Bitemporal hypometabolism in Creutzfeldt-Jakob disease measured by positron emission tomography with [18F]-2-fluorodeoxyglucose.

It is well established that Creutzfeldt-Jakob disease (CJD) is caused by a slow infectious agent similar to the scrapie prion. However, the pathogenesis of this infection is poorly understood. Positron emission tomography (PET) was performed on a 54-year-old man with autopsy confirmed CJD using [18F]-2-fluorodeoxyglucose (FDG) and the Donner 280-crystal tomograph. Temporal lobe hypometabolism with hemispheric asymmetry was observed. These findings are similar to those previously obtained in PET-FDG studies of patients with clinically defined Alzheimer disease (AD). The similarities in the regi

Health awareness correlates with whole-brain gray matter volume in healthy adults.

As populations continue to age, interest in brain health is increasing. However, little is known about the relationship between health awareness and brain health in healthy adults. In this study, psychological questionnaire data from 184 participants were analyzed using multiple regression, with brain structural measures obtained by MRI as the dependent variables. Health awareness was positively associated with gray matter volume (GMV) in the whole brain, temporal lobe, and occipital lobe, after controlling for sex, age, education, and BMI. These associations remained significant after correct

[A case of young woman with intracranial gumma developed within 1 year after Treponema pallidum infection].

A 17-years-old woman visited the hospital due to convulsions. T2-weighted images showed high intensity areas in right temporal lobe and left frontal lobe. Enhanced T1-weighted images showed mass-like lesions on the dura mater. Based on mononuclear pleocytosis and a reactive fluorescent treponemal antibody-absorption test in the cerebrospinal fluid, neurosyphilis and intracranial gumma were diagnosed, and antibiotic therapy was initiated. After treatments, the high intensity areas improved, and she had no recurrence of symptoms or MRI images. Intracranial gumma usually develops in tertiary syph

Headache attack followed by rapid disease progression in pediatric moyamoya disease--how should we manage it?

CASE REPORT: A 4-year-old female was presented at our hospital with frequent right frontal headache attack. She was diagnosed with moyamoya disease and was conservatively followed up. One year later, the frequency of headache gradually decreased. However, follow-up MR imaging revealed that the disease stage markedly progressed in the right side and cerebral infarction occurred in the temporal lobe with atrophy of the right frontal lobe. She underwent direct and indirect revascularization on the right side. CONCLUSION: Aware of this case, we would like to emphasize that headache may be one sub

[Amnestic syndrome after right temporo-occipital subcortical hemorrhage].

A case of right temporo-occipital subcortical hemorrhage with amnestic syndrome was reported. A 65-year-old woman presented with visual disturbance and headache. CT and MRI demonstrated a hemorrhagic lesion in the right occipital lobe involving parahippocampal gyrus. Neurological examination on admission revealed left homonymous hemianopsia and anterograde and retrograde amnesia. Neuropsychological examination revealed marked recent memory disturbance, but she had visual retention and verbal retention, and her value in WAIS (Wechsler Adult Intelligence Scale) was normal. After 40 days, she was

Executive Functions in Healthy Older Adults Are Differentially Related to Macro- and Microstructural White Matter Characteristics of the Cerebral Lobes.

Aging is associated with microstructural white matter (WM) changes. WM microstructural characteristics, measured with diffusion tensor imaging (DTI), are different in normal appearing white matter (NAWM) and WM hyperintensities (WMH). It is largely unknown how the microstructural properties of WMH are associated with cognition and if there are regional effects for specific cognitive domains. We therefore examined within 200 healthy older participants (a) differences in microstructural characteristics of NAWM and WMH per cerebral lobe; and (b) the association of macrostructural (WMH volume) and

Adjuvant Chemoradiation Therapy Versus Chemotherapy Alone for Resected Oligodendroglioma: A Surveillance, Epidemiology and End Results (SEER) Analysis.

OBJECTIVE: The benefit of postoperative adjuvant therapy for survival of oligodendrocyte glioma remains unclear. In this study, we compared the effect of chemoradiation therapy (CRT) and chemotherapy (CT) alone in patients who underwent resection. We aim to identify which adjuvant therapy provides more survival benefits. METHODS: We identified patients who underwent oligodendroglioma resection in the Surveillance, Epidemiology and End Results (SEER) database. A multivariate Cox regression analysis was used to evaluate the factors affecting survival rates. We used a propensity matching analysi

[Dide-Botcazo syndrome associated with Anton's syndrome after a cardioembolic infarction in the distribution of the bilateral posterior cerebral arteries].

Dide-Botcazo syndrome (Rev Neurol, 1902) is a unique neuropsychological syndrome, characterized by combinations of cortical blindness, amnesia, and topographical disorientation. We report 82-year-old right-handed man manifesting such syndrome associated with Anton's syndrome after a cardioembolic infarction in the distribution of the bilateral posterior cerebral arteries. The MRI study demonstrated recent extensive infarctions bilaterally in the occipital lobes and the medial temporal lobes, and thalamus. Following the resolution of unconsciousness and tetraparesis, the patient persistently pr

Regional excitability, not epileptic pathology, drives stimulation-evoked interictal spike increases.

