Klebsiella sp.-related infectious spondylitis in a bearded dragon (Pogona vitticeps).

BACKGROUND: Spondylitis is an inflammation of the vertebrae that leads to a destructive process with exuberant new bone formation. Osteomyelitis can produce a distortion of the bone architecture, degenerative joint changes and ankyloses of adjacent vertebrae. In reptiles, intervertebral discs are absent, so the term discospondylitis is not used. In lizards, vertebral lesions have not been well studied. The present paper describes the first case of Klebsiella sp.-related spondylitis in a pet lizard (Pogona vitticeps). CASE PRESENTATION: A 2-year-old, female bearded dragon (Pogona vitticeps) wa

Nanoparticle strategies for cancer therapeutics: Nucleic acids, polyamines, bovine serum amine oxidase and iron oxide nanoparticles (Review).

Nanotechnology for cancer gene therapy is an emerging field. Nucleic acids, polyamine analogues and cytotoxic products of polyamine oxidation, generated in situ by an enzyme-catalyzed reaction, can be developed for nanotechnology-based cancer therapeutics with reduced systemic toxicity and improved therapeutic efficacy. Nucleic acid-based gene therapy approaches depend on the compaction of DNA/RNA to nanoparticles and polyamine analogues are excellent agents for the condensation of nucleic acids to nanoparticles. Polyamines and amine oxidases are found in higher levels in tumours compared to t

Clinical studies.

Recognizing that type 1 diabetes was characterized not only by insulin deficiency, but also by amylin deficiency, Cooper (Cooper, 1991) predicted that certain features of the disease could be related thereto, and he proposed amylin/insulin co-replacement therapy. Although the early physiological rationale was flawed, the idea that glucose control could be improved over that attainable with insulin alone without invoking the ravages of worsening insulin-induced hypoglycemia was vindicated. The proposal spawned a first-in-class drug development program that ultimately led to marketing approval b

Increase in haemoglobin concentrations in growth hormone deficient adults during human recombinant growth hormone replacement therapy.

OBJECTIVE: Recombinant human growth hormone is being used increasingly in the treatment of GH deficiency in adults. In addition to erythropoietin, insulin-like growth factor-I (IGF-I) has been demonstrated to stimulate hematopoiesis in animals, especially rodents. We have determined the effect of growth hormone (GH) administration on haematopoiesis. PATIENTS AND DESIGN: Seventeen patients (10 males and 7 females; mean age 47 +/- 2 years; range 30-59) with biochemically proven adult-onset GH deficiency (GHD) were treated with human recombinant growth hormone (rhGH) as replacement therapy for m

Defining the optimal spacing between repeat radioantibody doses in experimental models: is there an accurate measurement for hematopoietic recovery?

BACKGROUND: Single doses of radioantibody are effective at treating single cells or small clusters of cancer cells. However, large tumor masses require either multiple doses of radioantibody or a multimodal approach to therapy using two or more therapeutic agents. Timing of the second dose in a multiple cycle scheme or the second treatment in a multimodal protocol will depend on recovery from toxicity associated with the first treatment. METHODS: BALB/c mice were dosed with a maximal tolerated dose (MTD) of I-131-MN-14 anti-carcinoembryonic antigen immunoglobulin G (IgG) (250 microCi) or F(ab

Thyroid dysfunction in African trypanosomiasis: a possible role for inflammatory cytokines.

OBJECTIVE: Sleeping sickness (African trypanosomiasis) is an anthropozoonosis transmitted by the tsetse fly. The treatments of choice are the antiparasitic agents suramin and/or melarsoprol. Experimental infection of animals with Trypanosoma brucei results in inflammatory lesions in the pituitary and/or the thyroid gland. In biochemical terms, these animals have hypothyroidism. We evaluated the functional integrity of the hypothalamic-pituitary-thyroid axis in patients with African trypanosomiasis before, during and after specific therapy. DESIGN: Prospective, controlled, cross-sectional stud

Oculopharyngeal Muscular Dystrophy: From Molecular Pathogenesis to Gene Therapy.

Oculopharyngeal muscular dystrophy (OPMD) is a rare, late-onset, autosomal dominant muscular dystrophy caused by a trinucleotide GCN repeat expansion in the PABPN1 gene on chromosome 14q11.2. The disease is clinically characterised by progressive ptosis, dysphagia, and proximal limb weakness, with onset typically between the fifth and sixth decades. OPMD affects approximately 1:100,000 individuals in Europe, with substantially higher prevalence in specific founder populations, reaching 1:600 among Bukharan Jews and 1:1,000 in French Canadians. The expanded polyalanine tract in PABPN1 promotes

Emerging therapeutic strategies for mitochondrial DNA-related diseases.

Primary mitochondrial diseases (PMDs) are among the most common inherited metabolic disorders, affecting approximately 1 in 4,300 individuals. They result from pathogenic variants in mitochondrial DNA (mtDNA) or nuclear DNA (nDNA) that disrupt oxidative phosphorylation and lead to multisystem disease. Although advances in genomic testing have significantly improved diagnostic rates in PMDs, effective disease-modifying therapies remain limited. Therapeutic development increasingly focuses on mtDNA-targeted approaches because mtDNA variants are a major cause of disease and may offer opportunitie

Deaths in gene therapy of Duchenne muscular dystrophy and other diseases: Underlying mechanisms and mitigating strategies.

