Symptoms and signs of temporomandibular disorders in patients with sudden sensorineural hearing loss.

Sudden sensorineural hearingloss (SSH) usually affects one ear and leads to life long deafness in some cases. There are many theories about the origin of the condition but the etiology and pathophysiology are still unknown. However, tinnitus and vertigo frequently occur in patients with SSH, but are also frequent symptoms reported by patients with temporomandibular disorders (TMD). We hypothesized that TMD symptoms and signs are frequent in SSH patients. The objective of this study was therefore to investigate the presence of TMD symptoms and signs in SSH patients compared with healthy individ

Tensor tympani muscle: strange chewing muscle.

This work seeks to alert medical and odontological staff to understanding and using interdisciplinary handling for detecting different pathologies common otic symptoms. It offers better tools for this shared symptomatology during therapy s conservative phase. Tensor tympani muscle physiology and function in the middle ear have been veiled, even when their dysfunction and anatomical relationships may explain a group of confused otic symptoms during conventional clinical evaluation. Middle ear muscles share a common embryological and functional origin with chewing and facial muscles. This articl

[Acute unilateral total deafness and vestibular findings after gunshot noise].

BACKGROUND: Acute acoustic trauma is usually acquired during military service after exposure to impulse or blast wave noise. The typical audiometric shape is a notch centered at about 4 kHz with some recovery above this frequency. This is the first case of an immediate induced unilateral total hearing loss in a young soldier following exposure to gunfire noise. CASE REPORT: A 25-year-old right-handed army officer during military training, after realizing a series of five shots with a rifle (G3), immediately experienced on the right ear otalgia, tinnitus and severe hearing loss, without imbala

[The diagnostic value of TEOAE-glycerine test in patients with vertigo on the first attack].

OBJECTIVE: To investigate the diagnostic value of transiently evoked otoacoustic emissions (TEOAE) combined with glycerine test in patients with vertigo on the first attack. METHOD: PTT and TEOAE-glycerine test were conducted on the 28 patients who suffered the first attack of vertigo without hearing loss, tinnitus or a sensation of fullness in the ear. At the same time, history and clinical manifestation of the patients were not supportive for the non-Meniere's disease. RESULT: Among the 28 cases, four of them had nothing changed on the PTT and TEOAE parameters after glycerol administration

Focal and segmental glomerular sclerosis (FSGS) in a man and a woman with Fabry's disease.

We describe a man and a woman with Fabry's disease. Renal biopsies showed late and early stages respectively of focal and segmental glomerulosclerosis (FSGS) and vascular changes. Clinically the hemizygous patient had advanced renal disease with nephrotic range proteinuria and serum creatinine 122 micromol/l. The female carrier had minimal albuminuria, borderline GFR with a normal serum creatinine, acroparesthesias, moderate fatigue, tinnitus and headache accompanied by ischemic cerebral lesions. Enzyme replacement therapy (ERT) was initiated according to our Fabry protocol, partly due to the

Management of paragangliomas. Clinical and angiographic aspects.

Head and neck paragangliomas are highly vascular tumors with variable clinical behavior. The goal of this paper is to determine specific clinical and radiological findings and extract from these findings a treatment algorithm. Twenty-three patients with paragangliomas were referred from different surgical centers for angiography and pre-operative embolization. Clinical records were analyzed retrospectively, and focused on impairment of cranial nerves. Angiographic features of paragangliomas, such as arterial supply, tumor flow characteristics, and venous drainage, were evaluated to find charac

Immunohistochemical localization of cyclooxygenase isoforms in the organ of Corti and the spiral ganglion cells of guinea pig cochlea.

Prostaglandins have been used in experimental models and clinical studies for the therapy of sudden hearing loss and tinnitus with conflicting results. However, little is known about the rate-limiting enzymes of prostaglandin synthesis in the inner ear, the generally constitutively expressed cyclooxygenase 1 (COX-1) and the distress-inducible cyclooxygenase 2 (COX-2). To extend our knowledge concerning the physiological expression and localization of these two enzymes, immunohistochemical stainings of the guinea pig cochlea were performed. Light microscopical analysis revealed a homogenous dis

Cost comparison of auditory brainstem response versus magnetic resonance imaging screening of acoustic neuroma.

The cost-effectiveness of current diagnostic approaches employed in patients with suspected acoustic neuroma was evaluated. Currently, patients with signs and symptoms suggestive of acoustic neuroma, such as sudden unilateral hearing loss and/or tinnitus, undergo auditory brainstem response (ABR) screening tests to rule out this condition. If the ABR is normal, acoustic neuroma can be ruled out. However, if the ABR is abnormal, magnetic resonance imaging (MRI) or computed tomography is necessary to confirm the diagnosis. When one considers the total costs of this screening approach, one can as

Treatment of Ménière's disease by intratympanic gentamicin application.

