[The current conception of the unconscious - empirical results of neurobiology, cognitive sciences, social psychology and emotion research].

The influence of the unconscious on psychosomatic medicine and psychotherapy: a comprehensive concept of unconscious processes based on empirical evidence. The theory of the Unconscious constitutes the basis of psychoanalysis and of psychodynamic therapy. The traditional description of the Unconscious as given by Freud is of historical significance and not only gained widespread acceptance but also attracted much criticism. The most important findings of neurobiology, the cognitive sciences, social psychology and emotion research in relation to the Unconscious are compared with this traditiona

[Screening program for hemoglobinopathies based on blood donors from Bragança Paulista, São Paulo, Brazil].

Screening programs for hemoglobinopathies target different population groups such as neonates, students, pregnant women, military personnel, recruits, and blood donors. The feasibility and efficiency of these programs basically depend on the population's receptivity, which in turn is related to highly complex economic, psychological, and sociocultural factors. The purpose of this study was to evaluate the population of Bragança Paulista, São Paulo, based on results from the blood donors' group and to compare these results with those previously obtained in a group of students from the same city

Psychological features in persons at risk for familial Alzheimer's disease.

Persons at risk for inherited neurodegenerative diseases may experience symptoms of anxiety and depression because of concern over the possibility of developing the disease in the future. The purpose of this study was to assess psychological and emotional symptoms in persons at the age of risk for developing early-onset familial Alzheimer's disease (FAD). Their responses on a psychiatric rating scale (SCL-90-R) were compared with four groups: patients with mild FAD; head injury patients; patients with clinically diagnosed depression; and healthy control subjects. Mean scores of the at-risk FAD

Significance of autonomic nervous system activity in functional dyspepsia.

The purpose of this study was to examine if patients with functional dyspepsia could be separated into meaningful groups based on their autonomic function. Subjects were divided into two groups, and symptoms, gastric myoelectrical activity, gastric emptying, and psychological factors were compared. Group 1 had less autonomic variability but more cardiac reactivity than group 2. Symptom reports did not differ between groups. Group 1 had higher neuroticism scores than group 2, while group 2 showed greater tachyarrhythmia in response to drinking water than group 1. The relatively low autonomic va

The effect of surgery upon the quality of life in snoring patients and their partners: a between-subjects case-controlled trial.

Snoring is a common complaint in the community. Its psychosocial effect upon the relationship between sleeping partners is not known. This study examines prospectively by detailed tailored questionnaires the differences in the physical, psychological and social aspects in 27 snoring patients and their partners presenting to a snoring clinic and compares them with a control group of 24 patients who had undergone successful Laser-Assisted Uvulopalatoplasty (LAUP) and their partners. All areas examined showed improvement in the non-snoring group, particularly for the patient. Improvement was demo

Relaxation to assist sleep medication withdrawal.

This study explores the usefulness of relaxation and gradual medication withdrawal in weaning insomniacs from sleep (hypnotic) medication. We recruited 40 volunteers from the community who had insomnia, half of whom were chronic users of hypnotics while the other half were nonmedicated. Half of all participants (10 medicated and 10 nonmedicated) received progressive relaxation. All medicated participants received a standard gradual drug withdrawal program. Medicated participants reduced sleep medication consumption by nearly 80%. Participants who received relaxation obtained additional benefit

The alternatives to hysterectomy.

Hysterectomy is one of the commonest major operations, with 72,362 procedures performed in England in 1993. However, for women with dysfunctional uterine bleeding other less invasive surgical options have been developed. In particular, hysteroscopic endometrial ablation has been shown to be an effective therapy. Prospective randomized trials have shown that hysteroscopic surgery is associated with a shorter operating time, fewer complications, less analgesic requirement, a faster resumption of normal activities and work and savings in treatment costs. Psychological and social functioning also

Profiles of heroin addicts in different treatment conditions and in the community.

This article describes profiles of heroin addicts in three types of treatment (methadone, detox and therapeutic community) and those not in treatment in the local community. The profiles are based on data at item-level of the revised Addiction Severity Index. In total, 310 heroin addicts were interviewed. The results show that on the one hand the community group and the methadone group are roughly comparable, and on the other hand the detox and therapeutic community groups have similar characteristics. The latter groups report significantly more psychological and social problems. The community

Control of human vascular smooth muscle cell proliferation by sera derived from 'experimentally stressed' individuals.

Recently we reported that individuals with high scores in standardized hostility evaluation tests, when placed in a environment, may have an association with increases in a blood-borne mitogenic substance(s) for arterial smooth muscle cells. To further investigate the molecular basis for such an association, PDS [plasma derived serum with platelet derived growth factor (PDGF) removed] from individuals showing the greatest differential pre/post-stress mitogenic activity, were tested for ability to modulate changes in the steady state of the c-myc mRNA in cultured VSMC (vascular smooth muscle ce

A family study of hypochondriasis.

To examine the diagnostic validity of hypochondriasis, we undertook a preliminary family study. Nineteen probands with and 24 without DSM-III-R hypochondriasis were identified among outpatients attending a general medicine clinic. Seventy-two first-degree relatives of hypochondriasis probands and 97 relatives of control probands were personally interviewed with the use of the Structured Clinical Interview for DSM-IV. These relatives also completed self-administered measures of hypochondriasis, psychological and somatic symptoms, and personality traits. No increase in the rate of hypochondriasi

Supportive versus cognitive-behavioral intervention programs in achieving adjustment to home peritoneal kidney dialysis.

