Craniosynostosis.

Craniosynostosis is a premature pathologic fusion of one or more cranial vault sutures that leads to abnormal shape of the skull. The fused sutures lead to restricted growth in some areas and compensatory bossing in other areas. The head may assume different shapes depending upon the site and timing of the abnormally fused suture. The exact cause of this suture pathology is still unknown, but the local dura, cranial base and the fibroblast growth factors seem to influence this. The diagnosis rests on clinical examination and confirmation is generally on the computed tomography scan. The need f

Giant suprasellar Rathke's cleft cyst mimicking craniopharyngioma: implications for a spectrum of cystic epithelial lesions of ectodermal origin.

Cystic epithelial lesions such as Rathke's cleft cysts (RCCs) and craniopharyngiomas may be difficult to distinguish on a clinical, radiographic, and sometimes histopathological basis. We describe a case of a giant 6.5 cm suprasellar cystic lesion that was presumed to be a craniopharyngioma based on the neuroimaging findings. The lesion extended from the anterior skull base and sella turcica to the lateral ventricle and sylvian fissure resulting in obstructive hydrocephalus. Complete surgical removal of the suprasellar lesion was achieved using an extended frontotemporal transbasal skull base

Cystic cavernous malformation of the cerebellopontine angle: case report and literature review.

BACKGROUND: Cavernous malformations (CMs) in the cerebellopontine angle (CPA) are rare, and most of such CMs reported to date are solid and extend from the internal auditory canal into the CPA. In contrast, cystic CMs that arise in the CPA and do not involve the internal auditory canal and dura of the skull base are extremely rare. CASE PRESENTATION: A 50-year-old man presented with vertigo and progressive hearing loss in the right ear. MRI examination revealed a lesion in the CPA with solid and cystic components. Surgery was performed. Well-circumscribed adhesion to cranial nerves, the cereb

Comparative study of two autogenous graft techniques using piezosurgery for sinus lifting.

PURPOSE: Maxillary sinus lifting is a technique, in which, a possible complication is sinus membrane perforation. The aim of this study was to compare two techniques using ultrasound surgery to perform autogenous graft for maxillary sinus lifting. METHODS: Ten rabbits were used in the study, one of them did not undergo surgery. The other nine rabbits had their maxillary sinuses filled with autogenous bone grafts collected from the external skull diploe in particulate form on the right side, and shaved on the left side, both with ultrasonic device. Data on bone density in left and right maxill

Rapid spontaneous redistribution of acute epidural hematoma : case report and literature review.

Acute epidural hematoma (AEDH) occurring as a result of traumatic head injury constitutes one of the most critical emergencies in neurosurgery. However, there are only several reports that show the rapid disappearance of AEDH without surgical intervention. We suggest redistribution of hematoma through the overlying skull fractures as the mechanism of rapid disappearance of AEDH. A 13-year-old female fell from a height of about 2 m and presented with mild headache. A computed tomography (CT) scan performed 4 hours after the injury revealed an AEDH with an overlying fracture in the right tempora

Surgical treatment of trigeminal schwannomas.

Schwannomas that arise from the trigeminal nerve are rare, but this nerve is the second most frequent intracranial site of schwannoma occurrence next to the vestibular nerve. The advent of microsurgical techniques and skull-base approaches has greatly enhanced the surgical management of these tumors, and outcomes have improved markedly. This report documents 18 cases of histologically verified schwannomas that arose from the trigeminal nerve and were treated surgically in our clinic between January 1992 and July 2005. The patients were ten women and eight men of age 39.7 years (range, 22-62 ye

Craniosynostosis.

Skull deformity in infants continues to be a diagnostic and therapeutic challenge. Deformational plagiocephaly is a common and somewhat benign cause of skull deformity in infants that must be distinguished from the more serious craniosynostosis, which occurs alone or as a syndrome. Examining an infant's head from above can help the physician distinguish true lambdoid synostosis from deformational plagiocephaly. In infants with lambdoid synostosis, the posterior bossing is in the parietal area contralateral to the flat part of the head. Deformational plagiocephaly causes frontal bossing ipsilat

A Ser252Trp [corrected] substitution in mouse fibroblast growth factor receptor 2 (Fgfr2) results in craniosynostosis.

Apert syndrome (AS) is one of the most severe craniosynostoses and is characterized by premature fusion of craniofacial sutures. Mutations of either Ser252Trp or Pro253Arg in fibroblast growth factor receptor 2 (FGFR2) are responsible for nearly all known cases of AS. Here we show that mutant mice carrying the activation mutation, Ser252Trp [corrected] which corresponds to Ser252Trp in human FGFR2, have malformations mimicking the skull abnormalities found in AS patients. Mutant mice (Fgfr2(250/+)) are smaller in body size with brachycephaly and exhibit distorted skulls with widely spaced eyes

Pneumocephalus associated with ethmoidal sinus osteoma--case report.

