Pediatric kidney diseases in an African country: prevalence, spectrum and outcome.

Insufficient data to guide the authorities responsible for resource allocation and a focus on communicable diseases increase the challenges of care of children with kidney disease in resource-constrained settings like ours. This study was performed with the aim to describe the current spectrum of pediatric nephrology disease in a tertiary hospital in Sub-Saharan Africa and highlight the challenges encountered in their care. A 4-year retrospective review of pediatric renal admissions was carried out and the overall prevalence, disease-specific prevalence and mortality rates were determined. Res

End-of-Life Care in the Austere Military Environment.

INTRODUCTION: Throughout the Global War on Terror (GWOT), the U.S. military built a revolutionary capacity to deliver life-saving care-even in austere environments-through rapid evacuation, far-forward providers and resources, and advances in prehospital and trauma-critical care. However, the operational reality of future Large Scale Combat Operations (LSCO), as evidenced by wars in Ukraine, Sudan, and Gaza, will be marked by high casualty rates and limited medical resources. The number of patients with non-survivable injuries because of operational constraints such as inability to evacuate, m

User validation of an AI-assisted ultrasound guidance system for rapid vascular access.

OBJECTIVES: Vascular cannulation enables life-saving resuscitation in traumatic hemorrhage. The AI-GUIDE-LL is a handheld ultrasound-guided robotic device enabling inexperienced medical providers to obtain femoral vascular access. Device efficacy has been demonstrated in expert-operated porcine models, though performance among inexperienced operators and in humans is unknown. We evaluated the AI-GUIDE-LL's performance among novice clinicians to target human femoral vessels and accurately deploy needles in phantoms. MATERIALS AND METHODS: This single-site prospective study recruited medical pr

Identifying the dimensions and components influencing health literacy and self-care in endometriosis: a scoping review.

BACKGROUND: Endometriosis is a chronic, progressive inflammatory disorder affecting nearly 10% of reproductive-age women, yet it remains substantially underdiagnosed. Limited health literacy, inadequate access to reliable information, and reliance on variable-quality digital resources hinder timely symptom recognition and effective self-management. Understanding the determinants of health literacy and self-care is therefore essential to improving disease outcomes. METHODS: This scoping review followed the Arksey and O’Malley framework, refined by Levac et al., and reported in accordance with P

Bioelectrical impedance analysis versus dual-energy X-ray absorptiometry for body composition assessment in children with chronic kidney disease.

INTRODUCTION: Children with chronic kidney disease (CKD) exhibit progressive body composition alterations. While dual-energy X-ray absorptiometry (DXA) is the reference standard, its routine use is limited by cost, radiation, and availability. Bioelectrical impedance analysis (BIA) offers a portable, radiation-free alternative, but its accuracy relative to DXA in paediatric CKD remains insufficiently evaluated. METHODS: This cross-sectional study enrolled 102 children aged 5-14 years with CKD stages 3-5. Same-day BIA and DXA measurements were performed. Agreement between methods was assessed

Bridging the gap in headache care: a qualitative needs assessment for primary care education in Indonesia.

INTRODUCTION: Headache is a common complaint encountered in primary care. In Indonesia, however, only migraine and tension-type headache are expected to be fully managed by general practitioners (GPs). Effective management requires a comprehensive understanding of headache diagnosis, treatment, and appropriate referral in primary care settings. Educational interventions for GPs are therefore essential to address existing unmet needs. This study aims to conduct a needs assessment to identify barriers in headache management in primary care and to determine gaps that can inform the development of

Experiences, barriers, and enablers of integrated HIV and sexual and reproductive health services in Nigeria: a qualitative study of service users and healthcare providers.

BACKGROUND: Integrating HIV and sexual and reproductive health (SRH) services is a global priority for improving access to comprehensive, equitable, and people-centred care. However, people living with HIV (PLHIV) continue to experience unmet SRH needs, and evidence on how integrated services are experienced, delivered, and utilised in routine care settings remains limited in Nigeria. This study explored service users' and healthcare providers' experiences of integrated HIV-SRH services, including patterns of service delivery and utilisation, barriers, facilitators, and recommendations for str

A tiled amplicon protocol for culture-free whole-genome sequencing of M. tuberculosis from clinical specimens.

