"You rise up and then you start pulling people up with you": Patient experiences with a peer-delivered behavioral activation intervention to support methadone treatment.

BACKGROUND: Although medications for opioid use disorder (MOUD) are efficacious treatments for opioid use disorder, retention remains low. Peer recovery specialists (PRSs), individuals with lived substance use and recovery experience, may be particularly well-suited to support patients receiving MOUD. While PRSs are rarely trained in evidence-based behavioral interventions other than motivational interviewing, preliminary evidence suggests that peers can deliver brief behavioral interventions, such as behavioral activation, with efficacy and fidelity. This qualitative study sought to explore p

Machine learning-enabled behavioural and psychological symptoms of dementia management intervention for dementia caregivers: protocol for a hybrid factorial SMART-MRT trial.

BACKGROUND: Behavioural and psychological symptoms of dementia (BPSD) affect over 90% of people living with dementia and are a major contributor to stress and adverse health outcomes among family caregivers. Caregiver‑led behavioural management interventions based on the Antecedents-Behaviour-Consequences (ABC) framework are effective, yet their overall impact is modest. This may be due to the highly individualised and fluctuating nature of BPSD and insufficient adaptive support to sustain engagement. Mobile health interventions offer scalable support; however, engagement and adherence remain

Development of cognitive remediation interventions for people living with HIV in South Africa: participant experiences of adapted CogSMART and BrainHQ© programs.

INTRODUCTION: Previous studies suggest that cognitive remediation training (CRT) programs can help address neurocognitive impairment (NCI) and associated functional challenges in multiple populations. However, no such program has been developed for persons living with HIV in low- or middle-income countries (LMICs). METHODS: In this qualitative study, we aimed to describe how South African people living with HIV perceived and experienced the local adaptations of two US-designed CRT programs. We recruited 34 people living with HIV (27 women; age 30-50 years) from community healthcare clinics an

Frailty and heart failure: State-of-the-art review.

At least half of all patients with heart failure (HF) are affected by frailty, a syndrome that limits an individual ability to recover from acute stressors. While frailty affects up to 90% of patients with HF with preserved ejection fraction, it is also seen in ~30-60% of patients with HF with reduced ejection fraction, with ~26% higher prevalence in women compared with men. The relationship between frailty and HF is bidirectional, with both conditions exacerbating the other. Frailty is further complicated by a higher prevalence of sarcopenia (by ~20%) in HF patients compared with patients wit

A "motor learning based intervention for lower extremities (MOBILE)" to target walking performance in ambulant children with cerebral palsy: A feasibility study.

BACKGROUND: Cerebral Palsy (CP) is the largest contributor to childhood physical disability with abnormal gait pattern such as toe walking commonly reported. The International Classification of Functioning (ICF) in disabilities framework outlines three domains to consider when looking at impact of a disability on a child; body/structure, activity and participation. Activity and participation targeted interventions using Motor Learning Theory (MLT) have shown positive results on walking performance, gross motor skills and upper limb rehabilitation in CP. MLT encompasses multiple concepts that s

Complete diagnostic evaluation in colorectal cancer screening: research design and baseline findings.

BACKGROUND: While indicated by guidelines, complete diagnostic evaluation, or CDE (i.e., colonoscopy or combined flexible sigmoidoscopy plus barium enema X ray), is often not recommended and performed for persons with an abnormal screening fecal occult blood test (FOBT) result. We initiated a randomized trial to assess the impact of a physician-oriented intervention on CDE rates in primary care practices. METHODS: In 1998, we identified 1,184 primary care physicians (PCPs) in 584 practices whose patients received FOBTs that are mailed annually by a managed care organization screening program.
