Morphometry of the thoracolumbar vertebrae in sickle cell disease.

Patients with sickle cell disease (SCD) who have deformities and vertebral fractures due to osteoporosis may require surgery. Spinal surgeons must become familiar with the vertebral morphometry of patients with SCD and to that aim we have examined the morphometry of the thoracolumbar spine in these patients. A cohort of 100 patients with SCD was examined using plain thoraco lumbar anteroposterior/lateral radiographs and dual energy X-ray absorptiometry (DEXA). Vertebral morphometry (vertebral body diameters, pedicle, spinal canal and deformity) was assessed for different age groups. Results we

Phantom limb pain: are cutaneous nociceptors and spinothalamic neurons involved in the signaling and maintenance of spontaneous and touch-evoked pain? A case report.

A patient suffered multiple fractures of the right leg and a left brain-stem infarction involving the anterolateral fasciculus of the central nociceptive system following multiple trauma. Later, the right leg was amputated, resulting in spontaneous and touch-evoked phantom pain and mechanical stump allodynia. However, quantitative sensory testing revealed considerable impairment of sensations normally mediated by cutaneous nociceptors and central spinothalamic systems on the right body side, including the stump but nearly intact touch and vibration senses. Quantitative assessment of peripheral

Transpedicular screw-rod fixation of the lumbar spine: operative technique and outcome in 104 cases.

A total of 104 patients underwent transpedicular spinal instrumentation using the Cotrel-Dubousset (71 cases) or the Texas Scottish Rite Hospital (33) screw-rod system. Surgery was performed for lumbar vertebral column instability secondary to fractures (28 cases), spondylolisthesis (29), tumors (four), vertebral osteomyelitis (two), or postoperative causes (41). Pseudoarthrodesis due to failure of a prior fusion was present in 37 cases. The 55 men and 49 women (mean age 47 years, range 18 to 87 years) all presented with severe back pain. Signs or symptoms of neural compression were noted in 9

Management of Acute Subaxial Trauma and Spinal Cord Injury in Professional Collision Athletes.

Sports-related acute cervical trauma and spinal cord injury (SCI) represent a rare but devastating potential complication of collision sport injuries. Currently, there is debate on appropriate management protocols and return-to-play guidelines in professional collision athletes following cervical trauma. While cervical muscle strains and sprains are among the most common injuries sustained by collision athletes, the life-changing effects of severe neurological sequelae (ie, quadriplegia and paraplegia) from fractures and SCIs require increased attention and care. Appropriate on-field managemen

Seatbelt Syndrome in Children.

The seatbelt syndrome describes an injury pattern infrequently seen in restrained passengers in motor vehicle collisions. It occurs when sudden deceleration forces coupled with compression of the lap belt around the abdomen causes abdominal wall bruising, intra-abdominal injuries, and spinal fractures. Infrequent and improper use of appropriate belt restraints in children has led to high risks for injury in this population. We describe a case of the seatbelt syndrome with the uncommon finding of an associated posttraumatic intestinal obstruction. We also review the literature on the prevalence,

[Drug-induced osteonecrosis of the jaw - not just bisphosphonates].

Metastatic bone disease and osteoporosis have a large impact on quality of life and are associated with the development of skeletal-related events (SREs), such as fractures and spinal cord compression. Pharmacologic managing of metastatic bone disease and osteoporosis typically involves antiresorptive agents such as bisphosphonates and RANKL inhibitors. Undesired adverse effects resulting from the use of these drugs include osteonecrosis of the jaw (ONJ). Dentoalveolar surgery, particularly tooth extraction, appears to increase the risk of ONJ. However, spontaneous events were also documented.

Bone And Soft Tissue Changes In Patients With Spinal Cord Injury And Multiple Sclerosis.

In patients with spinal cord injury and multiple sclerosis, deterioration of body composition (changes in bone, fat and muscle mass) is associated with increased risk for diseases such as coronary artery heart disease, non-insulin dependent diabetes mellitus, lipid metabolism abnormalities, and osteoporotic fractures in these patients. Immobility leads to a changing pattern of loading in the paralyzed areas, and secondary alteration in structure. However, bone and soft tissue changes in these patients are usually neglected. The purpose of this article is to update on the pathophysiological mec

Spinal cord compression secondary to kyphosis associated with radiation therapy for metastatic disease.

Three patients were treated for pathologic fractures of the thoracolumbar spine causing progressive neurologic deficit. An anterior decompression with partial removal of the diseased vertebra was performed with improvement of neurologic function in each patient. Active tumor was not found, but necrotic bone with collapse and secondary kyphosis were the causes of anterior compression of the spinal cord. In two patients, the spine was stabilized with an anterior fibular strut graft that initially provided stability; later collapse progressed and deformity recurred. In the third patient, stabiliz

Management of fresh odontoid fractures using posterior C1-2 fixation without fusion: a long-term clinical follow-up study.

