Analysis of Functional Recovery and Subjective Well-Being after Arthroscopic Rotator Cuff Repair.

Background: Rotator cuff tears are common causes of functional shoulder instability and often lead to arthroscopic rotator cuff repair. A well-programmed rehabilitation leads to successful tendon healing, positive functional recovery and subjective well-being (SWB). Objective: To evaluate the changes in shoulder functioning and SWB pre-, post-outpatient rehabilitation and after one-month follow-up. Materials and Methods: A total of 44 patients were assessed three times: at the beginning (six weeks' post-surgery), at the end of outpatient rehabilitation (2-3 weeks) and one month after rehabilit

Comparison of surgical outcomes between rotator cuff repair with and without rotator interval capsular release for rotator cuff tears to prevent and improve postoperative stiffness: a meta-analysis.

BACKGROUND: A common complication of rotator cuff (RC) repair is a postoperative stiffness. Postoperative stiffness may develop and lead to inferior functional outcomes. Rotator cuff repair with additional rotator interval (RI) capsular release can be done to prevent postoperative stiffness in rotator cuff tears. However, routine rotator interval capsular release in rotator cuff repair is controversial. Therefore, we conducted a systematic review and meta-analysis to compare the postoperative outcomes between RC repair with and without RI capsular release for RC tears with and without concomit

Could superior capsule findings be used as a predictor for partial bursal-sided rotator cuff tears?

PURPOSE: The purpose of this study was to investigate the relationship between the superior capsule tear patterns and synovitis with subacromial pathologies, such as bursal-sided rotator cuff tear and subacromial impingement syndrome. METHODS: Fifty patients who underwent arthroscopic treatment for isolated bursal-sided tear were included in the study. Fifty more patients who underwent arthroscopic treatment for isolated Superior Labrum Anterior Posterior (SLAP) 2 lesion without pathology in the rotator cuff were included in the control group. Firstly, superior capsule tear and common synovit

Biomechanical evaluation of an arthroscopic transosseous repair as a revision option for failed rotator cuff surgery.

BACKGROUND: The number of revision rotator cuff cases is increasing. The literature is lacking guidance or biomechanical evaluation for fixation strength in a revision case scenario. Therefore, the aim of the study was to provide biomechanical data investigating primary fixation strength of a transosseous technique after anchor pullout failure of a single row reconstruction. It was hypothesized that an arthroscopic transosseous repair system as a procedure for rotator cuff revisions is providing equivalent stability compared to a primary single row suture anchor fixation due to change of fixat

Modeling a rotator cuff tear: Individualized shoulder muscle forces influence glenohumeral joint contact force predictions.

BACKGROUND: Rotator cuff tears in older individuals may result in decreased muscle forces and changes to force distribution across the glenohumeral joint. Reduced muscle forces may impact functional task performance, altering glenohumeral joint contact forces, potentially contributing to instability or joint damage risk. Our objective was to evaluate the influence of rotator cuff muscle force distribution on glenohumeral joint contact force during functional pull and axilla wash tasks using individualized computational models. METHODS: Fourteen older individuals (age 63.4 yrs. (SD 1.8)) were

Upper limb joint kinetics of three sitting pivot wheelchair transfer techniques in individuals with spinal cord injury.

STUDY DESIGN: Repeated measures design. OBJECTIVE: This study compared the upper extremity (UE) joint kinetics between three transfer techniques. SETTING: Research laboratory. METHODS: Twenty individuals with spinal cord injury performed three transfer techniques from their wheelchair to a level tub bench. Two of the techniques involved a head-hips method with leading hand position close (HH-I) and far (HH-A) from the body, and the third technique with the trunk upright (TU) and hand far from body. Motion analysis equipment recorded upper body movements and force sensors recorded their hand

2014 Neer Award Paper: neuromonitoring the Latarjet procedure.

BACKGROUND: We used intraoperative neuromonitoring to define the stages of the Latarjet procedure during which the nerves are at greatest risk. METHODS: Thirty-four patients with a mean age of 28.4 years were included. The Latarjet procedure was divided into 9 defined stages. Bilateral median and ulnar somatosensory evoked responses and transcranial motor evoked potentials from all arm myotomes were continuously monitored. A "nerve alert" was defined as averaged 50% amplitude attenuation or 10% latency prolongation of ipsilateral somatosensory evoked responses and transcranial motor evoked po

Arthroscopic release of shoulder internal rotation contracture in children with brachial plexus birth palsy.

