Young-onset colon adenocarcinoma masquerading as acute appendicitis.

This case report presents the findings of colon adenocarcinoma in a young adult male who presented with vague abdominal pain as his only complaint, suspicious of appendicitis. The patient underwent abdominal computed tomography (CT) imaging for further evaluation of his abdominal pain. CT findings showed pericecal fat stranding and prominent lymph nodes concerning for acute appendicitis, but the appendix could not be adequately visualized; due to the indeterminate CT findings, general surgery proceeded to perform an exploratory laparotomy on the patient and removed an appendiceal mass-like str

An Unusual Presentation of Acute Appendicitis Mimicking Sigmoid Diverticulitis: A Case Report.

Acute appendicitis typically causes right lower quadrant pain, but in elderly patients with comorbidities, it can present atypically, complicating diagnosis. This case highlights a rare presentation, mimicking sigmoid diverticulitis. A 70-year-old man with chronic heart failure, arrhythmia, and renal failure presented with two days of left lower quadrant pain. Examination showed tenderness, rebound, and guarding. Laboratory tests revealed a white blood cell count of 12,000/µL and creatinine of 2.5 mg/dL. C-reactive protein was elevated at 25 mg/L. Computed tomography revealed an inflamed appen

Extensive Osteonecrosis After Glucocorticoids: Clinical Response to Bisphosphonate.

Osteonecrosis is a devastating complication of long-term glucocorticoid therapy that has been described in both malignant and nonmalignant diseases. Its incidence has been found to greater than 50% using magnetic resonance imaging in asymptomatic patients, thus osteonecrosis is likely underdiagnosed. Recent studies have suggested that treatment with bisphosphonates can improve pain and mobility and decrease bone marrow edema. We describe a patient with acute lymphoblastic leukemia who presented with debilitating osteonecrosis after treatment with prednisone for a total cumulative dose of 5100

Capecitabine-induced chest pain relieved by diltiazem.

Five patients with primary colorectal adenocarcinoma or anal squamous cell carcinoma were started on a 2-weeks-on, 1-week-off capecitabine dosing regimen in addition to other chemotherapeutic agents and/or radiation. Within the first few doses, patients experienced chest pain and/or dyspnea at rest or with exertion. Acute electrocardiographic findings suggestive of ischemia were found in some cases at initial presentation, and 1 patient had troponin elevation consistent with an acute ST-segment elevation myocardial infarction. Subsequent ischemia evaluations were not suggestive of clinically s

Translation, validation, predictive analysis of the Italian version of the McSweeney Acute and Prodromal Myocardial Infarction Symptom Survey and symptom cluster characterization in women.

BACKGROUND: Nurses are the touch point for different aspects of patient care, particularly in identifying early prodromal symptoms of acute myocardial infarction in women. Although there is increasing awareness of women-specific symptoms like fatigue, anxiety, and sleep disturbances, acute myocardial infarction is frequently underdiagnosed or recognized too late in women. Instruments such as the McSweeney Acute and Prodromal Myocardial Infarction Symptom Survey have demonstrated effectiveness in identifying these prodromal symptoms. However, no validated Italian tool exists to assess prodromal

Hepatitis E Infection With Acute Liver Failure.

A 21-year-old man from India presented with acute hepatitis associated with a 1-week history of abdominal pain, pruritus, and dark urine. Over a 7-day admission, the patient's acute hepatitis evolved into acute liver failure with low-grade encephalopathy, markedly elevated transaminases, bilirubin, and impaired hepatic synthetic function. He was eventually diagnosed with acute hepatitis E virus (HEV) and was transitioned to supportive management. Acute HEV is a rare cause of acute liver failure. Hence, this case highlights the importance of early consideration of HEV in all patients with acute

Acute acalculous cholecystitis complicating chemotherapy for acute myeloblastic leukemia.

Acute acalculous cholecystitis is a rare complication in the treatment of acute myeloblastic leukemia. Diagnosis of acute acalculous cholecystitis remains difficult during neutropenic period. We present two acute myeloblastic leukemia patients that developed acute acalculous cholecystitis during chemotherapy-induced neutropenia. They suffered from fever, vomiting and acute pain in the epigastrium. Ultrasound demonstrated an acalculous gallbladder. Surgical management was required in one patient and conservative treatment was attempted in the other patient. None treatment measures were effectiv

Clavicle Nonunion RepairWhat Can Patients Expect?

BACKGROUND: The purpose of this study was to compare the long-term functional status of patients treated surgically for a clavicular nonunion using patients treated either op-eratively or non-operatively for an acute clavicle fracture as a comparison group. METHODS: Twenty consecutive patients treated by a single surgeon for a clavicle fracture nonunion were identified. For comparison of outcomes, acute clavicle fractures were identified from an electronic medical record (EMR) query of the same orthopedic surgeon. Ninety acute clavicle fracture patients were identified and 27 (30%) patients w

Current concepts in chronic pain management.

The majority of people afflicted with orofacial pain have acute pain that resolves quickly, but some are left with chronic and disabling pain. Therapy must be provided to deal with the nociception, behavior, and suffering. Appropriate behavioral evaluation may be required prior to developing a treatment plan. The treatment should then be carefully outlined and presented in a treatment-planning visit and may include physical, pharmacologic, and behavioral aspects.

Penetration and efficacy of transdermal NSAIDs in a model of acute joint inflammation.