Ménière's disease is a vestibular disorder characterized by episodic vertigo, tinnitus, fluctuant hearing loss, and ear fullness, with vertiginous attacks being the most disabling complaint. The effectiveness of conservative treatment has been variable, while surgical techniques provide more permanent relief from vertigo, but pose possible morbidity and cochlear risk. Intratympanic administration of gentamicin has been proposed as an alternative for patients with debilitating Ménière's disease who have failed to respond to standard medical treatment. The goal of such treatment is to control ve

High prevalence of symptoms of Menière's disease in three families with a mutation in the COCH gene.

We report the genetic analysis of one large Belgian and two small Dutch families with autosomal dominant non-syndromic progressive sensorineural hearing loss associated with vestibular dysfunction. Linkage studies in the Belgian family mapped the disease to the DFNA9 locus on chromosome 14. Mutation analysis of the COCH gene, which is responsible for DFNA9, revealed a missense mutation changing a highly conserved residue. One of the patients, who had an earlier age of onset in comparison with most of the affected family members, was shown to be homozygous for the mutation. After the mutation w

[Cerebral findings following cervical spine distortion caused by acceleration mechanism (whiplash injury). Assessment of current diagnostic methods in nuclear medicine].

In any grade of distortion of the cervical spine as a result of acceleration forces in addition to cervical symptoms cerebral symptoms like headache, vertigo, auditory disturbances, tinnitus, disturbances in concentration and memory, difficulties in swallowing, impaired vision and temporo-mandibular dysfunctions may appear. These symptoms can persist and become invalidating. Cerebral single-photon emission tomography (SPECT) and positron emission tomography (PET) enable new diagnostic horizons for neurotraumatology. In this article we summarize the actual findings of these nuclear medical meth

Alternobaric and hyperbaric oxygen therapy in the immediate and long-term treatment of Menière's disease.

Forty-five patients suffering from Menière's disease were submitted to pressure chamber therapy: 20 with constant pressure (2.2 ATA, hyperbaric treatment) and 25 with continuous variations in pressure levels (from 1.7 to 2.2 ATA, alternobaric treatment). Oxygenation therapy consisted of one session per day lasting 90 minutes for 15 days during the acute attacks followed by five consecutive sessions per month during a follow-up of two years. For a control group we used 18 patients treated with 10 per cent intravenous glycerol during the acute episode and 8 mg tid of betahistine thereafter. We c

[Cervicogenic hearing loss].

Existing investigations of cervical hearing disorders have been carried out predominantly in patients in whom those with vertebrobasilar insufficiencies (VBI) could not be distinguished from patients suffering from functional deficit of the upper cervical spine. Since two different syndromes exist, no statement can be made about cervical hearing disorders. That deafness can occur in VBI is uncontested. In contrast, there remains a dispute whether a "vertebragenic hearing disorder" exists. This latter disorder is believed accompanied by tinnitus, a feeling of ear pressure, otalgia and deafness

A clinical study of type 2 neurofibromatosis.

The clinical features, age at onset of symptoms and survival of 150 patients with type 2 neurofibromatosis were studied. The mean age at onset was 21.57 years (n = 110) and no patients presented after 55 years of age. Patients presented with symptoms attributable to vestibular schwannomas (acoustic neuroma), cranial meningiomas and spinal tumours. In 100 patients studied personally by the authors 44 per cent presented with deafness and this was unilateral in the majority (35/44). Deafness was accompanied by tinnitus in a further 10 per cent and muscle weakness or wasting was the first symptom

Treatment of vestibular disorders.

At our department, we generally perform inner ear anesthesia with lidocaine and intratympanic dexamethasone injection for treatment of patients with Menière's disease or labyrinthine vertigo due to circulatory disturbances of the inner ear, having failed other conservative treatments. These treatments were performed in 322 patients who consulted us during the past 7 years, including 136 patients with Menière's disease and 186 patients with labyrinthine vertigo. The criteria for evaluation of the therapeutic effect included the AAOO (1972), the AAO-HNS (1985) and Sakata's (1987) method. Both th

Single cochlear fibre responses in guinea pigs with long-term endolymphatic hydrops.

Some cochlear fibre response properties have been measured in two GPs approximately one year after induction of endolymphatic hydrops (by surgical obliteration of the endolymphatic sac and duct). These animals are considered as models of the effects of hydrops in Menière's disease, and the purpose of the study was to examine any modifications of fibre response properties which may underly auditory symptoms of the disease in man. Neurones towards more apical cochlear regions (with low characteristic frequencies) showed the greatest deterioration in tuning properties; on average, in the 1-6 kHz
