Radiographic assessment of ligamentous injuries in distal radius fractures after open reduction and internal fixation.

OBJECTIVES: Concomitant ligamentous injury in distal radius fractures (DRF) may explain continued pain following surgery. The purpose of this study was to compare radiographic measurements assessing scaphoid translation in DRF after reduction, to measurements performed on normal radiographs. This may allow noninvasive evaluation of radiocarpal ligamentous integrity. METHODS: Fifty postoperative radiographs were evaluated. The distance between the ulnar border of the radial styloid and the radial border of the scaphoid was measured midway between the styloid tip and scaphoid base, and then div

[Mental practice has influence on limitation of motion and muscle atrophy following immobilisation of the radiocarpal joint - a prospective randomised experimental study].

PURPOSE: Distal radial fracture is the most common bony injury in man. Still there are unsatisfying treatment results, such as limited joint movement, muscle atrophy and pain, resulting from immobilisation while the fracture is healing. During this period, also joint movement seems to be "forgotten". This study experimentally examined if the method of mental practice, meaning the systematic repetition of a consciously imagined movement or action without simultaneous practical execution, can positively influence these findings. MATERIALS AND METHODS: 21 right-handed males had application of a

Volar fixed-angle plate osteosynthesis of unstable distal radius fractures: 12 months results.

BACKGROUND: With an incidence of about 2-4 per 1,000 residents per year, the distal radial fracture is the most common fracture in the human skeleton. The introduction of fixed-angle plate systems for extension fractures at the radius was evaluated in a prospective study performed at our hospital after selection and acquisition of a new system. The focus of our interest was whether a secondary loss of reduction can be avoided by this plating system. METHODS: We reviewed 80 patients treated for unstable distal radius fractures using a volar fixed-angle plate. Postoperative management included

[Diagnostic value of unspecific requested and implemented MRI for detecting intracarpal lesions, compared to arthroscopic findings at 217 patients].

AIM: The purpose of this study was to evaluate the diagnostic value of MRI for detecting intracarpal lesions in clinical routine. PATIENTS AND MATERIALS: In a retrospective study, we reviewed the charts of 506 patients who had undergone wrist arthroscopy in our department between May 1998 and November 2002. Out of 506 patients 217 had an MRI. The MRI was performed at 31 different radiology facilities using a number of techniques. The MRI results were compared with the arthroscopic findings, taking the arthroscopic results as a "gold standard" (sensitivity = SEN, specificity = SPE, positive pr

3-year follow-up of 215 fracture patients from a prospective and consecutive osteoporosis screening program. Fracture patients care!

BACKGROUND AND PURPOSE: Fractures can be prevented if osteoporosis is identified and treated. Starting in 2002, we have been using a screening program in which patients between 50 and 75 years of age with a wrist, shoulder, vertebral, or hip fracture are assessed by DEXA of the hip and spine and if osteoporotic or osteopenic, they are encouraged to see a doctor of their own choice. The patients receive documents containing information, the results of DEXA, and a letter to present to their doctor with suggestions regarding blood tests and treatment. Here we report the 3-year follow-up regarding

[Scapho-trapezio-trapezoid arthrodesis (triscaphe arthrodesis)].

BACKGROUND: Scapho-trapezio-trapezoid (STT)/triscaphe fusion is used to stabilize the radial column of the wrist. However the reported results are controversial. MATERIAL AND METHODS: 111 patients were treated with STT fusion from 1992 to 1997. Indications were chronic dissociation of the scapholunate joint (n = 15), idiopathic arthrosis of the scaphotrapeziotrapezoid joint (n = 11), Kienböck's disease in advanced stage (n = 84) and dislocation of the trapezium (n = 1). This study reviews the results after an average follow-up period of four years (range, two to eight years). RESULTS: Patien

Peripheral tears of the triangular fibrocartilage complex cause distal radioulnar joint instability after distal radial fractures.

The aim of this prospective study was to determine whether peripheral tears of the triangular fibrocartilage complex (TFCC) in patients younger than the osteoporotic age (males,<60 years; females, <50 years) were related to chronic distal radioulnar joint (DRUJ) instability. Fifty-one patients (27 women) with displaced distal radial fractures were included in the study. The median age was 41 years (range, 20-57 years). Arthroscopy at the time of fracture showed complete or partial TFCC tears in 43 patients (24 had only peripheral tears, 10 had only central perforations, and 9 had combined tear

Prospective multicenter trial of a plate for dorsal fixation of distal radius fractures.

A new plate designed specifically to address complex wrist pathology was used for the internal fixation of 22 complex fractures of the distal radius in 22 patients in a prospective multicenter trial. The majority of fractures were group C2- and C3-type fractures according to the Comprehensive Classification of Fractures. No plate failures, loss of reduction, nonunions, or infections occurred. Within the average follow-up time of 14 months, the functional results (including an average motion of 76% and an average grip strength of 56% of the contralateral side) were comparable to those reported

An algorithm for acute wrist trauma. A systematic approach to diagnosis.