PURPOSE: Prescription and OTC non-steroidal anti-inflammatory drugs (NSAIDs) are ubiquitous treatments for pain and inflammation; however, oral administration of these drugs may produce gastrointestinal (GI) side effects. Transdermal (TD) administration of NSAIDs circumvents these adverse events by avoiding the GI tract and, presumably, achieves regional drug levels of therapeutic effect and thereby, fewer off-target complications. METHODS: A drug quantification method was developed for ibuprofen and celecoxib in canine plasma and synovial fluid using liquid chromatography and mass spectromet

Comparison of ketorolac-chlorpromazine with meperidine-promethazine for treatment of exacerbations of chronic pain.

BACKGROUND: The aim was to compare the efficacy and safety of a combination of intramuscular ketorolac and chlorpromazine for the treatment of acute exacerbations of chronic pain with the more commonly used regimen of intramuscular meperidine and promethazine. METHODS: Use-effective case series were drawn from a real-life, rural emergency department practice, in which 200 consecutive patients coming to a rural emergency department with acute exacerbations of chronic pain syndromes were assigned on an every-other basis in a single-blind fashion to one of the two treatment conditions. Mesenteric Lymphadenitis on Point-of-Care Ultrasound in Pediatric Patients With Group A Streptococcal Pharyngitis: A Case Series.

Abdominal pain is a common complaint in pediatric patients with Group A streptococcal (GAS) pharyngitis. Etiology of abdominal pain from GAS infections are unknown. Point-of-care ultrasound (POCUS) is often used by pediatric emergency medicine physicians to evaluate children with abdominal pain. We describe 12 pediatric patients who presented to our Pediatric Emergency Department with symptoms suggestive of acute pharyngitis and abdominal pain, tested positive for GAS pharyngitis, and were found to have mesenteric lymphadenitis (mLAD) on POCUS. Findings on POCUS suggest that abdominal pain in

Utilization of Propofol as an Analgesic Adjuvant in the Multimodal Management of Acute Erythromelalgia Secondary to Lyme Disease Neuropathy: A Case Report.

Pediatric neuropathic pain can be difficult to treat, may require a multimodal approach, and is often guided by adult literature given the paucity of pediatric studies. A healthy adolescent male presented with acute onset of severe bilateral lower extremity neuropathic pain diagnosed as erythromelalgia secondary to Lyme disease. After standard medication regimens failed to control his symptoms, popliteal nerve blocks improved pain control and subhypnotic doses of propofol provided significant pain relief. This case describes a complex multidisciplinary workup and multimodal treatment-including

Continuous peripheral nerve block for upper limb ischemic pain: a case report.

Peripheral Arterial Obstructive Disease (PAOD) may course with severe ischemic pain. In low-income health systems, patients may wait for vascular surgery. Continuous peripheral nerve block may be an effective alternative, with fewer side effects, in this scenario. A female patient with acute arterial obstruction of upper limb evolving with severe ischemic pain. She was submitted to a continuous infraclavicular brachial plexus block, which led to a satisfying pain control until the amputation surgery. The early postoperative period evolved with good pain management. This approach may be effecti

Acute celiac trunk thrombosis revealed by biliary peritonitis.

Acute thrombosis of the celiac trunk is a very uncommon condition, which is a life-threatening emergency. The clinical presentation is highly variable depending on the extent of the ischemic territory. We report a case of biliary peritonitis related to an acute thrombosis of the celiac trunk. This case highlights the role of abdominal computed tomography in the diagnosis of acute upper abdominal pain.

Diagnosing and managing acute diverticulitis.

Diverticulosis is the most common structural abnormality of the colon and affects up to one third of those over the age of 60 years. Its complications account for more than 3,000 deaths per year. Acute diverticulitis is the most common complication and 10-25% of patients with diverticulosis may have one or more episodes. Patients present with left-sided abdominal pain that localises in the left iliac fossa. This left-sided pain reflects the underlying predominant left-sided distribution of the diverticula on this side. Patients may also present with right-sided or suprapubic pain, caused eithe

Allodynia in migraine: frequent random association or unavoidable consequence?

Allodynia, the perception of pain induced by a non-painful stimulus, is frequently associated with migraine, especially when chronic, and mainly in the aura subtype. Among migraineurs, allodynia is thought to be caused by the headache and the activation of nociceptors with the development of central sensitization in subjects with an altered regulation of the central nociceptive pathway. The persistence of pain sensation seems to be able to induce central sensitization in the caudal nucleus of the trigeminal nerve by lowering the neuronal pain threshold. Different pathogenetic mechanisms may be

Who finds cervical laser therapy painful?

To identify women who find outpatient laser cervical surgery painful we designed a prospective observational study correlating surgical pain with multiple variables. The pain scoring systems were validated by comparing the subjective linear analogue score with an objective scoring method. The pain of surgery was compared with age, parity, acute anxiety scores, psychological state, premenstrual syndrome scores, phase of the menstrual cycle, contraceptive use, menstrual history, nature of the lesion, and intraoperative bleeding. Anxious women with no children suffered most. Nulliparity, acute pr

[Biomarkers in the diagnosis of cardiovascular emergencies : Acute coronary syndrome and differential diagnoses].

In emergency situations, patients present with symptoms rather than diagnoses. Due to its high prevalence, the acute coronary syndrome (ACS) dominates acute diagnostics as a consequence of its chief complaint chest pain. The challenge for the attending physicians is that only a minor part of patients with chest pain are finally diagnosed with an acute myocardial infarction (AMI) and that other rare but dangerous differential diagnoses have to be kept in mind and-vice versa-severely ill patients with AMI may present with symptoms other than chest pain. Against this background, the initial evalu