A 35-year-old female suffered sudden onset of severe headache upon blowing her nose. No rhinorrhea or signs of meningeal irritation were noted. Computed tomography (CT) with bone windows clearly delineated a bony mass in the right ethmoid sinus, extending into the orbit and intracranially. Conventional CT demonstrated multiple air bubbles in the cisterns and around the mass in the right frontal skull base, suggesting that the mass was associated with entry of the air bubbles into the cranial cavity. T1- and T2-weighted magnetic resonance (MR) imaging showed a low-signal lesion that appeared to

[Possible indications for temporalis muscle flaps in plastic-reconstructive surgery of the head and neck].

BACKGROUND: In reconstructive plastic surgery, the temporalis muscle has proven to be a reliable and versatile flap for a wide field of indications. The temporalis muscle is a flat, fanshaped muscle with a generous blood supply and innervation from the trigeminal nerve. METHODS AND RESULTS: Based on our own experiences in a total of 25 cases (12 cases involving muscle transposition for mouth reanimation, six cases involving stabilisation of a duraplasty including protecting the internal carotid artery in one case, four cases involving reconstruction of the orbital floor or the base of the sku

A craniometric study of bicoronal craniosynostosis (BCC).

We compared 20 cases of bicoronal craniosyntosis (BCC) using the linear and angular values of the normal statistical material for the same age and sex. The skull in BCC is significantly shortened and flattened but with normal width. The height of the frontal arch (FNBR) is significantly decreased. The base of the skull is depressed with angle AB2 significantly increased, and angle NTNO significantly decreased and, even in some cases, with negative angular values. This is inducing a sinking of LEA (length from N to T), and in some cases LEA is lower than the line LNO (distance from N to O). The

Teratologic evaluation of dermally applied diethylene glycol monomethyl ether in rabbits.

Diethylene glycol monomethyl ether (DEGME) was applied to the skin of pregnant rabbits on Days 6 through 18 of gestation in order to assess the fetotoxic and teratogenic potential by the dermal route. Rabbits were treated with 0, 50, 250, or 750 mg/kg/day of DEGME, and fetuses were examined for external, visceral, and skeletal alterations. Topical application of the highest dose, 750 mg/kg/day, produced slight embryotoxicity, fetotoxicity, and toxicity in the maternal animal. Maternal effects were characterized by decreased weight gain and a concurrent physiologic decrease in red blood cells a

Craniosynostosis in vitamin D-resistant rickets. A mouse model.

Craniosynostosis and associated craniofacial deformities, such as frontal bossing, often occur as symptoms of vitamin D-resistant rickets in children. Similar skull deformities develop in mice with X-linked dominant hypophosphatemia, the most common form of vitamin D-resistant rickets. These mice have a short, wide, high neurocranium, which suggested an inhibition of coronal suture growth. To study this question, we compared histologically the postnatal development of the coronal sutures in normal and hypophosphatemic mice between 1 and 13 weeks of age. Premature fusion of the coronal suture o

[Computer tomography in emergency diagnosis (author's transl)].

The results of 150 examinations by computed tomography in emergency situations are described (about 5% of all CT examinations). There were 80 examinations of the skull, 65 of the abdomen and five of the mediastinum. The results are shown in two tables and include the diagnosis of bleeding, infarcts and inflammatory lesions (abscesses, encephalitis, acute pancreatitis). The morphologic and densitometric findings are compared with the literature and with other imaging procedures. Computer tomography has assumed the most important place in the emergency diagnosis of the skull; as regards the abdo

Ossifying Fibroma of Non-odontogenic Origin: A Fibro-osseous Lesion in the Craniofacial Skeleton to be (Re-)considered.

In the cranio-facial skeleton, a heterogeneous group of well characterized fibro-osseous lesions can be distinguished. Whereas fibrous dysplasia can affect any skeletal bone, ossifying fibroma and cemento-osseous dysplasia exclusively develop in the cranio-facial region, with most subtypes restricted to the tooth bearing areas of the jaws. Herein we present a series of 20 fibro-osseous lesions that developed mostly in the frontal bone and in the mandible, presenting as expansile intramedullary tumors with a unique histologic appearance and an indolent clinical course. We provide evidence that

Different types of traumatic lesions on mediaeval skeletons from archaeological sites in Varese (North Italy): diagnosis on ante mortal fractures using macroscopic, radiological and CT analysis.

This is the medical history of three skeletons from archaeological sites in the province of Varese that are interesting because they show signs of traumatic injury to the skull and postcranial skeleton. Fractures of two of the skeletons were analysed by CT scan and radiological investigations to understand the extent of the damage and to hypothesise the cause. The study of lesions on ancient human remains is important to understand the degree of interpersonal violence within ancient communities and to reconstruct the dynamics of a violent episode.

Craniofacial reconstruction following oncologic resection.

The ability to reliably reconstruct complex and sizable wounds has decreased the morbidity of skull base surgery substantially, preventing major complications and allowing treatment of tumors previously considered inoperable. Addressing facial nerve function with static and dynamic procedures as well as fabrication of craniofacial prostheses to replace delicate facial landmarks has further increased surgeons' ability to restore the appearance and function of the face.
