[Role of host response during severe bacterial infection].

Explosion of knowledge both in human genomics and in host inflammatory response explains the increasing interest in infectious disease genetics over the last 5 years. However, twin and adoptee studies have suggested more than 15 years ago, that host genetic factors are major determinants of susceptibility to infectious diseases in humans. Recently, candidate gene studies (association studies) and human genomewide analysis have been used to identify infectious diseases susceptibility and resistance genes. Rarely, a single gene defect has been directly related to devastating consequences such as

Treatment of Melioidosis.

Melioidosis is caused by Burkholderia pseudomallei, a Gram-negative environmental saprophyte found in tropical and subtropical regions globally. The aims of treatment for melioidosis are to prevent death and other complications of septic shock, and to eradicate B. pseudomallei and prevent relapse. To achieve these aims, treatment comprises an intensive phase involving minimum 10-14 days of intravenous ceftazidime, meropenem, or imipenem, and a prolonged eradication phase of at least 3 months of oral trimethoprim-sulfamethoxazole. Here, we review the clinical trial and other evidence that suppo

An Atypical Cause of Pneumatosis Intestinalis: Norovirus Infection in an Elderly Male.

Norovirus is a major cause of gastroenteritis, commonly presenting with nausea, vomiting, diarrhea, myalgia, and fever. Most cases are self-limited, but serious complications can occur, particularly in high-risk populations. Pneumatosis intestinalis is an uncommon but serious condition that may require surgical intervention and is characterized by the presence of gas in the bowel wall. Pneumatosis intestinalis is a known, albeit extremely rare, complication of norovirus infection. We report a case of norovirus gastroenteritis in a 77-year-old male complicated by septic shock, pneumatosis intes

A Review of Current Evidence for the Use of Steroids in the Medical Intensive Care Unit.

Systemic steroids are frequently used in critically ill patients for their anti-inflammatory properties. Potential benefits of these agents should be balanced against their known side effects. In this paper, we review trials assessing the use of systemic steroids in common conditions requiring admission to the intensive care unit. These include septic shock, the acute respiratory distress syndrome, severe pneumonia, COVID-19, and hypercapnic respiratory failure due to chronic obstructive pulmonary disease. We will mainly focus on well-conducted randomized controlled trials to determine whether

Concomitant Gastric Leak, Portal Vein Thrombosis, and Liver Abscesses Following Sleeve Gastrectomy: A Report of a Rare Case.

One of the most serious complications after sleeve gastrectomy (SG) is a postoperative leak. Early diagnosis and treatment are essential due to potential secondary complications, such as sepsis, septic shock, and death. Less commonly known and rare complications include portal thrombosis and liver abscesses, which have been reported in only a few cases. Here, we present a patient who developed a leak, portal thrombosis, and concomitant liver abscesses, requiring both surgical and medical management, ultimately leading to a favorable outcome. To our knowledge, this is the first case report of t

A novel case of Vibrio bacteremia in an immune-competent patient.

Vibrio Cholera, a gram negative bacterium, is notoriously known to cause diarrheal epidemics. The serotypes O1 and O139 are mainly responsible for the diarrheal outbreaks due to the enterotoxin they produce. This enterotoxin however seems to be protective against bacteremia and hence bacteremia is rarely encountered. We report a case of an immune-competent young female who initially presented with hypokalemic periodic paralysis secondary to acute on chronic diarrhea. She was discharged after rehydration and bounced back in septic shock and her blood cultures grew Vibrio Cholera identified on V

Viewpoint: Weak Scientific Basis for the Recommendation of Executive Summary of Surviving Sepsis Campaign Guidelines 2021.

UNLABELLED: The Executive summary of Sepsis 2021 was published recently, which for the first time, recommended that in septic shock, the vasopressor infusion should be commenced through a peripherally inserted venous catheter (PiVC) for up to 6 hours. We discuss the scientific basis for such a recommendation regarding the safety of vasopressor infusion through a peripherally inserted vascular catheter or the accepted duration. Pediatric Sigmoid Volvulus: A Report on Two Cases.

Sigmoid volvulus is a rare cause of intestinal obstruction in the pediatric age group. Rotation of the redundant sigmoid colon about its narrow mesenteric base results in vascular compromise and large bowel obstruction. Predisposing factors for sigmoid volvulus are Hirschsprung's disease, congenital anomalous fixation of the colon, and chronic constipation. Here, we report two cases of sigmoid volvulus in children with redundant sigmoid colon in the South Indian subcontinent. If it is not diagnosed in time, it may lead to serious complications such as gangrene, perforation, septic shock, and e

Calciphylaxis After Aortic Valve Replacement in a Patient with End-Stage Renal Disease.

In this study, we report a case of a patient on dialysis who presented necrotic lesions on the legs and penile ulceration 7 years after a mechanical aortic valve replacement. The diagnosis of calciphylaxis was not confirmed even after skin biopsy, and multidisciplinary management was not initiated until the patient was admitted with septic shock. Cardiovascular surgeons should be aware of warfarin-induced calciphylaxis, whose pathophysiology differs from that of atherosclerosis. Considering poor long-term survival of dialysis patients, mechanical valves should be reserved only for those patien

The first reported case of colonic infection caused by Candida glabrata.

