Congenital infections and the nervous system.

Despite vaccines, new antimicrobials, and improved hygienic practices, congenital infections remain an important cause of death and long-term neurologic morbidity among infants world-wide. Important agents include Toxoplasma gondii, cytomegalovirus, Treponema pallidum, herpes simplex virus types 1 and 2, and rubella virus. In addition, several other agents, such as the varicella zoster virus, human parvovirus B19, and Borrelia burgdorferi, can potentially infect the fetus and cause adverse fetal outcomes. This article provides an overview of these infectious disorders and outlines current stra

A review of toxoplasmosis in pigs.

Toxoplasma gondii infection is highly prevalent in pigs in most countries, but little is known of its epizootiology. Transplacental infection appears to be less common than post-natal infection. Most pigs acquire subclinical infection. Clinical toxoplasmosis occurs mostly in young pigs. The frequency of oocyst- vs tissue cyst-acquired infections is unknown. Why clinical toxoplasmosis is more prevalent in Japan than in other countries is puzzling and needs investigation. In experimentally infected pigs, tissue cysts persist in edible organs for several months. They persist longer in the brain,

A case of cerebral toxoplasmosis: "Eccentric and concentric sign" in MRI.

Central nervous system (CNS) toxoplasmosis is an opportunistic infection caused by the intracellular protozoan parasite Toxoplasma gondii. This organism typically causes disease in immunocompromised patients with human immunodeficiency virus (HIV). We reported a case of a 52-year-old woman with neurology symptoms and Magnetic resonance imaging (MRI) brain shows both eccentric and concentric target signs, which are typical signs in a patient with cerebral toxoplasmosis but rarely displayed in the same lesion. The MRI played a crucial role in diagnosing the patient and distinguishing CNS disease

Plentiful melanin pigment containing histiocyte-like cells in Coats disease: Awareness avoids diagnostic pitfall.

Coats disease is an exudative retinal vasculopathy characterised by presence of yellow-golden deposits in the retina and retinal detachment. Subretinal fluid drainage performed as a part of therapeutic management makes the fluid amenable to cytological examination. Infection by Toxoplasma may closely simulate the ocular symptoms seen in Coats disease. Awareness of the cytological findings in Coats disease helps to clinch accurate diagnosis. We herein present a case of Coats disease with many histiocyte-like cells with plentiful intracytoplasmic melanin pigment in cytology smears from subretina

Acute toxoplasmosis and pox-viral dermatitis in a juvenile bald eagle (Haliaeetus leucocephalus) in New Brunswick, Canada.

Although birds of prey are commonly subclinically infected by Toxoplasma gondii tissue cysts, clinical disease is relatively rare in these species. The present report describes a rare case of fatal toxoplasmosis in a juvenile bald eagle in New Brunswick. Necropsy investigation revealed severe emaciation and poxviral dermatitis which partially obscured the palpebral fissures. Microscopically there was severe lymphoplasmacytic inflammation and necrosis of the lung that was associated with abundant protozoal tachyzoites. Infection with T. gondii was confirmed in the lung via immunohistochemistry

Fatal disseminated toxoplasmosis in a brown-throated sloth (Bradypus variegatus) from Northern Brazil - Case report.

The clinical and pathological findings of a case of fatal disseminated toxoplasmosis in a captive brown-throated sloth (Bradypus variegatus) from the northern region of Brazil are reported. Clinical signs were nonspecific and included apathy, prostration, dyspnoea, and loss of appetite. Treatment with penicillin was attempted, but the animal died within five days of the onset of clinical signs. Microscopically, there was acute inflammation in the liver, spleen, and lungs associated with necrosis and a few cysts and extracytoplasmic tachyzoites, with a morphology compatible with Toxoplasma gond

Reassessing the mechanics of parasite motility and host-cell invasion.

The capacity to migrate is fundamental to multicellular and single-celled life. Apicomplexan parasites, an ancient protozoan clade that includes malaria parasites (Plasmodium) and Toxoplasma, achieve remarkable speeds of directional cell movement. This rapidity is achieved via a divergent actomyosin motor system, housed within a narrow compartment that lies underneath the length of the parasite plasma membrane. How this motor functions at a mechanistic level during motility and host cell invasion is a matter of debate. Here, we integrate old and new insights toward refining the current model f

CNS toxoplasmosis presenting with obstructive hydrocephalus in patients of retroviral disease--a case series.

CNS toxoplasmosis presenting as hydrocephalus is a very rare entity. We present three cases of HIV positive patients whose brain imaging revealed hydrocephalus and who improved with anti toxoplasma medication along with intravenous steroids and did not require any CSF shunting procedures. The mechanism of hydrocephalus in CNS toxoplasmosis is usually due to compression of CSF outflow pathway by ring enhancing lesions but even in their absence hydrocephalus can be rarely seen due to ventriculitis. Hence in HIV positive patients with unexplained hydrocephalus CNS toxoplasmosis should be consider

Calcium regulation in protozoan parasites.

