[Consistency and inconsistency of psychosomatic symptoms in adolescents].

The term psychosomatic is sometimes mistakenly used as a virtual synonym of "somatoform", whereas there is a structural difference between alexythymia and hysteria. In adolescent psychopathology, where the interpretation of phenomena follows a dynamic and relational pattern, it is difficult to draw a distinction between psychosomatosis and hysteria. Pathological expressivities are often transient and do not depend on the course of an illness, despite being linked to moments of juvenile crisis in which the structural and dynamic reorganisation of the personality accentuates some regressive aspe

Restless legs syndrome in childhood and adolescence.

Restless legs syndrome (RLS) is believed to be a condition primarily of middle to older age. However, it can have its onset in childhood. Five illustrative case histories with an autosomal dominant mode of inheritance are described. A mother and her 3 children (age: 6 1/2, 4 and 1 1/2 years) as well as a 16-year-old patient from a second family have typical RLS signs of leg discomfort (paresthesias) and motor restlessness prevalent at night and at rest, with temporary relief by activity. Polysomnography or videotaping revealed periodic limb movements in sleep (PLMS) and, in some cases, involun

Assessing anger expression in children and adolescents.

Anger expression styles are associated with psychological and physical well-being among adults. Little is known about the role of anger expression in children's functioning. This lack of knowledge has resulted, in part, from a lack of validated tools for anger expression measurement. The Pediatric Anger Expression Scale-3rd edition (PAES-III; Jacobs, Phelps, & Rohrs, 1989; Jacobs & Kronaizl, 1991) has been proposed as a reliable and valid assessment instrument of anger expression styles. The PAES-III includes three scales that measure anger turned inward, anger expressed outwardly, and anger c

Premature exfoliation of teeth in childhood and adolescence.

Although the premature loss of primary teeth in conjunction with early eruption may be of no clinical significance, the loss of primary or permanent teeth in the absence of trauma should not be overlooked by the clinician. Premature loss of teeth associated with systemic disease usually results from some change in the immune system or connective tissue. This chapter presented some conditions associated with loosening and/or premature loss of teeth that may be encountered in children and adolescents. The most common of these conditions appear to be hypophosphatasia and early-onset periodontitis

Giant cell tumor in children and adolescents.

The radiological appearance of giant cell tumors (GCT) in the developing skeleton was retrospectively assessed in 49 (10.6%) of the 462 patients with GCTs seen in consultation by the Netherlands Committee on Bone Tumors. There were 31 female and 18 male patients, all below 19 years of age. Thirty-four tumors were located in short and long tubular bones, two in the tarsus, while the others were in the pelvis, vertebral spine, and a rib. Involvement of the epiphysis in tubular bones was closely related to the age of the patient: the average age of the 3 patients with a lesion in the metaphysis w

Colon cleanout preparations in children and adolescents.

To determine the effectiveness and associated side effects of a variety of bowel cleansing preparations before colonoscopy, 52 children, ages 4 months to 18 years, were evaluated. Data gathered included type of cleanout preparation used, adequacy of the preparations, and side effects. Twenty-two combinations of laxatives, lavages, enemas, and rectal suppositories were used. Side effects were reported in 90% of patients. In 79% of the patients the colon was clean or clean with some liquid feces, allowing a complete examination of the colon. Formed feces were present in 21% of the subjects, and

Obsessive compulsive disorder in children and adolescents.

During 1992, further progress has been made in determining the incidence of obsessive compulsive disorder and change in symptoms over time. Advances have been made in determining what category of mood disorder obsessive compulsive disorders belong to; contributions have been made in the theoretic understanding of the cause of obsessive compulsive disorder, and why it may be associated with trichotillomania and Tourette's syndrome. Finally, the treatment of children with both medication and behavior therapy has been explored.

[Attempted suicide by defenestration in children and adolescents].

