Phimosis as a cause of the prune belly syndrome: comparison to a more common pattern of proximal penile urethra obstruction.

The pathogenesis of the prune belly syndrome (PBS) remains controversial, but two theories predominate. The first theory supports an obstructive phenomenon early in gestation leading to irreversible damage to the genitourinary tract and abdominal wall. The second theory suggests mesodermal injury between the 6th and 10th weeks of gestation as the primary abnormality. This paper reports of two fetuses with the PBS phenotype that were examined postmortem at our institution. Thorough examination of the lower urinary tract allowed demonstration of anatomic obstruction of the urethra in both cases.

Plasma exchange in severe postpartum HELLP syndrome.

We present a case of extremely severe postpartum HELLP (hemolysis, elevated liver enzymes, low platelets) syndrome, associated with activation of coagulation, massive recurrent intra-abdominal bleeding requiring two laparotomies, renal failure, and central nervous system symptoms. This case underlines the complexity of pregnancy-related thrombotic microangiopathies regarding their differential diagnosis, multiple organ dysfunction, as well as management. Systemic endothelial cell injury plays a central role in the pathogenesis of thrombotic microangiopathies. Treatment of HELLP syndrome usuall

Eventration of the diaphragm in children.

Diaphragmatic plication is a safe, well tolerated procedure for congenital diaphragmatic eventration. It is the treatment of choice in neonates with respiratory distress following phrenic nerve injury. The abdominal approach is as good as the thoracic and may be safer in cases with central eventration and when the diagnosis of congenital diaphragmatic hernia cannot be excluded with confidence. However, in phrenic nerve paralysis the thoracic approach is superior in showing the anatomical distribution of the phrenic nerve. Experience with eight paediatric patients treated for diaphragmatic even

Asymptomatic rectal perforation with retroperitoneal emphysema.

A 57-year-old man administered an enema to himself, preparatory to intravenous pyelography. He left after the films were taken and could not be reached when retroperitoneal emphysema was detected, He was located 12 days later and found in good health. Abdominal x-ray films and rectosigmoidoscopy were normal. The patient refused further investigations. It is assumed that a small perforation occurred at the rectosigmoid junction during the self-administration of the enema. This assumption is borne out by the pattern of air distribution. The total absence of complaints and physical signs is unusu

Early computed tomography may miss main pancreatic duct injury in distal pancreatic trauma: a prospective surgically verified diagnostic accuracy study.

PURPOSE: To assess the diagnostic performance of computed tomography (CT) for predicting main pancreatic duct injury in surgically explored patients with distal pancreatic trauma. METHODS: We conducted a prospective single-center diagnostic accuracy study from August 2024 to March 2026. Consecutive patients with distal pancreatic trauma underwent contrast-enhanced abdominal CT before surgery. On CT, main pancreatic duct injury was defined by either a deep pancreatic parenchymal laceration involving at least 50% of pancreatic thickness or complete pancreatic transection. CT findings were compa

[Emergency management of polytrauma patients].

Adequate prehospital care of the severely traumatised patient is important to prevent or attenuate early as well as late life threatening complications, such as tissue hypoxia, ischemia/reperfusion injury and finally multiple organ failure. A mismatch of oxygen supply and oxygen demand is a hallmark in the pathophysiology of multiple trauma. Oxygen supply may be diminished by the following factors: shock-related decrease of cardiac output, anemia and hypoxia. On the other hand, oxygen demand may be increased by pain, panic, and agitation. Hence, it is a central point in prehospital care to red

The influence of the fundal pressure manoeuvre at delivery on the anal sphincter injury diagnosed with endoanal ultrasonography.

OBJECTIVES: The fundal pressure manoeuvre (FPM) is a procedure where the fundus of the uterus is pushed through the abdominal wall by the midwifes and doctors to shorten the terminal phase of the second stage vaginal delivery. Nowadays its use is controversial and associated with many adverse effects. The aim of the study was to evaluate benefits and adverse maternal outcomes after FPM. The correlation of the FPM with episiotomy was evaluated. The role of the FPM on pelvic floor dysfunction such as anal incontinence due to anal sphincter injury was assessed. STUDY DESIGN: The retrospective st

Platelet Inhibition Prevents NLRP3 Inflammasome Activation and Sepsis-Induced Kidney Injury.