The calcium ion (Ca(2+)) is used as a major signaling molecule in a diverse range of eukaryotic cells including several human parasitic protozoa, such as Trypanosoma cruzi, Trypanosoma brucei, Leishmania spp, Plasmodium spp, Toxoplasma gondii, Cryptosporidium parvum, Entamoeba histolytica, Giardia lamblia and Trichomonas vaginalis. Ca(2+) is critical for invasion of intracellular parasites, and its cytosolic concentration is regulated by the concerted operation of several transporters present in the plasma membrane, endoplasmic reticulum, mitochondria and acidocalcisomes. Recent findings have

AIDS related opportunistic infections, going but not gone.

It is now more than two decades since the AIDS epidemic began with a cluster of Pneumocystis carinii pneumonia (PCP) in a community of homosexual men. Since then, many other infections have been characterized as opportunistic infections secondary to HIV infection. These include, but are not limited to, infections with Toxoplasma gondii, Cytomegalovirus (CMV), Mycobacterium avium complex (MAC), and Cryptococcus neoformans. Over the last two decades, there have been dramatic improvements in diagnosis, prevention and treatment of all these infections. As a result, in North America and Western Eur

[Congenital toxoplasmosis with hydrocephalus diagnosed in utero: outcome of treatment].

BACKGROUND: The neurological outcome for severe toxoplasmosis can be poor despite appropriate management. CASE REPORT: A maternal toxoplasma infection occurred at 16 weeks of amenorrhoea; prenatal diagnosis was attempted at 20 weeks, fetal infection was confirmed by mouse inoculation at the 30th week. Pyrimethamine plus sulfadiazine treatment was initiated. However, at 37 weeks of amenorrhoea, sonographic examination of the fetus detected hydrocephalus. Despite prompt ventriculo-peritoneal shunting after birth and medical treatment of toxoplasmosis, the neurological developmental outcome was

[Ocular infections in immunodepressed patients].

Two kinds of immunosuppression must be distinguished: malignant diseases on one hand, and acquired immunodeficiency syndrome on the other hand. Ocular infections can involve any part of the eye and they can lead to blindness. They usually attest of a systemic infection. Their treatment consist of an induction course to provide complete healing of the lesions, immediately followed by a maintenance course to prevent relapses. Retinitis are the most common posterior involvement, especially the retinitis due to cytomegalovirus and Toxoplasma gondii. Zoster ophtalmicus is the most common anterior i

Central pain from cerebral abscess: thalamic syndrome in AIDS patients with toxoplasmosis.

We describe two patients with acquired immunodeficiency syndrome (AIDS) who developed classic thalamic syndrome (TS) due to Toxoplasma abscesses in the thalamic region. Treatment with amitriptyline provided substantial relief in both patients. Postmortem examination in one case revealed a lesion in the internal capsule and thalamic reticular nucleus. These observations indicate that (1) TS can result from an isolated lesion in the internal capsule and reticular nucleus of the thalamus, (2) cerebral abscess can cause classic TS, (3) central pain can be added to the many pain syndromes that affl

Use of stable isotopes to reveal trophic relationships and transmission of a food-borne pathogen.

Predators in food webs are valuable sentinel species for zoonotic and multi-host pathogens such as Toxoplasma gondii. This protozoan parasite is ubiquitous in warm-blooded vertebrates, and can have serious adverse effects in immunocompromised hosts and foetuses. In northern ecosystems, T. gondii is disproportionately prevalent in Inuit people and wildlife, in part due to multiple routes of transmission. We combined data on T. gondii infection in foxes from Nunavik (northern Québec, Canada) with stable isotope data tracking trophic relationships between foxes and several of their main prey spec

Understanding the Antiplasmodial Action of Resistance-Refractory Xanthoquinodin A1.

Our previous work identified a series of 12 xanthoquinodin analogues and 2 emodin-dianthrones with broad-spectrum activities against Trichomonas vaginalis, Mycoplasma genitalium, Cryptosporidium parvum, and Plasmodium falciparum. Analyses conducted in this study revealed that the most active analogue, xanthoquinodin A1, also inhibits Toxoplasma gondii tachyzoites and the liver stage of Plasmodium berghei, with no cross-resistance to the known antimalarial targets PfACS, PfCARL, PfPI4K, or DHODH. In Plasmodium, inhibition occurs prior to multinucleation and induces parasite death following 12 h

Comprehensive Insights into the Development of Antitoxoplasmosis Drugs: Current Advances, Obstacles, and Future Perspectives.

Current therapies for toxoplasmosis rely on a few drugs, most of which have severe side effects, and seeking ideal therapies for different types of toxoplasmosis is a long-term and challenging mission. Research and development (R&D) of novel drugs against Toxoplasma gondii (T. gondii) has focused on two main directions, the structural modification of lead compounds and natural products. Here we summarize the recent advances in the development of anti-T. gondii drugs from these two perspectives and provide comprehensive insights, reflecting on the advantages and selected molecules in each field

Pathogen Prevalence in Cetaceans Stranded along the Italian Coastline between 2015 and 2020.

The monitoring of stranded marine mammals represents a strategic method to assess their health, conservation status, and ecological role in the marine ecosystem. Networks worldwide track stranding events for the passive monitoring of mortality patterns, emerging and reemerging pathogens, climate change, and environmental degradation from a One Health perspective. This study summarizes pathogen prevalence data from the Italian Stranding Network (ISN) derived from post-mortem investigations on cetaceans found dead stranded along the Italian coastline between 2015 and 2020.
