathy using POCUS after strangulation: A case report.

RATIONALE: Stress-induced cardiomyopathy (SCMP), also known as Takotsubo syndrome, is a transient cardiac condition often precipitated by severe emotional or physical stress. It is commonly mistaken for acute coronary syndrome due to similar clinical presentations. The use of point-of-care ultrasound (POCUS) provides a noninvasive, rapid diagnostic alternative that can potentially reduce the need for invasive coronary angiography, especially in emergency settings. PATIENT CONCERNS: A 26-year-old woman with type 1 diabetes presented to the emergency department following a suicidal hanging atte

Blood urea nitrogen-to-albumin ratio independently predicts 30-day mortality in acute respiratory failure patients: a retrospective cohort study.

BACKGROUND: It is crucial to identify patients at high risk for acute respiratory failure (ARF) to provide appropriate and optimal clinical treatment. While previous studies have explored the use of prognostic biomarkers based on a combination of blood urea nitrogen (BUN) and albumin levels, no reports to date have evaluated its utility across a wide range of ARF etiologies in a large and diverse critical care population. Therefore, we aimed to ascertain the association between the BUN-to-albumin ratio (BAR) and mortality in these patients. METHODS: Data recorded in the first 24 h following i

Empowering healthcare professionals in West Africa-A feasibility study and qualitative assessment of a dietary screening tool to identify adults at high risk of hypertension.

Dietary risks significantly contribute to hypertension in West Africa. Food frequency questionnaires (FFQs) can provide valuable dietary assessment but require rigorous validation and careful design to facilitate usability. This study assessed the feasibility and interest of a dietary screening tool for identifying adults at high risk of hypertension in Nigeria. Fifty-eight (58) consenting adult patients with hypertension and their caregivers and 35 healthcare professionals from a single-centre Nigerian hospital were recruited to complete a 27-item FFQ at two-time points and three 24-hour reca

Perspectives of people living with HIV and health workers about a point-of-care adherence assay: a qualitative study on acceptability.

Current antiretroviral therapy (ART) adherence monitoring is premised on patients' self-reported adherence behaviour (prone to recall error) and verified by blood viral load measurement (which can delay results). A newly developed Urine Tenofovir Rapid Assay (UTRA) assesses tenofovir in urine at point-of-care and is a novel tool to test and immediately respond to adherence levels of people living with HIV (PLHIV). We explored PLHIV and health workers' initial perceptions about integrating the UTRA into routine medical care for adherence support. We conducted a series of once-off in-depth quali