A systematic approach to the acute traumatized wrist is of importance to achieve early diagnosis, with efficient use of advanced imaging modalities. In a 6-month period 641 patients aged 15 years or over were examined using an algorithm for acute wrist trauma in order to secure early and appropriate treatment. In 293 (46%) cases routine radiographic examination supplied sufficient information to establish a fracture diagnosis. All 33 scaphoid fractures (including five scaphoid non-unions) were demonstrated on the initial X-ray examination. In 56 patients (9%) scintigraphy proved useful for sel

[Diflunisal, a new analgesic, and oxyphenbutazone in the treatment of sprains and dislocations (author's transl)].

In a randomized double-blind study efficacy and tolerance of diflunisal, 375 mg twice daily, and oxyphenbutazone, 200 mg twice daily, were compared in 40 patients (aged 21 to 70 years, average 38 years), with moderate or severe complaints after spraining or dislocating ankle or wrist. The drugs were given for five days. Both proved to be highly efficacious, diflunisal slightly more so. Among the 20 patients in the diflunisal group one developed diarrhoea, another gastritis; in the oxyphenbutazone group one developed gastritis, another herpetiform pustules. There were no clinically significant

Beyond BMI: The Role of Anthropometric Measurements in Predicting Plantar Pressure Distribution and Gait Biomechanics.

Plantar pressure (PP) is a key biomechanical parameter for assessing foot function, gait abnormalities, and injury risk. Although body mass index (BMI) correlates with PP magnitude/distribution during gait, it is a coarse metric that collapses body composition into a ratio of mass to height, implicitly assuming uniform body proportions and mass distribution. The effects of individualized anthropometric parameters on PP signatures remain under-characterized, with a gap in incorporating body-shape measures beyond BMI. This exploratory study examined associations among anthropometric measurements

Excision without Reconstruction of a Traumatically Ruptured Extensor Indicis Proprius Tendon in a Diabetic Patient: A Case Report.

INTRODUCTION: Isolated traumatic rupture of the extensor indicis proprius (EIP) is uncommon and may be masked by preserved index finger extension through the intact extensor digitorum communis (EDC). While primary repair or tendon transfer is standard when reconstruction is feasible, the functional redundancy of the dual-extensor index finger raises the question of whether excision alone can suffice in selected cases. We report what we believe to be the first documented case of intentional EIP stump excision without reconstruction as definitive surgical treatment following delayed-presentation

Triquetral Alignment: Measurement Techniques, Reliability, and Normal Values.

PURPOSE: Assessing triquetral alignment aids in evaluating carpal instability, but no reliable methods have been established. This study assessed factors affecting the reliability of triquetral angle measurements by experienced observers and used a computer-aided analysis software to establish normal values for triquetral alignment. METHODS: A mathematical modeling-based software analyzed 121 cone-beam computed tomography scans of asymptomatic wrists and measured the radiotriquetral and lunotriquetral angles. Thirty scans were digitally reconstructed into lateral radiographs and measured by h

Radioulnar Synostosis Using a Vascularized Pedicled Radial Periosteal Flap for Supination Deformity in Residual Brachial Plexus Birth Injury.

BACKGROUND: Supination posture is a common secondary deformity in brachial plexus birth injury (BPBI) that results in functional upper limb impairment. Radioulnar synostosis using a vascularized ulnar periosteal flap has recently been reported, with dorsal forearm scarring as the main reported disadvantage. This report evaluates outcomes in a series of children with forearm supination deformity and complete pronation palsy secondary to severe BPBI undergoing radioulnar synostosis in slight pronation using a novel proximal pedicled vascularized radial periosteal flap (VRPF). METHODS: We comple

Robotic Single-Port Trans-stomal Hartmann Reversal.

BACKGROUND: The colostomy site provides natural access for a single-port Hartmann reversal, making it an ideal scar-sparing procedure. Although single-incision laparoscopic surgery for this purpose is technically challenging, the da Vinci SP robotic platform may overcome single-incision laparoscopic surgery limitations: its wristed instruments and 3-dimesional visualization could enhance the feasibility of a full single-port approach. IMPACT OF INNOVATION: The robotic single-port approach for Hartmann reversal retains all the advantages of a single-port access through the stoma site while ove

Exploring the relationship between frailty and nonunion fractures in upper extremity injuries: insights from the national inpatient sample.

INTRODUCTION: Frailty, a physiological decline in functional capacity, may influence nonunion risk. This study aimed to investigate the association between frailty, as measured by the modified Frailty Index-5 (mFI-5), and the likelihood of nonunion fractures of the upper extremity. METHODS: This retrospective cohort study utilized the national inpatient sample (NIS) from 2015 to 2019. Patients aged 18 and older with upper extremity fractures, identified by ICD-10-CM codes, were included. Patients were categorized into routine healing and nonunion groups. Frailty was assessed using the mFI-5,
