BACKGROUND: Infective endocarditis (IE) continues to show a rising global incidence and significant associated mortality. Among its complications, pseudoaneurysms (pAs) are peri-annular lesions resulting from localized tissue destruction caused by infection, leading to a focal dilatation. These lesions usually occur as isolated findings, and the presence of multiple pAs is rare. Importantly, pAs are recognized predictors of in-hospital mortality.
CASE SUMMARY: We describe three distinct patients exhibiting IE complicated with multiple pAs with particular and unique features. Patient 1 is a 49

PURPOSE OF REVIEW: The novel coronavirus disease 2019 (COVID-19) is caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) and has developed into a pandemic. A unique challenge of this pandemic has been the emergence of multisystem inflammatory syndrome in children (MIS-C), a rare post-infectious hyperinflammatory disorder associated with SARS-CoV-2. This syndrome is characterized by overwhelming systemic inflammation, fever, hypotension, and cardiac dysfunction. This disorder may also have features overlapping with Kawasaki disease (KD), macrophage activation syndrome (MAS), a

We present the case of a 68 year old Caucasian woman, in extremis, with left hemiparesis and right hemothorax, in hypovolemic shock, secondary to a blow-out of a large penetrating ulcer at the junction of innominate trunk and aortic arch. She underwent interposition graft replacement of innominate trunk and repair of aortic arch, on cardiopulmonary bypass, employing total circulatory arrest and selective antegrade cerebral perfusion and had total resolution of hemiparesis. She, however, represented, 6 months later, with threatened exsanguination after a sternal wire cheese-wired through the st

OBJECTIVES: Patients with acute respiratory distress syndrome (ARDS) undergoing venovenous extracorporeal membrane oxygenation (ECMO) may experience severe cardiogenic shock, necessitating ECMO mode "escalation" for cardiopulmonary co-support. There is a paucity of data regarding the prevalence, risk factors, and mortality of venovenous ECMO escalation. We aimed to conduct a large study to address this issue.
DESIGN: A multicenter cohort study.
SETTING: The analysis was conducted based on data from the Chinese Society of Extracorporeal Life Support registry, a nationwide voluntary platform c

BACKGROUND: Despite advancements in management strategies, patients with refractory cardiogenic shock (CS) have high mortality. Veno-arterial extracorporeal membrane oxygenation (VA-ECMO) is increasingly utilized as temporary mechanical support in these patients. Current predictive models for mortality suffer from practical limitations, including complexity and the extensive variables required.
OBJECTIVES: We aimed to develop a simplified, practical predictive model, the Extracorporeal Life Support Outcome score (ELSO-Score), using readily available pre-ECMO variables to predict in-hospital m

OBJECTIVE: Few reliable tools exist to predict weaning patient outcomes from venoarterial extracorporeal membrane oxygenation (ECMO; VA-ECMO). Pulmonary artery pulse pressure indexed on right atrial pressure (PAPi) reflects the ventricle-pulmonary coupling and may be representative of right ventricular recovery. Radial artery pulse pressure (rPP) may be representative of left ventricular recovery. We intended to explore the usefulness of these indices in the weaning from VA-ECMO.
DESIGN: Retrospective cohort study.
SETTING: Single center in a tertiary university hospital.

BACKGROUND: The clinical outcome and fluid balance of patients with veno-arterial extracorporeal membrane oxygenation (VA ECMO) or after extracorporeal cardiopulmonary resuscitation (eCPR) may be improved by addressing the high fluid demand with an early albumin administration.
METHODS: In this prospective observational study, patients supported with VA ECMO or eCPR received early albumin administration (25 g/L) to prime the VA ECMO system. These patients were compared to patients who received a regimen based solely on balanced crystalloids (crystalloid group) or a regimen based on a 1:4 volu

Catheter-based micro-axial ventricular assist device Impella® (Abiomed, Danvers, MA) has been used in Japanese patients with drug-refractory acute heart failure (AHF) since 2017. This is the first interim analysis of the ongoing Japan Registry for Percutaneous Ventricular Assist Device (J-PVAD) to investigate the safety and efficacy of Impella support. Between October 2017 and January 2020, 823 Japanese patients, who were treated with the Impella 2.5, CP, or 5.0 pump, were enrolled. The primary endpoints were safety profiles and cumulative 30-day survival. Among them, 44.8% of patients were ac

BACKGROUND: Over the past 20 years, the development of regional ST-elevation myocardial infarction (STEMI) care systems has led to remarkable progress in achieving timely coronary reperfusion with attendant improvement in clinical outcomes, including survival. Despite this progress, contemporary STEMI care does not consistently meet the national guideline-recommended goals, which offers an opportunity for further improvement in STEMI outcomes. The lack of single, comprehensive, national STEMI registry complicates our ability to improve STEMI outcomes in particular for high-risk STEMI subsets s

BACKGROUND: The main risk factor for bleeding in patients with continuous-flow mechanical circulatory support (CF-MCS) is the acquired von Willebrand factor (VWF) defect related to the high shear-stress forces developed by these devices. Although a higher bleeding rate has been reported in CF-MCS recipients who had reduced pulsatility, the relation between pulsatility and the VWF defect has never been studied.
OBJECTIVES: The purpose of this study was to investigate the relation between pulsatility and VWF under CF-MCS.
METHODS: We assessed the effect of 2 CF-MCS on VWF multimer degradation

Patients with multiple comorbid conditions visit the dental office every day, and although rare, complications from their conditions may occur during treatment. A case is presented of a 65-year-old African American woman with a history of severe cardiovascular disease, renal disease, and a reported local anesthetic allergy who experienced complete cardiovascular collapse during routine dental treatment from which she was successfully resuscitated. Treating clinicians should recognize the emerging symptoms and be proficient with a basic and advanced cardiac life support protocol to care for the
