Parent and Adolescent Preferences for Online Youth Mental Health Support Programs: A Discrete Choice Experiment.

BACKGROUND: Digital mental health programs can expand access to psychological support for children and adolescents, especially where stigma, cost, and workforce shortages limit traditional care. Understanding which features young people and parents value most is essential for improving engagement and real-world uptake. METHODS: We conducted a cross-sectional discrete choice experiment to quantify preferences for online youth mental health program features among adolescents and parents. Data were collected through an online survey of BRAVE Program families with prior experience using the BRAVE

Creating a More Efficient and Equitable Autism Evaluation Pathway Through Integrated Behavioral Health in Pediatric Primary Care.

BACKGROUND: Long wait times for autism spectrum disorder diagnostic evaluations delay access to early intervention and disproportionately affect Hispanic families and families who speak a primary language other than English. Integrated behavioral health models in pediatric primary care offer an opportunity to improve early identification, streamline referrals, and reduce inequities in access to specialty care. METHODS: We describe the implementation of a tiered, integrated autism screening and diagnostic pathway within Hasbro Children's Pediatric Primary Care Clinic in Providence, Rhode Islan

Radiotherapy and diagnostic capacity in relation to the changing cancer burden in the Baltic States.

BACKGROUND AND PURPOSE: Cancer mortality rates in the Baltic States (Estonia, Latvia, and Lithuania) exceeds the European Union (EU) average, in part due to limited access to radiation therapy (RT). We updated RT capacity and utilization to inform regional planning. Patient/material and methods: We conducted a census of all 11 RT centres (2016-2023) via a standardized questionnaire, cross-validated with national registries and international databases. We compared technology availability, workforce, and utilization with EU countries in relation to the present cancer burden and projections to 20

Maximising Efficient Endoscopy Training-Implementation Analysis of a Tool to Improve Endoscopy Training.

Introduction UK demand for gastrointestinal (GI) endoscopy exceeds capacity, with trainee access to dedicated training lists below the Joint Advisory Group on GI Endoscopy (JAG) recommendation of ≥20 lists per year [1-5]. Barriers include rota conflicts and limited trainer availability, which restrict equitable access [6-12]. Improving visibility of training opportunities may improve access to supervised procedures. Methods The Maximising Efficient Endoscopy Training (MEET) tool is a web-based planner built using Laravel PHP and the TALL stack. It matches trainees to suitable training lists an

Veterans Health Administration Peritoneal Dialysis Program Infrastructure and Clinical Practices: Findings from a Nationwide Questionnaire.

Peritoneal dialysis (PD) use among the US Veteran population is lower than that in the non-Veteran kidney failure population. Increasing choice of and access to high quality home dialysis within the Veteran population is a Veterans Health Administration (VHA) strategic priority. To date, no comprehensive assessment of existing VHA PD programs has been conducted. The VHA PD Committee conducted a nationwide stakeholders survey to inform strategies ( 1 ) to improve Veterans ability to select PD (VHA PD Choice subgroup), ( 2 ) to access PD (VHA PD Access subgroup), and ( 3 ) to receive excellent q

Experiencing medical desertification: A care-seeker-centered qualitative comparison across rural and urban sites in France and Slovenia.

"Medical desert," referring to conditions of unequal access to medical care, remains contested in its definition and measurement. Predominantly supply-focused, most studies rely on quantitative indicators and few examine how care-seekers perceive and navigate shortages, nor do they address desertification as a dynamic, policy-driven process. None, to our knowledge, compare these experiences across rural and urban settings or countries. We conducted a qualitative comparative investigation during the COVID-19 pandemic among people experiencing health and social vulnerabilities in France and Slov

Community Coalitions Implementing and Sustaining Policy, Systems and Environment Strategies for Rural Health Equity.

Rural communities have assets that can be leveraged to address health disparities, particularly through complex policy, systems and environment (PSE) change approaches. Eleven rural community coalitions across Georgia participated in The Two Georgias Initiative, a 5-year health equity project from 2017 to 2022 designed to address rural-urban health disparities. The coalitions addressed community health concerns through implementing county-specific PSE and programmatic strategies tracked through a Community Change Tracking Tool. This mixed-methods analysis assessed the variation in strategies i

Models of Care for Mental Health Nurse Practitioners in Emergency Department Settings: A Scoping Review Across High-Income Countries.