In adults, suicidal attempts by jumping from elevated sites have been extensively studied and usually occur in patients with major psychiatric disorders such as schizophrenia or melancholia. Among the children and adolescents admitted to the Robert Debré Hospital in Paris for attempted suicide from 1989 through 1992, 8 (1 boy and 7 girls) had jumped out of a window. Ages ranged from 11 1/2 years to 15 years. Two patients had depression but in the six other cases there was no evidence of a psychiatric disorder. The suicidal attempt occurred after a prolonged period of conflicts within the famil

[Maternity in adolescents at high social risk].

OBJECTIVE: To analyse for the 1987-1990 period the indicators of prenatal care and the social-health follow-up of adolescent girls included in the mother-child programme of Ciutat Vella (Barcelona). This programme was aimed at pregnant women resident in the district and with high social risk profiles. DESIGN: Descriptive and retrospective study. MEASUREMENTS AND MAIN RESULTS: 175 adolescent mothers were included in the programme during the period under study. This represented 82.9% coverage of all births in the district to this age group. In 10.9% of cases the mother stated she lived alone.

Pituitary gonadal dysfunction in adolescents with varicocele.

Gonadotropin-releasing hormone stimulation testing was performed on 104 adolescent males with a unilateral left varicocele to determine the frequency of testicular dysfunction in this group of patients. An abnormal test result was noted in approximately 30 percent of varicocele patients. We believe that an abnormal test result indicates a higher risk for future fertility problems, and early treatment may reduce this risk.

[Acute spinal syndromes in children and adolescents].

23 children with acute spinal syndromes are presented. The clinical signs were caused by trauma in 11 patients, by extramedullary tumors in 6, by operations (coarctation of the aorta, scoliosis) in 4 and by encephalomyelitis in 2. Motor signs (flaccid paraplegia or paraparesis, monoplegia, bilateral weakness) and changes of reflex intensities were always observed. Most children presented with disturbances of sensory functions and micturition, some exhibited ataxia. In addition to the neurological signs, plain films of the spine were of diagnostic importance after trauma, for tumors myelography

Mania associated with fluoxetine treatment in adolescents.

Fluoxetine, a selective serotonin reuptake inhibitor, is gaining increased acceptance in the treatment of adolescent depression. Generally safe and well tolerated by adults, fluoxetine has been reported to induce mania. The cases of five depressed adolescents, 14-16 years of age, who developed mania during pharmacotherapy with fluoxetine, are reported here. Apparent risk factors for the development of mania or hypomania during fluoxetine pharmacotherapy in this population were the combination of attention-deficit hyperactivity disorder and affective instability; major depression with psychotic

Behavioral treatment of children and adolescents with trichotillomania.

Trichotillomania is a behavior disorder with onset generally in childhood, characterized by repetitive, compulsive pulling out of hair from the scalp, eyebrows, or other parts of the body, often leading to disfigurement. Trichotillomania tends to be persistent and is often resistant to counselling and standard psychiatric care. A systematic behavioral treatment program for children and adolescents and pilot findings with three patients are described. Methodological issues in relation to compliance and assessment are discussed. The relative safety and potential effectiveness of behavioral techn

Chronic pancreatitis beginning in childhood and adolescence.

We identified 10 patients with symptoms beginning before the age of 20 years in a group of 69 patients with proven chronic pancreatitis. Six of the 10 patients were women. There was a family history of pancreatitis in three patients and alcohol exposure in seven patients. Three patients were dependent on narcotics at the time of presentation. Six of the 10 patients had pancreatic duct dilatation to 10 mm or more in diameter during observation. These six patients underwent pancreaticojejunostomy, with clinical improvement in five patients. The median time of follow-up was 19 years from presenta

Prevalence of caries in adolescents in relation to diet.

The aim of the present study was to compare dietary habits between adolescents with high or low dental caries prevalence. All 15-yr-old adolescents living in a small community in Northern Sweden were studied in 1987, 1988, and 1989. The median values for energy and nutrient intake exceeded or were close to the recommendations (SNR89) given by the Swedish National Food Administration. The fiber intake did not reach the recommended daily amount even for the 90th percentile of the studied cohort. Nineteen percent of the adolescents had caries free teeth and 10% had nine or more decayed or filled