Two psychological interventions given for 8 weeks, supportive and cognitive-behavioral, were compared in achieving psychosocial adjustment to home peritoneal kidney dialysis. Participants were divided into 3 groups of patients and their spouses: a supportive group (18 couples), a cognitive-behavioral group (18 couples), and a no-intervention control group (24 couples). A group of 97 healthy participants served as a baseline control group. Self-report measurements were made before treatment (T1), halfway through (T2), and after treatment (T3). Results indicated that, without treatment, the no-i

A brief history of psychotherapy and physical disability.

In this brief history, the authors have attempted to highlight salient aspects of psychotherapy with physically disabled individuals across a span ranging from the prepsychoanalytic to the contemporary. Particular attention is given to the prepsychoanalytic work of Charcot and Janet on neurological diseases and trauma, respectively. Psychoanalytic concepts are reviewed as they relate to physical disability and they are compared with contemporary themes involving trauma and loss. The paper has a distinctly psychoanalytic bias about the psychology of congenital and acquired physical differences.

Controlled trial using computerized feedback to improve physicians' diagnostic judgments.

The goal of this study was to test an innovative method to improve physicians' diagnostic judgments by integrating the use of a computer program (employing cognitive feedback to teach a clinical rule that predicts the probability of streptococcal pharyngitis), a traditional lecture, and periodic disease-prevalence reports. In a controlled trial using pre- and postintervention measures involving 885 patients, the authors compared the effects of the integrated method on the diagnostic judgments of seven experienced physicians at a university health service (from 1982 to 1985) with the effects of

Platelet serotonin decrease in alcoholic patients.

This study investigated the platelet 5-HT levels and their changes according to different physiological and pathological factors in 30 young alcoholic patients (16 alcohol abusers and 14 alcohol-dependent subjects) and 26 healthy controls. Platelet 5-HT levels were determined by a fluorescent-ortho-phthalaldehyde assay. The mean platelet 5-HT levels obtained in patients during withdrawal and after 2 weeks of abstinence were significantly lower than in controls. Presence of positive history of impulse control disorders (ICD) influenced the mean platelet 5-HT levels in patients. These preliminar

Appraisal of individual radiation risk in the context of probabilistic exposures.

There exists a growing desire to base safety criteria in different fields on the same principles. The current approach by the international Commission on Radiological Protection (ICRP) to control radiation exposure touches many aspects such as social, psychological, or economic factors that are important for such principles. This paper attempts to further explore possible ways of defining a common basis for dealing with radiation risks and other safety problems. Specifically, it introduces the following issues: (1) different types of risk are judged differently. To account for this, the concep

A model for predicting depression in elderly tenants of public housing.

To assess the prevalence of and risk factors for depression in an older population, the Multilevel Assessment Instrument (MAI) and the Center for Epidemiological Studies Depression Scale (CES-D) were administered to 176 elderly residents of a public housing apartment building in Indianapolis. The 41 patients diagnosed by the CES-D as depressed had significantly lower scores on MAI measures of psychological adjustment, cognitive function, and physical health compared with the nondepressed respondents and were more likely to have had episodes of anxiety and depression in the year before the stud

Clinical implications of lesbian mother studies.

Recent surveys of lesbians have revealed that one-third have been heterosexually married, and one-half of these have had children. Studies comparing lesbian mothers and their children with divorced heterosexual mothers and their children provide data of value to clinicians preparing to evaluate or treat members of this population. Studies show similarities between the two groups in marital history, pregnancy history, child-rearing attitudes, and lifestyle. Motherhood, not sexual orientation, is the most salient factor in both group's identity. Lesbian mothers had more congenial relations with

The development of normal and autistic children: a comparative study.

This study tests the hypothesis that the development of normal and autistic children differs only in rate and asymptote. A total of 195 normal children between 1 and 5 years of age, 160 normal children between 3 months and 24 months of age, and 41 autistic children between 5 and 11 years of age were evaluated on the eight psychological variables constituting the Behavioral Rating Instrument for Autistic and other Atypical Children (BRIAAC). While many similarities were found, there were a sufficient number of differences to justify DMS-III's statement that certain autistic behaviors are not no

Medical aspects of drug misuse during one year in a rehabilitation unit.

The medical work in a voluntary drug rehabilitation unit near Glasgow was examined. During one year 174 residents were admitted of whom 103 (59%) developed illnesses which required medical treatment. The need for drug misusers to receive general medical services during and after drug misuse was confirmed. Although withdrawal from barbiturate misuse required the prescription of controlled drugs, opiate and other withdrawals were satisfactorily managed with psychological support and general care; substances which could be abused were not prescribed. Blood testing of 129 residents showed that 114

Sexual dysfunction and disorder patterns of working and nonworking wives.

The sexual dysfunction and disorder patterns of 218 married working and nonworking women were compared in a retrospective analysis of couples presenting for sexual and marital therapy at the Masters & Johnson Institute. Results indicated that women who were pursuing careers of an ongoing, developmental nature were twice as likely to present with a primary complaint of inhibited sexual desire than women who were employed in jobs that emphasized the immediate organization of activities, or women who were unemployed outside of the home. "Career" women were also significantly more likely to presen