This scoping review examined and synthesised existing literature on models of care involving Mental Health Nurse Practitioners (MHNPs) practising or consulting within Emergency Department (ED)-based Emergency Mental Health (EMH) services. EDs are a key access point for mental health care and often provide first contact or after-hours support for people experiencing crisis, reinforcing their central role within the mental health system. The review highlights the need for timely, safe and specialised mental health care within EDs to strengthen the quality, safety and responsiveness of EMH servic

Development of a clinical decision support system for exercise prescription among cancer survivors: a modified Delphi study.

PURPOSE: Exercise is safe and beneficial for cancer survivors, with evidence-based guidelines supporting its use to improve health-related outcomes. However, access to structured exercise oncology programs remains limited, and available services are typically accessible only for individuals living near major health care centers. The aim of this study was to develop the content and layout of a clinical decision support system (CDSS) that enables the delivery of evidence-based, personalized exercise prescriptions for cancer survivors across a nationwide network of health care institutions in Swi

'Veni, Vidi, Vaccini': consensus-based vaccination pathway implementation in a paediatric tertiary hospital in Tuscany, Italy.

Fragile paediatric patients are at higher risk of vaccine-preventable diseases, yet coverage remains suboptimal. Hospitalisation offers an opportunity to review and update immunisation, but fragmented practices create inequities, with access depending more on hospital context than clinical need. This study aimed to design, implement, and evaluate an in-hospital vaccination pathway for these patients. A mixed-methods approach was undertaken at Meyer Children's Hospital IRCCS, Florence, Italy. Baseline assessment included nine focus groups and an online questionnaire with healthcare professional

Bridging the digital pathology divide: reciprocal learning between high-resource and low-resource settings.

Digital pathology (DP) represents a major advance in global health, integrating digital whole-slide imaging, telepathology and clinical-grade artificial intelligence into cohesive workflows that can improve diagnostic accuracy and educational access. Realising these gains, however, depends on local infrastructure, governance, reimbursement and workforce capacity. The COVID-19 pandemic accelerated DP uptake in some settings, but adoption remains highly uneven between high-resource settings (HRS) and low-resource settings (LRS), reflecting unequal infrastructure, workforce, policy and financing.

The APLC expert consensus recommendations on the management of chronic lymphocytic leukaemia in Asia.

INTRODUCTION: Targeted therapies have significantly transformed the management of chronic lymphocytic leukaemia (CLL), yet most recommendations continue to reflect Western practice patterns. Variations in disease biology, healthcare resources and treatment accessibility across the Asia-Pacific (APAC) necessitate region-specific guidance. The Asia-Pacific Leukaemia Consortium (APLC) therefore developed updated consensus statements to support standardised, context-appropriate care for patients with CLL. METHOD: A modified Delphi process was conducted with 17 haematology experts from multiple AP

Spike on a bike: mobile harm reduction in action.

BACKGROUND: Access to harm reduction services remains a significant challenge in rural areas, where geographic and structural barriers can prevent people who use drugs from engaging with fixed-site support. In response to declining service use during the COVID-19 pandemic, the Dyfed Drugs and Alcohol Service (DDAS), led by Barod, a Welsh harm reduction and treatment provider, developed a peer-led mobile intervention known as Spike on a Bike (SOAB), designed to deliver harm reduction equipment and relational support directly to service users in remote communities. METHODS: This study draws on

"Why should I pay for this vaccine?" - Norwegian adolescents views on meningococcal vaccination.

BACKGROUND: As one of few countries in Europe, Norway does not offer vaccination against invasive meningococcal disease (IMD) as part of the National Immunization Program for Children (NIP). Instead, adolescents aged 16-19 are recommended to consider having the meningococcal ACWY conjugate vaccine (MCV4) if they participate in activities that increase the risk of IMD. Meningococcal vaccination rates in Norway are low compared to vaccines offered through the NIP. Whereas all vaccinations in the NIP are free and organized through schools or municipal health clinics, there is no uniform system in
