Urethral Complications Post-Holmium Laser Enucleation of the Prostate: A Seven-Year Experience.

Background: Urethral strictures (US) and bladder neck contracture (BNC) are troublesome complications of transurethral surgery. We aimed to report the incidence, risk factors, and management of US and BNC post-holmium laser enucleation of the prostate (HoLEP) together with review of literature. Patients and Methods: A retrospective review of prospectively managed HoLEP patients in one institution between 2015 and 2021 was performed. The study included patients with complete follow-up of at least 1-year. Multivariate regression analysis for risk factors of US or BNC was performed. Appropriate s

Stochastic modelling of African swine fever in wild boar and domestic pigs: Epidemic forecasting and comparison of disease management strategies.

African swine fever (ASF), caused by the African swine fever virus (ASFV), is highly virulent in domestic pigs and wild boar (Sus scrofa), causing up to 100% mortality. The recent epidemic of ASF in Europe has had a serious economic impact and poses a threat to global food security. Unfortunately, there is no effective treatment or vaccine against ASFV, limiting the available disease management strategies. Mathematical models allow us to further our understanding of infectious disease dynamics and evaluate the efficacy of disease management strategies. The ASF Challenge, organised by the Frenc

Gastroschisis: Impact of Bedside Closure on Ventilator-Associated Outcomes.

AIM: In our practice, preformed silos are routine rather than reserved for difficult cases. We aimed to identify whether silo and bedside closure can minimize: general anesthetic (GA) exposure, need for intubation and ventilation, or days intubated for neonates with simple gastroschisis (SG). METHODS: After approval, patients were identified via the neonatal discharge log (April 2010 to April 2019). Data were collected by case-note review and analyzed with respect to GA, ventilation, and core outcomes. RESULTS: Of 104 patients (50 female, mean birth weight 2.43 kg, mean gestational age 36 +

Spontaneous Repositioning of Isolated Blow-In Orbital Roof Fracture: Could Wait and See Be a Strategy in Asymptomatic Cases?

BACKGROUND: Treatment of isolated blow-in orbital roof fractures is still debated due to their anatomical complexity and the potential ocular and neurological related injuries. Surgery is advised in symptomatic cases while there is still controversy regarding the preferred treatment for those patients asymptomatic. OBJECTIVE: To explore the suitability of a conservative management with close imaging follow-up in asymptomatic isolated blow-in orbital roof fractures. METHODS: A single-case experience has been integrated into a systematic review of the literature to support the discussion on th

Management of Skull Base Tumors in the Obstetric Population: A Case Series.

BACKGROUND: Neoplasms rarely present during pregnancy; however, increases in plasma volume, hormone release-induced growth, and tumor hypervascularity can cause rapidly progressive symptoms of varying severity, ranging from those not requiring intervention to those necessitating emergent operations. Here we describe an algorithm for the management of symptomatic neoplasms in the obstetric population and provide recommendations for surgical indications and timing. METHODS: Patients who presented to the skull base clinic at a large tertiary care hospital between 2010 and 2016 were reviewed to i

Management of resistant distal femur non-unions with allograft strut and autografts combined with osteosynthesis in a series of 22 patients.

INTRODUCTION AND PURPOSE: Challenges to the surgeon in managing cases of resistant non-union of the distal femur include poor bone stock, disuse osteopenia and joint contractures. Procedures varying from simple bone grafting to megaprosthesis revision have been described. We successfully managed such cases using our technique of combining cortical allograft struts to augment the defect in the femoral condyle coupled with autogenous iliac crest bone grafting and locking plate osteosynthesis. MATERIALS AND METHODS: Between April 2012 and May 2014, 22 patients who presented with resistant nonuni

Late surgical repair of a traumatic ventricular septal defect.

Ventricular Septal Defect (VSD) complicates approximately 1-5% of patients presenting with penetrating chest trauma, however not all VSDs are evident at the time of initial presentation to the emergency department. Acute closure of traumatic VSDs is indicated in patients with a large defect and haemodynamic compromise, however closure may be delayed in smaller defects in order to minimise operative time, reduce operative mortality and allow for recovery from the initial trauma. We describe the case of a previously healthy 23 year-old Caucasian man who presented in extremis following stab wound

Gender differences in patients referred for atrial fibrillation management to a tertiary center.

BACKGROUND: Atrial fibrillation (AF) ablation is less frequently performed in women than in men. Although the prevalence of AF is slightly higher in men, this does not fully account for the lower number of AF ablations performed in women. This study sought to examine the effect of gender on referral for AF and subsequent AF management. METHODS: Consecutive patients referred to our tertiary arrhythmia outpatient clinic for AF management were retrospectively analyzed. RESULTS: Of 264 patients referred, only 27% were women. Women were older than men (63 +/- 9 vs 58 +/- 11 years, P = 0.002), mor

Device and Data Access Considerations for Digital Health Studies Using Consumer-Grade Wearables: An Experience-Based Tutorial.