Duchenne muscular dystrophy (DMD) is a fatal muscle degenerating disease caused by dystrophin deficiency. Adeno-associated virus (AAV)-based gene therapy holds promise for restoring missing dystrophin and improving quality of life. Many clinical trials have been conducted or are ongoing. Despite compelling preclinical data, the full potential of AAV gene therapy remains to be established in DMD patients. Importantly, high-dose intravenous AAV administration has resulted in hospitalizations and several deaths in patients afflicted by DMD and other inherited diseases due to innate and adaptive i

Gene modification: Exploring the potential in treating kidney diseases.

Chronic kidney disease (CKD) is a leading cause of death worldwide. Currently available drugs slow but do not cure or prevent progression to end-stage kidney disease. Unfortunately, dialysis and kidney transplant not only pose severe lifestyle constraints and healthcare costs on patients but are also associated with side effects such as electrolyte abnormalities and increased susceptibility to infections. It is imperative to research new therapeutic strategies for the management of CKD. Technological advances in genetics and genomics in recent years have revealed the role that genetics plays i

Alternative conditioning regimens for hemoglobinopathy gene therapy: balancing efficacy, toxicity, and the next frontier.

Gene therapy for sickle cell disease and transfusion-dependent thalassemia is now widely available, yet its broader application is constrained by the need for effective and tolerable conditioning. This review examines the central role of conditioning in enabling durable engraftment of gene-modified hematopoietic stem cells, emphasizing the narrow therapeutic window between insufficient niche clearance and excessive toxicity. Busulfan remains the standard conditioning agent, supported by consistent engraftment and clinical efficacy, but its use is limited by acute and long-term toxicities, incl

Immunogenicity in Approved Adeno-Associated Virus-Based Gene Therapies: A Framework for a Tailored Assessment Strategy.

Adeno-associated virus (AAV)-based gene therapy has emerged as a promising approach for treating a variety of genetic disorders by delivering therapeutic genes to target tissues. There have been numerous publications reporting nonclinical assessments of immune responses to various AAV-based gene therapy products (GTPs). However, the immune response data from approved GTPs in humans have not been systematically reviewed, and the utility of the data generated in these clinical studies has not been evaluated. This manuscript is intended as a comprehensive review of the nonclinical and clinical im

Ocular gene therapy targeting retinal angiogenesis and vascular leakage: translational and clinical evidence from a systematic review and meta-analysis.

INTRODUCTION: Retinal diseases driven by pathological angiogenesis and vascular leakage, including neovascular age-related macular degeneration and diabetic retinopathy, impose substantial visual and treatment burdens because current anti-vascular endothelial growth factor (anti-VEGF) therapy requires repeated intravitreal administration. Ocular gene therapy can provide durable intraocular expression of therapeutic proteins and sustained pathway-level disease control. METHODS: This systematic review and meta-analysis was prospectively registered in the International Prospective Register of Sy

Gene-Agnostic Therapeutic Strategies for Inherited Retinal Diseases: Neuroprotection and Immunomodulation.

Background/Objectives: Inherited retinal diseases (IRDs) represent a genetically heterogeneous group of disorders caused by mutations in over 280 genes with more than 3100 identified variants. While gene-specific replacement therapies have achieved landmark success with voretigene neparvovec (Luxturna) for biallelic RPE65-associated retinal dystrophy, developing individual therapies for each genetic subtype remains impractical. This review examines gene-agnostic therapeutic approaches utilizing neuroprotection and immunomodulation that target common pathophysiological mechanisms shared across

Gene Therapy in Hemophilia: Clinical Advances, Immunological Challenges, and Emerging Therapeutic Perspectives.

Gene therapy is reshaping the therapeutic paradigm in hemophilia by enabling sustained endogenous clotting factor production after a single administration. This approach moves disease management beyond lifelong replacement therapy. While clinical trials have demonstrated marked reductions in bleeding rates and treatment burden, real-world implementation has revealed emerging complexities. These include interindividual variability in transgene expression reflected by a progressive reduction in circulating FVIII or FIX activity over time, uncertainty regarding the long-term durability of express

Gene Therapy for Infants and Children With Otoferlin-Related Auditory Neuropathy Spectrum Disorder.

After decades of research focused on hair cell regeneration and the use of stem cells to regenerate the sensory epithelium, gene therapy has now emerged as the first therapy to accomplish the previously impossible task of curing hearing loss. While the current application of gene therapy is narrow, limited to otoferlin-related auditory neuropathy, early results are promising, and the tremendous success of some of the very first children receiving otoferlin gene therapy inspires an era of new research. How to identify potential candidates for otoferlin gene therapy and how to integrate this the

Targeting Neurological Aspects of Mucopolysaccharidosis Type II: Enzyme Replacement Therapy and Beyond.

Mucopolysaccharidosis type II (MPS II) is a rare, pediatric, neurometabolic disorder due to the lack of activity of the lysosomal hydrolase iduronate 2-sulfatase (IDS), normally degrading heparan sulfate and dermatan sulfate within cell lysosomes. The deficit of activity is caused by mutations affecting the IDS gene, leading to the pathological accumulation of both glycosaminoglycans in the lysosomal compartment and in the extracellular matrix of most body districts. Although a continuum of clinical phenotypes is described, two main forms are commonly recognized-attenuated and severe-the latte

Recent advances in understanding and managing myasthenia gravis.

Autoimmune myasthenia gravis (MG) is a neuromuscular junction disorder marked clinically by fatigable muscle weakness and serologically by the presence of autoantibodies against acetylcholine receptors (AChRs), muscle-specific kinase (MuSK), or lipoprotein-related protein 4 (LPR4). Over the past few decades, the mortality of patients with MG has seen a dramatic decline secondary to evolving interventions in critical care and medical management. In the past 2 to 3 years, there have been several changes in standard of care for the treatment of MG. These changes include confirmation of the benefi
