Pacemaker-induced apnoea in a patient with cervical medullary trauma: a case report.

BACKGROUND: Phrenic nerve capture is a well-known complication of pacemaker (PM) implantation. In contrast, vagus nerve stimulation resulting in apnoea has not been previously described. We report a unique case of pacing-induced apnoea, likely due to stimulation of vagal afferents during atrial lead capture. CASE SUMMARY: A 65-year-old man was admitted following a C3-C4 spinal cord injury resulting in tetraplegia. The patient developed complete third-degree heart block followed by asystolic cardiac arrest, complicated by ventricular tachycardia during cardiopulmonary resuscitation. After retur

Avoiding Orthostatic Intolerance During Early Ambulation After Total Knee Arthroplasty: The Impact of Lying-to-Standing Time.

BACKGROUND: Orthostatic intolerance (OI), characterized by dizziness, blurred vision, syncope, can occur during ambulation after total knee arthroplasty (TKA) causing delayed rehabilitation and hospital discharge. Standardized guidelines to prevent OI during early ambulation are lacking. This study aimed to provide evidence regarding the impact of postural transition timing or lying-to-standing time (LTST) on OI during early postoperative ambulation following TKA. METHODS: We evaluated 120 patients undergoing unilateral primary TKA for OI with varying LTST. Patients with significant comorbidi

Discrimination of ischemic versus hemorrhagic stroke type by presenting symptoms or signs: A systematic review and meta-analysis.

BACKGROUND: Valid discrimination of ischemic and hemorrhagic stroke relies exclusively on neuroimaging. Novel biomarkers in acute stroke present an opportunity to reconsider combination diagnostic approaches with standardized clinical assessment. Our systematic review evaluates diagnostic properties of acute clinical features in determining primary stroke etiology. METHODS: We conducted a systematic review and meta-analysis according to PRISMA guidelines. PubMed and EMBASE were searched from inception to 6th March 2025. Eligibility criteria included cohort, cross-sectional, case-control, or r

Comparative Analysis of Leadless Versus Transvenous Pacemakers in Non-Heart Failure Patients: A Multicenter US Retrospective Study.

BACKGROUND: Despite leadless pacemakers (LPMs) showing promise, real-world data comparing them to transvenous pacemakers (TV-VVI) are insufficient and often contradictory, especially in patients without major comorbidities like heart failure. OBJECTIVES: Comparing LPMs with TV-VVIs in real-world patients without HF. METHODS: The TriNetX database (Jan 2015-June 2023) was queried to include adults receiving their first single-chamber ventricular pacemaker for heart block, bradycardia, sick sinus syndrome, atrial fibrillation, or syncope, excluding those with prior pacemakers, heart failure or

Characteristics, Management, and Outcomes of Hospitalized Patients with Orthostatic Hypotension.

Orthostatic hypotension (OH) is a common inpatient condition associated with falls, syncope, and mortality. However, standardized approaches for inpatient management of OH are lacking and may vary across clinical specialties. In this retrospective observational cohort study, we reviewed the electronic medical records of patients admitted to Beth Israel Deaconess Medical Center between April 1, 2015 and June 1, 2021 with a diagnosis of OH or medication-related hypotension. Variables of interest included admitting service, presenting symptoms, suspected etiology, and management. Among the 400 in

Heart rate optimization to reduce tricuspid regurgitation in patients with atrial fibrillation and relative bradycardia: A case report.

UNLABELLED: A 74-year-old male with end-stage renal disease was referred for treatment of syncope and hypotension episodes during dialysis. The patient had a history of sick sinus syndrome that was managed with a VVI pacemaker, maintaining a heart rate of approximately 60 bpm, due to atrial fibrillation. Transthoracic echocardiography revealed massive tricuspid regurgitation (TR), which was identified as a significant contributor to the patient's symptoms. Surgical intervention for TR was initially considered, however echocardiographic examination with pulse Doppler of the tricuspid inflow wav

Deep transcranial magnetic stimulation for adolescents with treatment-resistant depression: A preliminary dose-finding study exploring safety and clinical effectiveness.

BACKGROUND: Transcranial magnetic stimulation (TMS) is an intervention for treatment-resistant depression (TRD) that modulates neural activity. Deep TMS (dTMS) can target not only cortical but also deeper limbic structures implicated in depression. Although TMS has demonstrated safety in adolescents, dTMS has yet to be applied to adolescent TRD. OBJECTIVE/HYPOTHESIS: This pilot study evaluated the safety, tolerability, and clinical effects of dTMS in adolescents with TRD. We hypothesized dTMS would be safe, tolerable, and efficacious for adolescent TRD. METHODS: 15 adolescents with TRD (Age,

Intracoronary thrombolysis combined with drug balloon angioplasty in a young ST-segment elevation myocardial infarction patient: A case report.

BACKGROUND: The combination of acute ST-segment elevation myocardial infarction (STEMI) and gastric ulcers poses a challenge to primary percutaneous coronary intervention (PPCI), particularly for young patients. The role of drug-coated balloons (DCBs) in the treatment of de novo coronary artery lesions in large vessels remains unclear, especially for patients with STEMI. Our strategy is to implement drug balloon angioplasty following the intracoronary administration of low-dose prourokinase and adequate pre-expansion. CASE SUMMARY: A 54-year-old male patient presented to the emergency departm

The European Voice of the Patient living with pulmonary hypertension associated with interstitial lung disease: Diagnosis, symptoms, impacts, and treatments.

Pulmonary hypertension (PH) adds a substantial disease burden, including higher mortality, when associated with interstitial lung disease (ILD), a severe, chronic, progressive condition. Yet little is known of the lived experiences, perspectives, priorities, and viewpoints of patients and carers living with PH-ILD. The Voice of the Patient meeting at the center of this qualitative research study aims to provide these difficult-to-obtain insights from a European perspective for the first time. The multistakeholder approach brought together four PH-ILD patients, three primary caregivers, two pat