OBJECTIVE: Posterior C1-2 fixation without fusion makes it possible to restore atlantoaxial motion after removing the implant, and it has been used as an alternative technique for odontoid fractures; however, the long-term efficacy of this technique remains uncertain. The purpose of the present study was to explore the long-term follow-up outcomes of patients with odontoid fractures who underwent posterior C1-2 fixation without fusion. METHODS: A retrospective study was performed on 62 patients with type II/III fresh odontoid fractures who underwent posterior C1-2 fixation without fusion and

Percutaneous vertebroplasty for osteoporotic compression fractures: quantitative prospective evaluation of long-term outcomes.

PURPOSE: Osteoporotic vertebral compression fractures may cause debilitating pain that lasts for weeks or months, and which is often neither quickly nor completely relieved by conventional conservative therapy. Previous retrospective studies have suggested significant and nearly immediate pain relief, as well as rapid and sustained functional recovery, after percutaneous polymethylmethacrylate vertebroplasty (PPV). This prospective, quantitative study with long-term follow-up was designed to evaluate the safety and efficacy of PPV as a new treatment for patients with osteoporotic vertebral bod

Posterior-Only Vertebral Column Resection for Neglected Post-traumatic Thoracolumbar Fracture-Dislocation With Neurological Recovery: A Case Report.

Severe neglected post-traumatic thoracolumbar fracture-dislocation may result in rigid kyphotic deformity, sagittal imbalance, chronic pain, and delayed neurological compromise. We report the case of a middle-aged patient who presented with symptomatic rigid thoracolumbar kyphosis three years after an inadequately treated vertebral fracture. Radiological evaluation demonstrated a fixed angular deformity with translational instability and significant sagittal imbalance. The patient underwent single-stage, posterior-only, vertebral column resection with long-segment pedicle screw fixation and ci

Malformed vertebrae: a clinical and imaging review.

A variety of structural developmental anomalies affect the vertebral column. Malformed vertebrae can arise secondary to errors of vertebral formation, fusion and/or segmentation and developmental variation. Malformations can be simple with little or no clinical consequence, or complex with serious structural and neurologic implications. These anomalies can occasionally mimic acute trauma (bipartite atlas versus Jefferson fracture, butterfly vertebra versus burst fracture), or predispose the affected individual to myelopathy. Accurate imaging interpretation of vertebral malformations requires k

Closed internal degloving injury with conservative treatment.

In the emergency department, we frequently manage patients with multiple contusions and bruise over the trunk without severe injuries. Emergency department discharge is a common option for these patients, and we may neglect the existence of closed internal degloving injury, which is a soft tissue injury with pelvic trauma, combining the subcutaneous tissue torn away from the underlying fascia followed by a cavity being filled with hematoma and liquefied fat created in the next few days (Harefuah 2006;145:111-3:66, J Trauma 1997;42:1046). We report the unusual occurrence of this entity in an 18

Investigation of thoracolumbar T12-L1 burst fracture mechanism using finite element method.

A finite element model of the T12-L1 motion segment was subjected to dynamic vertical impact to investigate vertebral burst fracture mechanism at the thoracolumbar junction. A rigid ball was directed vertically towards a rigid plate fixed on top of the T12 vertebral body to simulate the axial impact. The results show that upon impact, the T12 vertebra exhibited a vibratory motion. At its maximum compression, the endplates bulged towards their vertebral bodies. The central parts of the endplates adjacent to the nucleus experienced the highest effective stress, and localized stress concentration

Delayed vertebral collapse with neurological deficits secondary to osteoporosis.

Delayed vertebral collapse after osteoporotic spinal fractures may cause progressive kyphosis, neurological deficits, and chronic back pain. We treated 14 consecutive patients with posterolateral decompression and posterior reconstruction and followed them over a period ranging from 24 to 54 months. The mean age was 67 (range: 62-72) years and the fracture level was T12 and L1. Seven patients were graded as Frankel stage C and seven as stage D. The mean segmental kyphotic angle was 22.6 degrees (7-29 degrees ) preoperatively, 4.4 degrees (1-6 degrees ) postoperatively, and 6.8 degrees (2-15 de

Combined effects of compression and hypotension on nerve root function. A clinical case.

Previous animal experiments suggest that mild compression may increase susceptibility of nerve roots to the effects of hypotension. The authors report the case of a patient with an unstable L2 burst fracture whose motor skills and senses were intact. During fracture reduction and spinal distraction, sensory-evoked potentials were recorded from the epidural space after right and left femoral and tibial nerve stimulation. Induced hypotension was used during the surgery. All responses were normal at the outset of the surgery. With hypotension, a marked drop in the amplitude of the right femoral e

Challenges with properly positioning children treated with a hip spica cast: Does the vehicle or cast type matter?

OBJECTIVE: Children treated with a hip spica cast for developmental dysplasia of the hip (DDH) or femur fractures often experience difficulty fitting properly within conventional child safety seats (CSS), potentially compromising safety. While prior laboratory crash studies have evaluated the crash performance of casted pediatric surrogates, these investigations have typically used a standardized test bench that does not represent the spatial constraints and seating geometry of modern passenger vehicles. The objective of the current study was to evaluate the ability to properly restrain pediat
