Bilateral peroneal nerve palsy secondary to prolonged sitting in an adolescent patient.

Peroneal nerve palsy most commonly occurs due to focal compression of the peroneal nerve at the level of the fibular head and causes foot drop and variable sensory loss. Mononeuropathies infrequently occur in childhood, bilateral peroneal palsy is very rare. In this article, we report the case of a 14-year-old boy with bilateral peroneal nerve entrapment which developed secondary to a prolonged sitting posture. We discuss the clinical assessment, diagnosis, and treatment of this rare case of bilateral peroneal palsy following prolonged immobility in light of the current literature.

Blunt traumatic great saphenous vein pseudoaneurysm.

Traumatic venous pseudoaneurysm is extremely rare. Only two cases of great saphenous venous (GSV) pseudoaneurysm have been reported. We present a case in which a gradually enlarged mass occurred at the calf after local injury. It was nonpulsatile and varied in size with posture. It was initially misdiagnosed by contrast MRI. Duplex ultrasound and surgical intervention eventually identified it as a GSV pseudoaneurysm. This case demonstrates the need for physicians to consider venous pseudoaneurysm as a possible diagnosis for nonpulsatile masses in patient with traumatic history, and to use Dupl

Surgical Management of Spasticity of the Elbow.

A spastic limb refers to one with increased tone. This commonly results from an upper motor neuron injury, which, in turn, leads to disinhibition of reflex arcs. At the level of the elbow, affected individuals typically exhibit a flexion posture secondary to spastic contracture of the biceps, brachialis, and brachioradialis muscles. Surgical treatment aims to improve access for hygiene, function, and cosmetic appearance of the affected limb. The specific surgical intervention performed depends on the degree of elbow flexion contracture and whether there is an associated joint contracture or so

Palatal and lingual augmentation prosthesis for patients with dysphagia and functional problems: A clinical report.

This clinical report describes the treatment of a patient with insufficient tongue movement due to bilateral hypoglossal nerve palsy. A palatal augmentation prosthesis and a mandibular intraoral prosthesis (lingual augmentation prosthesis) were provided for this patient to address his functional dysphagia problems. These problems included insufficient oral to pharynx bolus transportation and displacement of the tongue to the right posterior. The 2 prostheses improved these 2 problems and also the head extension posture during swallowing. After insertion of the 2 prostheses, the patient could t

Sleep and respiratory physiology in adults.

Respiration during sleep is determined by metabolic demand; respiratory drive is determined by a central respiratory generator. Changes in pharyngeal dilator muscle tone resulting in increased upper airway resistance and collapsibility contribute to hypoventilation. Relative hypotonia of respiratory muscles, body posture changes, and altered ventilatory control result in additional physiologic changes contributing to hypoventilation. This article reviews mechanisms of central control of respiration and normal upper and lower airway physiology. Understanding sleep-related changes in respiratory

Algorithmic approach to the management of the patient with lumbar spinal stenosis.

A 71-year-old generally healthy woman presents for her first visit in 3 years. She ambulates slowly from the waiting room, with a more stooped posture than previously. She reports a 2-year history of slowly worsening buttock and leg pain when she walks any distance. She has noticed that her symptoms are much less when she leans on a shopping cart in the grocery store. Her buttock/leg pain resolves within a few minutes when she sits down. The patient exhibits signs and symptoms suggestive of lumbar spinal stenosis.

[Bent spine straightens up--a case of camptocormia].

Bent spine, i.e. camptocormia appears in several disease. Segmental dystonia is one of these. A few cases benefiting from botulinum toxin therapy have been described in the literature. We describe a case, in which segmental dystonia is associated with camptocormia, causing the patient to use a wheelchair. The condition fulfilled the criteria of both clinical and electromyographic dystonia. Administration of botulinum toxin into rectus abdominus muscles was started. The response was good: the patient was able to walk in a fairly erect posture assisted by a rolling walker.

Cardiac magnetic resonance and "augmented" right-to-left intracardiac shunting through a patent foramen ovale.

Platypnea-orthodeoxia is an uncommon condition characterized by the development of hypoxia and breathlessness in the upright posture, relieved by resuming a supine position. First described in 1969, the condition has since been associated with intracardiac and intrapulmonary shunts, liver disease and a host of other conditions. We report an unusual case of episodic breathlessness and hypoxia cured by percutaneous cardiac intervention and discuss the evolving role of cardiac magnetic resonance imaging in the demonstration of functional distortions of cardiac and mediastinal anatomy that may not

Nerve disorders in dancers.

Dancers are required to perform at the extreme of physiologic and functional limits. Under such conditions, peripheral nerves are prone to compression. Entrapment neuropathies in dance can be related to the sciatic nerve or from a radiculopathy related to posture or a hyperlordosis. The most reproducible and reliable method of diagnosis is a careful history and clinical examination. This article reviews several nerve disorders encountered in dancers, including interdigital neuromas, tarsal tunnel syndrome, medial hallucal nerve compression, anterior tarsal tunnel syndrome, superficial and deep

An unusual murine behavior following infection with log-phase Nocardia asteroides type 6 strain GUH-2 (Nocardia cyriacigeorgica GUH-2).

Nocardia asteroides (Nocardia cyriacigeorgica) strain GUH-2 infects the brains of mice following intravenous injection. Non-lethal infections resulted in a transitory increase of bacterial numbers in the brain followed by the development of permanent impaired movements at a time when bacteria appeared to be eliminated from the brain. These signs included headshake, rigidity, stooped posture, dyskinesia, retropulsion, and abnormal tail positioning in approximately 20% of infected animals. The attached video presents a typical mouse following infection with this organism, as compared to an age-

Wheelchair-bound residents in nursing facilities: the basics.