Platelets, cellular mediators of thrombosis, are activated during sepsis and are increasingly recognized as mediators of the immune response. Platelet activation is significantly increased in sepsis patients compared to ICU control patients. Despite this correlation, the role of activated platelets in contributing to sepsis pathophysiology remains unclear. We previously demonstrated NOD-like receptor protein 3 inflammasome (NLRP3) inflammasome activation in sepsis-induced platelets from cecal-ligation puncture (CLP) rats. Activated platelets were associated with increased pulmonary edema and g

Association of Psychosocial Factors and Hospital Complications with Risk for Readmission After Trauma.

BACKGROUND: Unplanned hospital readmissions are associated with morbidity and high cost. Existing literature on readmission after trauma has focused on how injury characteristics are associated with readmission. We aimed to evaluate how psychosocial determinants of health and complications of hospitalization combined with injury characteristics affect risk of readmission after trauma. MATERIALS AND METHODS: We conducted a retrospective cohort study of adult trauma admissions from July 2015 to September 2017 to Harborview Medical Center in Seattle, Washington. We assessed patient, injury, and

Origin an importance of increased alkaline phosphatase activity in peritoneal fluids of horses with colic.

The origin of increased alkaline phosphatase (ALP) activity in peritoneal fluid (PF) of horses with clinical signs of abdominal pain was investigated to determine the usefulness of measuring ALP in PF in the diagnosis of small intestinal injury. The ALP isoenzymes in PF from 10 clinically normal horses and from 50 horses with clinical signs of acute abdominal pain were analyzed for their sensitivities to inhibition by L-phenylalanine, L-homoarginine, and levamisole and to inactivation by heat (56 C, 15 minutes). The enzymes also were discriminated by their patterns of migration during polyacry

Hepatic angiography in patients undergoing damage control laparotomy.

OBJECTIVE: Patients undergoing damage control (DC) laparotomy require intensive and aggressive resuscitation, and may require additional maneuvers to control parenchymal bleeding. Those patients suffering significant liver injury are at high risk for arterial bleeding deep within the liver, and many require hepatic angiography in addition to hepatic packing. We reviewed our experience with hepatic angiography, and sought to determine its safety in the DC population of penetrating and blunt trauma patients. METHODS: A 3-year (June 1997-May 2000) retrospective review generated 37 DC patients. P

Iron, Copper, and Zinc Dyshomeostasis in Cardiovascular and Cerebrovascular Diseases: Redox Mechanisms, Evidence Levels, and Translational Prospects.

Iron (Fe), copper (Cu), and zinc (Zn) are essential trace elements for cardiovascular and cerebrovascular homeostasis, acting as enzymatic cofactors in energy metabolism and antioxidant defence. However, disruption of metal homeostasis can amplify oxidative injury through direct Fe/Cu redox chemistry or indirect Zn-dependent pathways. Ferroptosis and cuproptosis have been investigated as context-dependent mechanisms in various cardiovascular and cerebrovascular diseases (CCVDs), such as atherosclerosis, ischaemic/haemorrhagic stroke, diabetic vascular disease, and abdominal aortic aneurysm. Preventing Recurrent Laryngeal Nerve Injury During Transhiatal Esophagectomy Through Targeted Cervical Dissection: A Case Report.

Injury to the recurrent laryngeal nerve (RLN) is a well-recognized complication of esophagectomy and is associated with significant postoperative morbidity, including dysphonia, aspiration, and pulmonary complications. The risk is particularly relevant during transhiatal esophagectomy, where limited mediastinal visualization increases the likelihood of traction-related nerve injury. We report a case of a 68-year-old male with distal esophageal carcinoma who underwent transhiatal esophagectomy following neoadjuvant therapy, in whom a targeted cervical dissection strategy was employed to achieve