Lower gastrointestinal tract infection caused by Candida species are rarely reported and, Candida albicans and tropicalis have been the only pathogens identified. We present the first documented case of candida colitis caused by Candida Glabrata in a 56-year-old man with a personal history of morbid obesity and bariatric surgery. The presenting symptoms were diarrhea, rectal bleeding and septic shock. Diagnosis was obtained by histological and microbiological study of the colonoscopy biopsies. Gastroenterologists should be aware of Candida as a cause of colonic infection. Fungal culture is the

An unusual cause of massive upper gastrointestinal bleeding-gastric mucormycosis.

Mucormycosis of the gastrointestinal tract is a life threatening infection most commonly seen in patients with severe immunosuppression. A 42-year-old male with history of choriocarcinoma was admitted to the intensive care unit with septic shock. He developed massive hematemesis requiring upper endoscopy which showed multiple deep gastric ulcers. Due to uncontrollable bleeding he underwent an emergent gastrectomy which revealed necrotic ulcers with evidence of angioinvasion in the ulcer bed with mucor organisms. The PCR revealed the mucor to be Mycotypha microspora which is extremely rare. We

Intestinal Tuberculosis Masquerading as Crohn's Disease? A Case of Disseminated Tuberculosis after Anti-TNF Therapy for Suspected Crohn's Disease.

Intestinal tuberculosis (ITB) and Crohn's disease (CD) very closely resemble each other in symptomatology, imaging, appearance, and pathology. While ITB is rare in the United States, its prevalence is significantly higher in endemic areas, thus presenting a diagnostic dilemma in immigrant populations from high-risk countries. This patient was diagnosed with CD and treated with anti-TNF agents after indeterminate screening for latent tuberculosis. He was then admitted with septic shock and intestinal perforation due to disseminated tuberculosis. This case demonstrates the importance the conside

Melioidosis in Brunei Darussalam.

Melioidosis continues to be a major health care problem in Brunei Darussalam. The age of patients, gender distribution, risk factors, and clinical presentations are similar to those reported from other countries in the region. The incidence of melioidosis was high during the wet months and in the Temburong district, which has the highest annual rainfall. In spite of adequate facilities for diagnosis and treatment, the mortality remains high (27%). Women and those presenting with septic shock had higher mortality. There is a case for making melioidosis a notifiable disease in Brunei Darussalam.

"The clock is ticking": the timely management of a painful skin rash in a seventy-year-old woman.

Necrotizing fasciitis is an uncommon but a potentially fatal condition and can affect any part of the body. Most patients have pre-existing conditions that render them susceptible to infection, although etiology is unclear. Diagnosis is primarily clinical and is often delayed because of the unfamiliarity of the condition among clinicians. Management consists of immediate resuscitation, early surgical debridement, and administration of broad spectrum intravenous antibiotics. We report a case of a 70 year old woman who presented with a painful erythematous rash, was admitted as a case of celluli

Necrotising fasciitis due to an infected sebaceous cyst.

The current case presents a patient who was admitted to our hospital with the diagnosis of cellulitis of the right groin. In the following days, the patient's condition deteriorated and developed a septic shock. Exploration in the operating room showed a necrotising fasciitis of the adductor muscles, with an infected sebaceous cyst in the inguinal crest as port d'entrée. After extensive surgical debridement, antibiotic therapy, haemodynamic and respiratory support, the patient recovered. Necrotising fasciitis is a rare but very lethal condition, which necessitates aggressive surgical therapy a

[Treatment of Fournier's gangrene].

Fournier's gangrene is a necrotizing fasciitis and soft-tissue infection of the perineal region. The clinical course of the disease, in many cases, is fulminant. This case presents a patient with symptoms of Fournier's gangrene that developed during hospitalization following a vehicular accident. The disease was diagnosed six days into hospitalization. The patient underwent numerous surgical procedures. Escherichia coli and Enterococcus faecalis were found in a culture of the surgical wound. Because of the progressing septic shock, the patient was admitted to the Intensive Care Unit. Treatment

Hiding in plain sight: H1N1 influenza presenting with respiratory failure and normal chest radiography.

Sepsis and respiratory failure are uncommon complications of infection with H1N1 influenza. In the majority of cases of H1N1-associated sepsis and respiratory failure found in the literature, evidence of pneumonia or the acute respiratory distress syndrome was seen on chest radiography. This case report describes the clinical characteristics of a patient who presented with septic shock and multi-system organ failure due to H1N1 influenza, but had normal chest radiography. Beyond describing a unique presentation of H1N1-related sepsis, this report highlights the need for clinicians to maintain

Life-threatening disseminated tuberculosis as a complication of TNF-α blockade in an adolescent.

Life-threatening disseminated tuberculosis developed in a 17-year-old girl who was treated with the TNF-α blocker adalimumab for refractory SAPHO syndrome. The patient presented to the emergency department with dyspnea and somnolence and within 2 h developed the clinical picture of a septic shock. In addition to this unusual presentation, she showed a complicated course with increasing cerebral granuloma formation in spite of adequate antimycobacterial treatment. Immune reconstitution after discontinuation of TNF blockade may contribute to this "paradoxical reaction." Possible implications for

Sensitive, specific, predictive… statistical basics: how to use biomarkers.

Biomarkers are frequently used in critically ill patients, especially during inflammatory and/or infectious diseases such as severe sepsis and septic shock. The rationale of when to measure laboratory parameters, which marker may be useful, and how to interpret the results is not well defined. Terms like sensitive, predictive, or significant to describe the capabilities of specific markers are often mixed up or misused, which may have fatal consequences regarding diagnosis and treatment. This review reflects some statistical basics with clinical examples, showing possibilities as well as limit
