Health-economic implications of the onset of action of antimanic agents.

BACKGROUND: Three medications, lithium, valproate, and carbamazepine, have been shown to be effective in the treatment of acute mania in randomized, controlled clinical trials. Additional research over the past decade has provided data regarding potential differences between these medications according to their time course of onset and clinical predictors of response in acute mania. These differences have potential impact on both the choice of medication and the health-economics of the treatment of acute mania. METHOD: Published reports regarding the time course of onset, clinical factors ass

Polycystic ovaries and hyperandrogenism in women taking valproate for epilepsy.

BACKGROUND: Reproductive endocrine disorders are more common among women with epilepsy than among normal women. These disorders have been attributed to epilepsy itself, but could be related to antiepileptic-drug therapy. METHODS: We studied 238 women with epilepsy who were seen regularly at the Outpatient Department of the University Hospital, Oulu, Finland. Their mean age was 33 years (range, 18 to 45), and the mean duration of therapy was 9 years (range, 0 to 31). Twenty-nine (12 percent) were treated with valproate, 120 (50 percent) with carbamazepine, 12 (5 percent) with valproate and car

Omega-3 Fatty Acids and Mood Stabilizers Alter Behavioural and Energy Metabolism Parameters in Animals Subjected to an Animal Model of Mania Induced by Fenproporex.

Studies have shown that changes in energy metabolism are involved in the pathophysiology of bipolar disorder (BD). It was suggested that omega-3 (ω3) fatty acids have beneficial properties in the central nervous system and that this fatty acid plays an important role in energy metabolism. Therefore, the study aimed to evaluate the effect of ω3 fatty acids alone and in combination with lithium (Li) or valproate (VPA) on behaviour and parameters of energy metabolism in an animal model of mania induced by fenproporex. Our results showed that co-administration of ω3 fatty acids and Li was able to

Functional consequences of sulfhydryl modification of the γ-aminobutyric acid transporter 1 at a single solvent-exposed cysteine residue.

The aims of this study were to optimize the experimental conditions for labeling extracellularly oriented, solvent-exposed cysteine residues of γ-aminobutyric acid transporter 1 (GAT1) with the membrane-impermeant sulfhydryl reagent [2-(trimethylammonium)ethyl]methanethiosulfonate (MTSET) and to characterize the functional and pharmacological consequences of labeling on transporter steady-state and presteady-state kinetic properties. We expressed human GAT1 in Xenopus laevis oocytes and used radiotracer and electrophysiological methods to assay transporter function before and after sulfhydryl

Epigenetic treatment of neurological disease.

Neurological disease, and in particular neurodegenerative diseases, cause significant burdens on both patient and healthcare costs. Despite extensive research, treatment options for patients with these conditions remain limited, and generally, only provide modest symptomatic relief. Aberrant epigenetic post-translational modifications of proteins are emerging as important elements in the pathogenesis of neurological disease. Using Alzheimer's disease and Huntington's disease as examples in the following article, some of latest data linking both the histone code and the various proteins that re

Seizure-freedom with combination therapy in localization-related epilepsy.

We analyzed the effect of combination therapy on seizure frequency in all adult patients (N=193) with focal epilepsy followed at a single institution in a cross-sectional study. One hundred and thirty-five patients were on two AEDs, of them, 37 (27%) were seizure-free, 50 patients were on three AEDs including 5 (10%) seizure-free patients (p<0.01 for seizure-freedom with two AEDs versus three AEDs). Thirty-five different combinations were used in patients on two AEDs and 40 combinations on patients on three drugs emphasizing the difficulties involved in evaluation of the efficacy and tolerabil

Influence of the MTHFR genotype on the rate of malformations following exposure to antiepileptic drugs in utero.

UNLABELLED: Folate deficiency and the presence of the 677C > T (CT) polymorphism in the methylenetetrahydrofolate reductase (MTHFR) gene have been implicated in the causation of malformations in the fetus (particularly cleft lip and palate and neural tube defects). These birth defects are also recognised complications of exposure to antiepileptic drugs (AEDs). In pregnant women with epilepsy, the use of AEDs has an added effect on the physiological reduction in folate levels, which may be enhanced further by the presence of the 677C > T polymorphism in the mother. We studied the MTHFR genotype

Adverse neuropsychiatric effects of anticonvulsant drugs.