In children with brachial plexus birth palsy, the unopposed contraction of the shoulder internal rotators and adductors, secondary to weakness of the external rotators and abductors, leads to internal rotation contracture of the shoulder joint. Latissimus dorsi and/or teres major tendon transfers combined with open musculotendinous lengthening can improve shoulder function. Arthroscopic release of the anterior glenohumeral ligaments, capsule and upper intra-articular subscapularis tendon, with or without tendon transfer(s), can also be performed in young children to restore external rotation a

Management of failed arthroscopic rotator cuff repair.

Most patients experience pain relief and functional improvement following arthroscopic rotator cuff repair, but some continue to experience symptoms postoperatively. Patients with so-called failed rotator cuff syndrome, that is, with continued pain, weakness, and limited active range of motion following arthroscopic rotator cuff repair, present a diagnostic and therapeutic challenge. A thorough patient history, physical examination, and imaging studies (eg, plain radiography, MRI, magnetic resonance arthrography, ultrasonography) are required for diagnosis. Management is determined based on pa

Reverse anatomy shoulder replacement: comparison of two designs.

The aim of this study was to use a biomechanical model of the shoulder to examine the characteristics of two concepts of reverse anatomy implant; those that preserve the natural glenohumeral centre such as the Bayley-Walker (B-W) replacement and those medializing it such as Delta. A biomechanical model of the shoulder was used to evaluate the biomechanical characteristics of these prostheses. The moment arms of the shoulder muscles were predicted for the implanted models over a wide workspace and were compared to those of natural anatomy. The deltoid muscle moment arms through almost all of th

Experience of shoulder dystocia in a district general hospital: what have we learnt?

Shoulder dystocia is an obstetric emergency that currently affects 0.6% of all deliveries in the UK. This potentially serious obstetric emergency requires early recognition and prompt involvement of appropriately trained personnel to deliver the baby safely and without delay. Failure to do so may result in significant neonatal and maternal morbidity, with ensuing litigation. As part of clinical governance, hospital trusts are under pressure to continually improve the quality of service provided to their patients. Meticulous and accurate documentation is the crucial first step. The second step

Treatment option in a SLAP-related ganglion cyst resulting in suprascapular nerve entrapment.

INTRODUCTION: Our report shows a rare case of suprascapular nerve palsy due to a SLAP-related ganglion cyst resulting in isolated weakness of the infraspinatus muscle. CASE REPORT: We report on a 31-year old volleyball player with severe shoulder pain. A ganglion cyst was excised in an open procedure and was completely resolved in a postoperative magnetic resonance imaging (MRI). But the patient again had pain and disability 7 months after this procedure. A renewed MRI scan showed a cystic mass in the spinoglenoid notch. An electromyography revealed an isolated lesion of the suprascapular ner

Open surgical treatment of anterior glenohumeral instability: an historical perspective and review of the literature. Part I.

Anterior glenohumeral instability is an undesirable result of trauma to the shoulder. Several surgical treatments for this condition have been developed, beginning in the early years of the twentieth century. Although these procedures were usually popular at their inception, many of them have fallen out of favor as more information has been acquired concerning the long-term results and complications of their use. Often successful in preventing recurrent instability, these earlier procedures also often led to a loss of external rotation, and consequently, function. Newer procedures that aim to

Glenoid deformity secondary to brachial plexus birth palsy.

The association between internal rotation contracture secondary to brachial plexus birth palsy and deformity and posterior dislocation of the glenohumeral joint has been known for a long time. The precise nature of these deformities and their pathogenesis, however, remain unclear. Twenty-five children, ranging in age from 1.5 to 13.5 years, had an operation to release an internal rotation contracture secondary to brachial plexus birth palsy; eleven had a latissimus dorsi transfer to augment external rotation power as well. Arthrograms were made intraoperatively in order to clarify the patholog
