The past 25 years has yielded an impressive growth in our knowledge of vocal function. Interdisciplinary research cooperation in areas of laryngeal histology, vocal aerodynamics and acoustics, vocal fold vibratory characteristics, neurolaryngology, and phonatory models has led to a clearer view of voice production. This article offers a brief review of the progress that has been made in our understanding of the speaking voice and relates this knowledge to clinical practice. The importance of utilizing voice research to confirm traditional management techniques and to develop new physiologicall

The majority of previous studies that have looked at the management of delayed eruption secondary to a supernumerary tooth advocate exposure of the unerupted tooth at the time of supernumerary removal. However, accepted UK orthodontic practice is to extract the supernumerary and provide sufficient space for the unerupted tooth to erupt spontaneously. The aim of this retrospective study was to evaluate the management and outcome for 96 patients who had been referred to Newcastle Dental Hospital between 1976 and 1988 who between them had 120 teeth with delayed eruption secondary to a supernumera

BackgroundThere remains an ongoing controversy in the prevention and management of deep vein thrombosis (DVT) following varicose vein surgery.MethodA 32-question electronic survey was sent out to current Vascular Surgeons and Fellow Phlebologists in Aotearoa New Zealand.ResultsThe response rate was 32.4%. There was a significant difference in the application of pre-operative DVT scoring using the Caprini score between Vascular Surgeons and Phlebologists (p < .001). Variation in prophylactic DVT treatment and duration of compression therapy were seen.ConclusionFurther studies are warranted to a

Bilateral humerus fractures as a result of birth trauma are a rare occurrence in neonatal care, necessitating special consideration due to their potential long-term implications. Birth-related injuries involving neonatal skeletal structures, especially fractures of the humerus, require special attention and a comprehensive approach to diagnosis and management. Here, we present the case of a newborn female child who experienced bilateral humerus fractures due to birth trauma. The subsequent management involved the application of splints to immobilize the affected arms, a standard practice in th

Cellulitis is a frequent pathology in both outpatient and inpatient clinical practice. This article reviews the pathophysiology of cellulitis, as well as the various complementary examinations useful in its diagnosis. It describes current recommendations for its management. Several differential diagnoses may also be considered, such as stasis dermatitis, lymphedema or deep vein thrombosis. These must be distinguished from cellulitis to avoid unnecessary antibiotic therapy. There are also a number of specific cases of cellulitis (necrotizing fasciitis, bites, diabetic foot) requiring special ma

Cardiac arrest remains a significant cause of morbidity and mortality, although contemporary care now enables potential survival with good neurologic outcome. The core acute management goals for survivors of cardiac arrest are to provide organ support, sustain adequate hemodynamics, and evaluate the underlying cause of the cardiac arrest. In this article, the authors review the current state of knowledge and clinical intensive care unit practice recommendations for patients after cardiac arrest, particularly focusing on important areas of uncertainty, such as targeted temperature management, n

Pegaspargase (Oncaspar®), a pegylated form of native Escherichia Coli-derived L-asparaginase is an essential component chemotherapy used in the treatment of acute lymphoblastic leukemia (ALL) in pediatric and adult patients. Its particular toxicity profile requires a specific management to improve safety and tolerability and optimize treatment outcome and therefore survival. Within the framework of workshops of practice harmonization of the French Society of Children and Adolescent Cancers, diagnostic and management of the most commonly occuring toxicities (excluding coagulation abnormalities)

Cardiorenal syndromes (CRS) describe disorders effecting critically ill and hospitalized patients with concurrent heart and kidney dysfunction. The presence of CRS is associated with a poor prognosis. This article is a review of the epidemiology, pathology, and evidence-based evaluation and management strategies for cardiorenal syndromes. All nurses should understand the significance that chronic heart and kidney disease has upon a patient's risk for CRS. Registered and advanced practice nurses should maintain the knowledge and skills of understanding the pathology of CRS to improve the evalua

NCCN Clinical Practice Guidelines in Oncology (NCCN Guidelines) for Pediatric Aggressive Mature B-Cell Lymphomas include recommendations for the diagnosis and management of pediatric patients with primary mediastinal large B-cell lymphoma (PMBL) and sporadic variants of Burkitt lymphoma and diffuse large B-cell lymphoma. PMBL is now considered as a distinct entity arising from mature thymic B-cells accounting for 2% of mature B-cell lymphomas in children and adolescents. This discussion section includes the recommendations outlined in the NCCN Guidelines for the diagnosis and management of ped

Reporting of severe invasive group A streptococcal disease (IGAS) has increased in Canada over the past decade, highlighting the importance of optimal chemoprophylaxis and management strategies. Canadian guidelines have had variable uptake across Canada. This practice point updates relevant aspects of these guidelines, with a focus on chemoprophylaxis of contacts of IGAS cases and clinical management of IGAS. The importance of penicillin in treating group A streptococcal disease is reaffirmed, and the role of clindamycin is discussed. In situations in which chemoprophylaxis may be considered,

Perioperative care of the patients with neurological diseases can be challenging. Most important consideration is the management and understanding of pathophysiology of these disorders and evaluation of new neurological changes that occur perioperatively. Perioperative generally refers to 3 phases of surgery: preoperative, intraoperative, and postoperative. We have tried to address few commonly encountered neurological conditions in clinical practice, such as delirium, stroke, epilepsy, myasthenia gravis, and Parkinson disease. In this article, we emphasize on early diagnosis and management st

X-linked agammaglobulinemia (XLA) is one of the commonest primary immune deficiencies encountered in pediatric clinical practice. In adults, common variable immunodeficiency (CVID) is the most common primary immunodeficiency disease (PID). It is an X-linked disorder characterized by increased susceptibility to encapsulated bacteria, severe hypergammaglobulinemia and absent circulating B cells in the peripheral blood. Replacement immunoglobulin therapy is the main cornerstone of treatment. Aggressive management of intercurrent infections and prophylactic antimicrobials are needed. This review a

With demands for an evidence-based approach to patient care, the management of facial fractures will come under increasing scrutiny because there is an overall deficiency in higher level clinical evidence. This article reviews the management of facial fractures, focusing on an evidence-based approach. It focuses on select areas of facial trauma in which there is controversy and presents randomized studies and meta-analysis to help define best practice. The article notes the many areas where the evidenced-based literature is weak and looks at the future of evidence-based facial trauma care.