Consumer-grade wearables, including smartwatches, fitness trackers, and smart rings, are accessible tools for studying pharmacologic responses in real-time, monitoring participant health in decentralized clinical trials, optimizing outcome measures, and delivering digital interventions that promote behavior change. However, incorporating consumer wearables into clinical studies is not as straightforward as simply purchasing devices and providing them to participants. It requires engaging with a complex engineering ecosystem with multiple investigator decisions on device selection, associated d

Premyopia Management in China: From Biometrics to Clinical Management.

Myopia represents a major global public health challenge, with prevalence rising particularly among children in China, imposing a substantial socioeconomic burden. Premyopia, defined by the International Myopia Institute as spherical equivalent refraction ≤ +0.75 D and >-0.50 D, represents a pivotal intervention window. This review summarizes evidence on premyopia management in China, including (1) biometric predictors such as hyperopic reserve, axial length (AL), and AL to curvature radius ratio (AL/CR ratio); (2) interventions including outdoor time, optical interventions, low-concentration

A Review of the Current Status of Antiviral Drugs Targeting Human Immunodeficiency Virus, Hepatitis and Others.

Antiviral drugs are pivotal in the prevention and management of infections caused by viruses, ranging from common cold viruses to life-threatening pathogens like human immunodeficiency virus, hepatitis viruses, Ebola virus, etc. Furthermore, antiviral therapy is increasingly being integrated into the management of chronic viral infections to restrain their replication, avert disease progression, and minimize the risk of complications. The availability of newer generations of antiviral drugs, particularly direct-acting antivirals, and combination therapies have significantly improved treatment

Effective Pseudomonas aeruginosa Eradication in Cystic Fibrosis: Multicenter Study.

BACKGROUND: Pseudomonas aeruginosa (PA) eradication is crucial in cystic fibrosis (CF) management. At our center, the success rate was 21.6%, prompting a revised eradication protocol to evaluate a new regimen. METHODS: Cystic fibrosis patients with first-time PA detection were included. Nonmucoid PA cases received oral ciprofloxacin (3 weeks) and inhaled tobramycin (1 month). For mucoid PA or failed eradication, ciprofloxacin (3 weeks) and extended nebulized therapy (3 months, alternating tobramycin/colistin) were used. Intravenous antibiotics were given to unwell patients. RESULTS: Among 10

Current insights and challenges in the management of left ventricular thrombus.

Left ventricular thrombus is a relatively common and serious complication in patients with left ventricular systolic dysfunction, resulting in an increased risk of embolic events such as ischemic stroke and contributing to higher mortality rates. Current clinical guidelines recommend anticoagulation therapy for 3 to 6 months in patients with left ventricular thrombus; however, the evidence supporting this approach primarily derives from small-scale randomized trials or retrospective observation studies, which limit the generalizability of findings across diverse patient populations. Several cr

Thyroid Cancer Surgery in a Patient With Frontotemporal Dementia: Balancing Oncologic and Neurocognitive Challenges.

We describe the perioperative management of a patient with severe frontotemporal dementia (FTD) who presented with progressive dysphagia due to a large compressive thyroid mass. Despite a non-diagnostic fine needle aspiration (FNA - Bethesda I), a left lobectomy and isthmectomy were performed to relieve symptoms while minimizing risk of recurrent laryngeal nerve injury and reducing operative time. The patient had a poor neurological prognosis, severe cognitive impairment, and increased anesthetic risk. Airway management was challenging due to tracheal deviation and compression, limited neck mo

A case report of minimally invasive management of congenital retrocaval ureter.

Retrocaval ureter (RU) is a rare congenital malformation where the ureteric pathway is altered, passing posteriorly around the inferior vena cava (IVC). Occasionally, this leads to the IVC compressing the ureter, resulting in obstruction. In this report, we discuss a male who presented with severe right-sided flank pain and was otherwise well with no significant medical, urological, or birth history. Initial imaging revealed severe right-sided hydroureter with distinct obstruction point, and likely RU. Anecdotal and small centre cases of acute management and laparoscopic RU repair were reviewe

Use of NPWTi-d for simultaneous pleural and mediastinal drainage in deep sternal wound infection during mechanical circulatory support.

This case report describes the management of deep sternal wound infection in a female patient supported with an extracorporeal left ventricular assist device (LVAD) following the Bentall procedure for acute aortic dissection. She developed pleural effusions with pathological communications between the pleural and mediastinal cavities, necessitating continuous drainage. After radical debridement, we applied negative pressure wound therapy (NPWT) with instillation and dwell time (NPWTi-d) to simultaneously drain the pleural cavities and the infected mediastinum. 24 French silicone fluted drain t
