BACKGROUND: This study identified factors associated with medically serious suicide attempts (requiring medical hospitalization).
METHOD: Demographic information, current psychiatric mental state, suicide attempt and psychiatric history characteristics, and DSM-IV diagnoses were compared between 65 patients hospitalized for a medically serious suicide attempt (MSSA) and 32 patients seen in the emergency room for suicide attempt but not medically hospitalized (NMSSA).
RESULTS: Those with MSSAs had a higher rate of substance-induced mood disorder (but not substance abuse or dependence), while

Of 687 consecutive inpatients and outpatients with primary major depressive illness, 213 (31%) were categorized as single episode (SE) by DSM-III-R criteria. Systematic evaluation of familial, sociodemographic, temperamental and symptomatological characteristics permitted the nearly equal division of SE into two categories: a. early-onset (< 45 years) "first episode" superimposed on either depressive or hyperthymic temperaments (against a bipolar and unipolar familial background), more severe depression, higher rates of suicide attempts, greater anxiety-somatization and psychotic tendencies, a

The efficacy of oral concentrate in the rapid tranquilization of 159 acutely disturbed patients in a psychiatric emergency service was evaluated. Patients were randomly assigned to 1 of 10 treatment groups: 7 oral groups and 3 comparison intramuscular groups. Although intramuscular medication produced a faster response, concentrate also proved to be effective for rapid tranquilization. Side effects were benign and minimal. Age, sex, race, and diagnosis had no effect on outcome. The use of concentrate as a paradigm for treating acutely psychotic patients is discussed.

With the development of the Behavioral Activation for Depression Scale (BADS) by Kanter, et al. [1], different behavioral aspects of depression like activation, rumination or avoidance, and functional impairment can be measured. The long and the short versions of the BADS, however, show differences in the quality of psychometric properties. We wanted to validate the short and long forms of the BADS. We therefore evaluated the factor structure, the psychometric properties, and the construct validity of the long version and the subscales in a clinically depressed German sample (n=258) in study 1

BACKGROUND: Studies that deepen in the aspects related to quality of life (QoL) of elderly with dementia living in nursing homes in Spain are needed. The aim of this study is to describe the QoL and related aspects in this population.
METHODS: Sample of 525 people with dementia older than 60 years in 14 nursing homes. QoL was assessed with EQ-5D (both index and visual analogue Scale, EQ-VAS) and Quality of Life in Alzheimer's Disease(QoL-AD, resident and caregiver versions). Other scales were also applied: Clinical Dementia Rating Scale (CDR), Barthel Index, Cornell Scale for Depression in De

Greater levels of socioeconomic position (SEP) are generally associated with better health. However results from previous studies vary across race/ethnicity and health outcomes. Further, the majority of previous studies do not account for the effects of life course SEP on health nor the effects of racial discrimination, which could moderate the effects of SEP on health. Using data from the Coronary Artery Risk Development in Young Adults (CARDIA) study, we examined the relationship between a life course SEP measure on depressive symptoms and self-rated health. A life course SEP was constructed

OBJECTIVES: PTSD prevalence rates in cardiac and cancer patients greatly vary probably due to diverging samples, designs and assessments. Aim of this study was therefore to compare prevalence rates of PTSD as well as instruments assessing PTSD in different cardiac and cancer patient groups.
METHODS: A total of 274 cardiac and cancer patients were examined with the Post-Traumatic Stress Scale 10 (PTSS-10) and the Impact of Event Scale - revised (IES-R). Presence of PTSD was validated by Structured Clinical Interview for DSM-IV (SCID).
RESULTS: Diagnostic criteria for the existence of PTSD wer

OBJECTIVES: We evaluated the impact of a diagnosis of serious mental illness on use of a primary care provider (vs the emergency department [ED]) as a source of care by people who were chronically homeless.
METHODS: We used data from 750 chronically homeless adults enrolled in the 11-site Collaborative Initiative to Help End Chronic Homelessness and identified demographic and clinical characteristics independently associated with using a primary care provider rather than an ED.
RESULTS: The factor most strongly associated with using the ED as a regular source of medical care was previous-yea

We examined the joint predictive effects of childhood and adolescent onset psychiatric and substance use disorders on failure to graduate high school (HS) on time. Structured diagnostic interviews were conducted with a US national sample of adults (18 and over). The analysis sample included respondents with at least 8 years of education who were born in the US or arrived in the US prior to age 13 (N = 29,662). Psychiatric disorders, substance use and substance use disorders were examined as predictors of termination or interruption of educational progress prior to HS graduation, with statistic