Whole-genome sequencing of Mycobacterium tuberculosis can be a valuable tool for TB surveillance and treatment, providing insights into transmission patterns and comprehensive drug susceptibility testing. However, the slow growth of M. tuberculosis means traditional culture-based sequencing methods can take weeks to return results, which has limited the widespread adoption of these techniques and limited their use in clinical decision-making. Tiled amplicon sequencing is a fast, reliable, and cost-effective method of whole-genome sequencing that can be done directly on clinical specimens and h

Assessing the consistency of mass spectrometry, a clinical-laboratory model, and immunohistochemistry in amyloid subtyping: a Brazilian experience.

BACKGROUND: Systemic amyloidosis is a potentially fatal protein misfolding disorder usually underdiagnosed in low- and middle-income countries, where limited awareness and restricted access to diagnostic tools contribute to prolonged diagnostic journeys and delayed diagnoses. Accurate identification of the precursor protein is essential but remains a challenge, particularly in resource-limited settings. This study aimed to perform mass spectrometry (MS) for amyloid subtyping and to use it as the reference method to evaluate the consistency of a clinical-laboratory model (CLM) and immunohistoch

Management challenges of postpartum rhinocerebral mucormycosis following spontaneous vaginal delivery in a resource-constraint setting: A case report and review of literature.

INTRODUCTION AND IMPORTANCE: Rhinocerebral mucormycosis (RM) is a rare and severe condition caused by filamentous fungi, characterized by infection of the nose, paranasal sinuses, and brain. It is the most common and fatal clinical form of mucormycosis, accounting for 50 % of reported cases. RM is seldom reported during the postpartum period. This case report presents a rare instance of RM following childbirth and reviews the most recent literature, highlighting the limited cases documented globally and even fewer within Africa. The aim is to contribute to the existing body of knowledge and pr

Enhanced detection of sepsis pathogens in hospitalized febrile neutropenia patients using blood culture broths with TaqMan array card.

BACKGROUND: In resource-limited settings, febrile neutropenia (FN) is one of the leading causes of morbidity and mortality among immunocompromised individuals particularly those undergoing chemotherapy for malignancies. The clinical diagnosis and management of disease is challenging, due to the diverse range of causative pathogens and overreliance on the traditional diagnostic techniques. TaqMan array card (TAC), a microfluidics-based technique that allows simultaneous and rapid detection of multiple pathogens, addressing these diagnostic needs. OBJECTIVE: The aim of this study was to determi

Client, health care provider, and policymaker perspectives on point-of-care/near-point-of-care testing for sexually transmitted infections in community pharmacies in Uganda.

INTRODUCTION: Point-of-care or near-point-of-care (POC/near-POC) tests for sexually transmitted infections (STIs) are often unavailable and unaffordable in low-resource settings, despite the World Health Organization's (WHO) recommendation that such testing be offered, leading to empiric treatment. Private community pharmacies (CPs) are often the first point of contact for people with symptoms of STIs, where pharmacists dispense treatment without diagnostic testing or prescriptions. This study evaluated clients,' providers,' and policymakers' perspectives on POC/near-POC STI testing in communi

Integrating ayurvedic eye care into primary health practice: an exploratory study on the combined effect of pratimarsha nasya, avagundana, and aschyotana in meibomian gland dysfunction.

BACKGROUND: Meibomian Gland Dysfunction (MGD) has become increasingly common in community practice, often presenting as dryness, irritation, and ocular fatigue. Factors such as prolonged screen exposure, environmental irritants, and advancing age contribute to its growing burden in primary health settings. Conventional management usually focuses on symptom relief, leaving a need for safer, sustainable options that can be applied easily in routine care. Ayurvedic eye therapies, known for their gentle yet restorative effects, may offer such an alternative. METHODS: A single-arm, open-label clin

Effectiveness of a locally made eye protector for neonates treated with phototherapy at Queen Elizabeth Central Hospital (QECH), Malawi: a bench study.

BACKGROUND: The highest incidence of severe neonatal jaundice occurs in Africa at 667.8 per 10,000 live births. At the Queen Elizabeth Central Hospital (QECH), approximately 75% of neonates have clinical jaundice, with up to 15% requiring treatment. While phototherapy is the standard treatment, the intense blue light poses the risk of retinal damage, which has led to the routine practice of eye protection for neonates receiving this therapy. Nonetheless, QECH, like other low-resource settings, lacks standard eye shields, forcing staff to improvise with different materials to shield the neonate