OBJECTIVE: Interictal spikes have been proposed as a biomarker for both localizing seizure onset zones (SOZ) and tracking changes in seizure risk with neurostimulation in patients with drug-resistant epilepsy. Electrical stimulation can modulate spike rates acutely, and it has been proposed that measuring this modulation can help localize the SOZ. However, it is unclear whether stimulation-induced spike rate changes reflect epilepsy-specific pathology in the stimulated network or simply intrinsic regional excitability, which limits our understanding of their utility in epilepsy surgery plannin

A decade with anomic primary progressive aphasia.

Some patients with primary progressive aphasia (PPA) demonstrate only anomia. The lack of longitudinal observations of anomic PPA precluded us from determining whether progressive anomic aphasia was simply an early stage of semantic or logopenic variants, or a relatively independent variant. Herein, we report the 10-year clinical course of a patient with PPA who presented with pure anomic aphasia for 9 years. He is a right-handed man with anomia, who noticed word-finding difficulty at age 73. He was admitted to the hospital at age 77. On admission, the patient showed pure anomic aphasia with p

Prefrontal cortex, temporal cortex, and hippocampus volume are affected in suicidal psychiatric patients.

Suicide is a leading cause of death in America, with over 40,000 reported suicides per year. Mental illness is a major risk factor for suicidality. This study attempts to validate findings of volumetric differences from studies on suicidality. Psychiatric inpatients classified as having mildly severe or severe depression were separated into two groups: suicide attempted in the past two months (SA; n=20), non-suicidal control group (DA; n=20); these patients were all depressed and not significantly different for age, gender, race, marital status, education level, anxiety level, and substance ab

Trends in incidence of primary brain cancer in New Zealand, 1995 to 2010.

OBJECTIVE: Case-control studies have linked mobile phone use to an increased risk of glioma in the most exposed brain areas, the temporal and parietal lobes, although inconsistently. We examined time trends in the incidence rates of brain malignancies in New Zealand from 1995 to 2010. METHODS: Data from the New Zealand Cancer Registry was used to calculate incidence rates of primary brain cancer, by age, gender, morphology and anatomical site. Log-linear regression analysis was used to assess trends in the annual incidence of primary brain cancer; annual percentage changes and their 95% confi

Transient Global Amnesia Triggered by Migraine in a French Tertiary-Care Center: An 11-Year Retrospective Analysis.

OBJECTIVE: The etiology of transient global amnesia (TGA) remains unclear, and flow disturbances in the mesial temporal lobes secondary to venous congestion have been proposed as a potential cause. The occurrence of TGA during a migraine attack is a rare condition. METHODS: This 11-year retrospective study in one French center describes patients' characteristics, type of migraine, investigations, treatment with vasoconstrictor during the TGA/migraine attack, and outcome in patients who had TGA during a migraine attack. RESULTS: Among 8821 new patients, 6 cases of TGA occurring during a migra

Effects of regularity on the processing of sound omission in a tone sequence in musicians and non-musicians.

Numerous studies have reported that perceptual grouping affects the pre-attentive processing of sound omission in a sequence of tones. However, it remains unclear whether or not the perceptual grouping and musical experience affect the attentive processing of sound omission. To this end, we created a sequence of loud (L) and soft (S) tones grouped as 'LLSLLS…' and a random sequence of the L and S tones. The omission of the L tones was inserted pseudo-randomly in the random sequence, and there were two positions at which it was inserted. For within-group omission, the omission was after the fir

[Joint therapy of a dural arteriovenous fistula in the transverse-sigmoid sinus using surgical excision and direct sinus packing: a case report].

Combined therapy of direct sinus packing and surgical excision for intracranial dural arteriovenous fistula (dAVF) has not been reported in the literature. A 53-year-old male was admitted to our hospital due to sudden onset of seizure and consciousness disturbance. Plain CT scan showed subcortical hematoma in the right temporal lobe. Cerebral angiography revealed dAVF involved in the right transverse and sigmoid sinuses. The lesion was associated with retrograde venous drainage into the right cerebellum, temporal, and occipital lobes. Positron emission tomography (PET) showed typical findings

Selective hippocampal lesions yield nonspatial memory impairments in rhesus monkeys.

Monkeys with removals of medial temporal lobe (MTL) structures are widely recognized as valid models of human global anterograde amnesia, a syndrome that arises consequent to damage to a finite set of brain structures situated in the medial temporal lobe and/or medial diencephalon. However, a comparison of memory deficits in human and nonhuman primates with MTL damage has presented a long-standing puzzle. Whereas amnesic patients are impaired in learning object discrimination problems, monkeys with MTL damage are typically not. One possible explanation for this difference is that object discri

Detection of changes in synaptic density in amyotrophic lateral sclerosis patients using 18 F-SynVesT-1 positron emission tomography.

BACKGROUND AND PURPOSE: Synaptic loss is well established as the major correlate of characteristic and consistent pathology in amyotrophic lateral sclerosis (ALS). We aimed to assess the possible discriminant diagnostic value of 18 F-SynVesT-1 positron emission tomography (PET) as a marker of ALS pathology and investigate whether specific synaptic density signatures are present in ALS with different subtypes. METHODS: Twenty-one patients with ALS and 25 healthy controls (HCs) were recruited. All participants underwent 18 F-SynVesT-1-PET. Synaptic density between ALS and HCs and between differ