Clinical and electrical evidence of peripheral neuropathy may result from long term treatment with phenytoin or barbiturates, especially in combination, or after repeated exposure to toxic blood concentrations of either drug. Prolonged acute toxicity with phenytoin may rarely lead to permanent residual ataxia. Reversible dystonia may occasionally be precipitated by phenytoin or carbamazepine; asterixis by phenytoin, barbiturates or carbamazepine; and, more commonly, tremor by valproate. All the major anticonvulsant drugs, especially in combination, can produce occasional subacute cognitive or

Efficacy and safety of cenobamate-based combination therapy in drug-resistant epilepsy: Secondary analysis by mechanisms of action of concomitant antiseizure medications.

INTRODUCTION: In a secondary analysis of a cohort of 475 adults with drug-resistant epilepsy treated with adjunctive cenobamate, we assessed whether treatment outcomes were influenced by concomitant antiseizure medication class and cenobamate dose. MATERIALS AND METHODS: Concomitant antiseizure medications (ASMs) were grouped as sodium channel blockers (SCBs), SV2A ligands, valproate, carbonic anhydrase inhibitors, or GABA analogs. Efficacy and safety outcomes were analyzed using multivariate logistic regression, adjusting for confounders. RESULTS: Baseline use of valproate was associated wi

Valproate inhibits orofacial and somatic hyperalgesia induced by maternal separation through decreasing IL-6 expression in the medial prefrontal cortex and spinal cord in rats.

Sodium valproate (VPA), a histone deacetylase (HDAC) inhibitor, has been shown to have analgesic effects and may be involved in analgesia through anti-inflammatory mechanisms. However, little is known about the involvement of VPA in the treatment of comorbid chronic pain conditions such as fibromyalgia syndrome (FMS) and temporomandibular disorder (TMD). In the present study, we used maternal separation (MS) stress model to simulate childhood stress-induced orofacial and somatic hyperalgesia at different age stages to mimic the overlapping pain symptoms of patients with TMD and FMS comorbidity

Building a dashboard to inform policy and clinical practice using the Irish epilepsy electronic patient record: Towards a learning health system.

OBJECTIVES: Ireland lags heavily in capacity and stakeholder interest for the development of learning health system tools. However, Ireland possesses a rich repository of electronic patient records for epilepsy patients stored at the National Epilepsy Centre at Beaumont Hospital. The objective of the current study was to develop a prototype dashboard from anonymized electronic records of Irish epilepsy patients to demonstrate the potential for the development of learning health systems in Ireland. METHODS: A cross-sectional extract of electronic patient records was extracted from the Irish Na

Potentiation of Nigra-Striatal Dopaminergic Projection Underpins Core Autism-Like Behaviors in Valproate-Exposed Mice.

Autism is characterized by two key diagnostic criteria including social deficits and repetitive behaviors. However, the underlying neural circuit dysfunction that accounts for these coexisting symptoms in autism remains poorly understood. Here we revealed that prenatal valproate exposure induced functional alterations of dopaminergic projections from substantia nigra pars compacta (SNc) to dorsomedial striatum (DMS). Specifically, we observed enhanced excitatory input and increased excitability in SNc→DMS dopamine (DA) neurons, resulting in a basal state of potentiation. This potentiated basel

Response characteristics of barrel cortical neurons in layers IV/V of juvenile rats with autism-like traits after tactile stimulation.

The barrel cortex is a specialized region of the primary somatosensory cortex that processes tactile information from whiskers. This study investigates how tactile stimulation (TS) affects excitatory receptive fields and surrounds suppression in barrel cortex neurons of male and female autistic-like rats, using various whisker displacement protocols. The animals were categorized into control, Valproic acid pre-treated (Val), and Val-TS treatment groups. In male Val-TS rats, TS reduced layer IV ON/OFF amplitudes for principal and adjacent whisker displacements, while only OFF response latency t

Plasma Lipidomic Profiles Investigating the Effects of Combined Therapy With Quetiapine and Magnesium Valproate in Emerging Adults With Bipolar II Disorder During Depressive Episodes: A Follow-Up Study.

Bipolar disorder (BD) is associated with alterations in plasma lipid profiles, which may contribute to its pathogenesis. However, the detailed plasma lipidomic composition of bipolar II disorder during depressive episodes (BPD-II) remains underexplored. This study investigates the temporal changes in plasma lipidomics in BPD-II patients at emerging adulthood undergoing treatment with quetiapine combined with magnesium valproate. We conducted a follow-up study involving individuals with BPD-II at emerging adulthood at baseline (BD-BL, n = 30), 3 weeks (BD-3w, n = 30), and 8 weeks (BD-8w, n = 30