More than 70% of nursing home residents spend at least part of their day in wheelchairs. Ideally, each resident would be fitted for his or her own wheelchair. A well-fitted chair can improve mobility, safety, and comfort. Wheelchairs that are ill fitting, however, can cause or exacerbate medical problems. These include posture-related complications like poor breathing or dysphagia, falls, pressure sores, and several other conditions. Pharmacists who are involved with quality improvement activities can make wheelchair use and selection a priority, improving care for wheelchair-bound residents.

Evolving concepts of laryngeal paralysis.

Accepted concepts of the pathophysiology and treatment of laryngeal paralysis have changed over the years. It has long been observed that symptoms of laryngeal paralysis vary greatly, both between patients and over time. There have been various theories to explain these differences. This article reviews how these ideas have changed over time as research has produced new information. Currently, the most popular view is that the laryngeal nerve regenerates after injury, albeit incompletely and inconsistently, and that variations in symptoms and laryngeal posture can be accounted for by muscle ac

Perineum push-up device to facilitate transabdominal suturing of the lower rectum.

Pressure on the perineum by the fist facilitates the placement of sutures into the rectal remnant, but put the assisting surgeon on an awkward and strenuous posture. The perineum push-up device to facilitate transabdominal suturing of the lower rectum was invented. The assisting surgeon can push up the perineum more comfortably with his abdominal wall or thigh. The device was tested in fifteen patients. The average distance that can be pushed up is 4.2 cm, which is a significant distance, facilitating in the maneuver of the distal rectal stump.

Overuse injuries in equestrian athletes.

Some 30 million Americans are involved in equestrian sports. Although there is significant literature regarding catastrophic injury, there is very little literature that addresses overuse injury and its effect on rider and horse. As the horse and rider function as a unit, overuse injuries to the rider can affect the horse's training and responsiveness. Sports physicians treating riders should understand correct posture in the saddle and its alteration by muscular imbalance and injury. This review discusses common equestrian events, and common overuse injuries in recreational and competitive ri

[Acquired right-to-left shunt with normal right heart pressure].

Intracardiac right-to-left shunting through a patent ovale foramen (POF) despite normal right-sided pressures is an uncommon cause of dyspnea with posture worsening in adults. We report herein 6 cases which sum up etiological circumstances, diagnostic workup and management of this syndrome. The main reported etiopathologic mechanism is consistent with a change in the internal configuration of the right atria leading to preferential blood flow from inferior vena cava through patent foramen ovale. The treatment lies on surgical closure of the septal defect, which can now be performed using a non

Relations between biased tonicity of the body and vertical judgement in poststroke hemiplegic persons.

The influence of muscular tension upon visual vertical judgement in a standing posture was investigated in 16 hemiplegic persons and 12 normal elderly persons. The normal elderly group showed that their judgements of verticality were accurate and stable. In hemiplegic persons, judgements were displaced opposite to the side in which there was high tension in the body. Differences between groups in body tension and the side to which judgement was displaced were significant. The peculiar judgement errors made by hemiplegic persons are thus an overcorrection for tonicity. It was related to one's o

Femoral neuropathy following cardiac catheterization for balloon mitral valvotomy.

Femoral neuropathy is a very rare complication of cardiac catheterization. We report an adult female who developed femoral neuropathy after undergoing cardiac catheterization through femoral vein for balloon mitral valvotomy. Neuropathy was confirmed by electromyography and nerve conduction studies and the patient showed spontaneous recovery over a course of 6 months. Use of prolonged digital pressure for post-procedural hemostasis is implicated as possible etiology. Such complications can be prevented by minimising the procedural time, avoiding injury to the vessels and maintaining optimal po

Transcranial electrical stimulation in patients with apallic syndrome.

Muscle responses (MEPs) to transcranial electrical stimulation were studied in 7 patients with apallic syndrome. All the patients showed clinical signs of upper motor neurone impairment in the upper and lower limbs. MEPs were absent or markedly delayed in 4 of the 7 patients. Since patients with apallic syndrome show only minimal voluntary movement, transcranial stimulation is the only way to demonstrate abnormalities of fast corticospinal axons in these patients. Even though these patients often look similar clinically, with tetraplegia and decorticate or decerebrate posture, only some cases

Epidemiology of low back pain in Chinese nurses.

This epidemiological study analyzed risk factors for low back pain (LBP) in nursing personnel in a 5000-bed medical center in the Republic of China. Of the 3212 eligible nursing personnel, 3159 (98.3 per cent) responded to the mail survey. Risk factors for LBP were age, stature, body weight, duration of work, work habits, and sitting posture. The lifetime prevalence of LBP was 77.9 per cent. The primary cause of LBP was lifting heavy objects. Muscle strain was the most common diagnosis. For LBP treatment, the nurses' first choice was physical therapy and rehabilitation.

Hip deformity and dislocation in spina bifida.

A group of 118 patients with spina bifida treated between 1960 and 1988 was reviewed with a focus on the problem of hip deformity and dislocation. Deformity can be avoided by physical therapy and correct posture. Hip dislocation often occurs in cases of high or mid-lumbar lesions; at lower levels subluxation is more common. In choosing appropriate treatment, the most important consideration is the patient's true functional potential. If the patient is either not ambulatory or severely impaired, his or her condition will not be exacerbated by a dislocated hip.