OBJECTIVE: There may be ethical issues associated with allowing certain inpatients to vote as some may be cognitively impaired. During the 2007 elections in France, we conducted a prospective observational study on voting among hospitalized patients.
METHOD: Patients hospitalized in an Internal Medicine and Geriatric Department on election day were included. The primary outcome was the turnout among registered inpatients, and secondary outcomes were Mini-Mental State Examination (MMSE) scores and reasons for abstention.
RESULTS: Of 142 inpatients (mean age 73 years), 84 were eligible to vote

Though the medical and mental health morbidity of incarcerated offenders has been discussed in a number of recent reports, very few data have been published concerning medical and mental health problems facing those on community corrections supervision. In this study of community corrections offenders utilizing residential facilities, we found that frequencies of substance use disorders, other mental health disorders, and medical problems exceeded frequencies found in the community and, in some cases, were higher than frequencies found in incarcerated individuals. Of particular concern were th

BACKGROUND: The CAMCOG is a brief neuropsychological battery designed to assess global cognitive function and ascertain the impairments that are required for the diagnosis of dementia. To date, the cut-off scores for mild cognitive impairment (MCI) have not been determined. Given the need for an earlier diagnosis of mild dementia, new cut-off values are also necessary, taking into account cultural and educational effects.
METHODS: One hundred and fifty-seven older adults (mean age: 69.6 +/- 7.4 years) with 8 or more years of formal education (mean years of schooling 14.2 +/- 3.8) attending a

OBJECTIVES: The aims of this study were to identify the clinical and demographic factors influencing health-related quality of life (HR-QoL) and to compare HR-QoL measures between various types of focal dystonia (cervical dystonia, blepharospasm, and writer's cramp).
METHODS: We examined 157 consecutive patients with adult-onset primary focal dystonia, and HR-QoL was assessed by using the SF-36 questionnaire. Univariate and multivariate regression analyses were performed.
RESULTS: Patients with writer's cramp scored better in all SF-36 domains, except role functioning physical (RP), while th

Although life stress has been shown to trigger relapse in bipolar disorder, little is known about how bipolar patients perceive daily hassles or their positive counterparts, uplifts. We used the experience sampling method to investigate the daily experience of hassles and uplifts in 38 patients with remitted bipolar disorder and 38 healthy controls. Largely because of current unemployment, patients were more often alone and at home and spent less time working and more time in passive leisure activities. Contrary to expectations, the groups did not differ in total frequencies or appraisals of e

BACKGROUND: Alzheimer s disease (AD), according to the free radical hypothesis, affects brain regions where free radical damage occurs. Antioxidant nutrients may help to protect these brain regions.
OBJECTIVE: To investigate whether plasma vitamin C and E status is lowered in subjects with AD and dementia.
DESIGN: A case control study was conducted in 93 institutionalized subjects aged 65 + yrs. The dementia group (N = 43) included 15 subjects with Alzheimer s Disease (AD) and 28 subjects with senile dementia, while the control group included 50 subjects with no cognitive impairment. Subject

Accumulating evidence suggests that posttraumatic stress disorder (PTSD) is linked to both objective and subjective indices of poorer health. Less is known about processes that may explain this association. This study examined anger/hostility as a possible mediator and moderator of PTSD and health status among a sample of 134 medical patients. Participants completed a structured interview of PTSD and questionnaires assessing health perceptions and anger and data on physician-diagnosed illnesses were gathered from computerized databases. Trait anger and anger-in partially explained the associat

BACKGROUND: Depression is underdiagnosed worldwide, and clinicians rely on interpreting patients' subjective speech. Qualitative analysis of patient language does not scale, and existing computational approaches describe topics with keyword lists that miss clinical nuance.
OBJECTIVE: We evaluated whether clustering spontaneous speech transcripts with large language models (LLMs) yields clusters whose membership is associated with validated clinical scales across multilingual cohorts, and whether LLMs can render those clusters human-readable through fine-grained natural-language descriptions.

We present a unique thirty-nine year old woman with both Huntington's disease (HD) and spinocerebellar ataxia type 10 (SCA10). She has 48 CAG repeats in the HD gene and 2511 ATTCT repeats in the ATX10 gene. Although both conditions are repeat expansion diseases they are thought to have quite different pathogenic mechanisms. The symptomatic age of onset in this patient (mid30s) is within the expected range for her repeat expansion sizes for each condition, but we discuss the evidence that the two conditions may interact to produce a more severe cognitive phenotype than would be expected for eit
