Determinants of penicillinase producing Neisseria gonorrhoeae infections in heterosexuals in Amsterdam.

BACKGROUND AND OBJECTIVES: At the STD clinic of the Municipal Health Service in Amsterdam, the annual number of infections with penicillinase-producing Neisseria gonorrhoeae (PPNG) strains remained relatively stable from 1983 until 1990, while the number of non-PPNG infections declined dramatically. To investigate the PPNG and non-PPNG epidemic, a cross sectional study was conducted in 1989 and 1990. The purpose of this study was to assess determinants of PPNG infections among heterosexuals diagnosed with gonorrhoeae. METHODS: In addition to routinely collected data in new consultations for S

Health needs and concerns of male adolescents.

Lifestyle factors established within the family help determine health-care functioning. Adolescents first may be challenged to meet their own health needs as freshmen in college. A 153-item questionnaire was utilized to examine concerns in the areas of alcohol and other drug use, auto safety, weight and dieting, smoking, sexuality, coping and stress, and selection and utilization of health-care services. One hundred fifty-nine male college students responded. Major problems with alcohol use, auto safety, weight control, stress and sexuality were identified. Positive lifestyle factors that were

Preventive measures in the tertiary care of spinal cord injured people.

There are complications of spinal cord paralysis peculiar to the extended care period. These may be motor skeletal, neurogenic, visceral and psychogenic. If practised regularly, prevention can be very effective in reducing the disability in all groups. Limb oedema, joint contractures, myasthenia and pain can be materially reduced by regular activity, maintaining joint mobility and the use of recreational motor skeletal activities. Urinary tract infection and decubiti can be largely eliminated by careful attention to anti-bacterial suppression and better hygiene, both personal and at home. Decu

Analgesic abuse, renal papillary necrosis and concomitant drug intake.

In a retrospective survey of 145 patients coming to autopsy at a hospital in Sydney, 31 were found to have some form of renal papillary necrosis (RPN). Sixteen (52%) of those abused analgesic mixtures, there being a marked female preponderance (70%). A further two cases may have been linked with indomethacin and phenylbutazone. On the other hand, 15 (48%) of other analgesic abusers did not develop RPN and the differences between the two groups of abusers and a thrid group of non-abusers were studied in the hope of isolating a precipitating factor. Examination of concomitant drug intake reveale

Evaluation of nasal symptoms to distinguish eosinophilic from noneosinophilic nasal polyps based on peripheral blood.

Background: Patients with eosinophilic chronic rhinosinusitis with nasal polyps (eCRSwNP) have poorer outcomes after endoscopic sinus surgery and a higher recurrence rate. Objective: This study aimed to investigate the profile of clinical symptoms of eCRSwNP and the related risk factors. Methods: We prospectively enrolled 298 inpatients with CRSwNP from February 2019 to December 2019. The patients were divided into eCRSwNP and non-eCRSwNP groups based on the percentage of blood eosinophils; the cutoff value was set at 3.05%. Clinical data on questionnaires, visual analog scale (VAS) scores, an

Evaluating the impact of Extension of Community Healthcare Outcome (ECHO) telementoring on enhancing knowledge and perceived skills in alcohol use disorders among the nonmedical District Mental Health Programme healthcare providers in Karnataka.

BACKGROUND: Alcohol use disorder (AUD) is a public health problem. In India, about 5.2% of the population aged 10-75 years, that is, approximately 5.7 crore individuals, need help for their alcohol use problems, and around 20 lakhs from Karnataka. As per the studies, among people dependent on alcohol who tried quitting, about 75% did not receive any treatment. One potential approach to reducing this gap is enhancing the knowledge and skills of the existing District Mental Health Programme (DMHP) Health Care Providers (HCPs) on a large scale by integrating a case-based tele-ECHO (Extension of C

Repetitive blast mild traumatic brain injury increases ethanol sensitivity in male mice and risky drinking behavior in male combat veterans.

BACKGROUND: Mild traumatic brain injury (mTBI) is common in civilians and highly prevalent among military service members. mTBI can increase health risk behaviors (e.g., sensation seeking, impulsivity) and addiction risk (e.g., for alcohol use disorder (AUD)), but how mTBI and substance use might interact to promote addiction risk remains poorly understood. Likewise, potential differences in single vs. repetitive mTBI in relation to alcohol use/abuse have not been previously examined. METHODS: Here, we examined how a history of single (1×) or repetitive (3×) blast exposure (blast-mTBI) affect

Psychosocial Interventions for Alcohol Use Disorder: A Review of Efficacy, Mechanisms, and Clinical Implications.

Alcohol use disorder (AUD) is a prevalent global health challenge associated with substantial morbidity, mortality, and socioeconomic burden, yet significant gaps remain in access to effective treatment. Psychosocial interventions are fundamental components of evidence-based care for AUD and play a critical role in promoting abstinence, reducing harmful alcohol consumption, preventing relapse, and improving psychosocial functioning. Despite the availability of several established therapeutic approaches, comprehensive syntheses comparing their effectiveness, mechanisms of action, and emerging a

Age-related impact of outcomes in hospitalized patients with alcohol overuse.

BACKGROUND: The incidence of alcohol use disorder (AUD) and related complications, such as alcohol-associated liver disease and alcohol-associated cirrhosis, continues to rise in the United States. However, limited data exist on the impact of AUD in aging hospitalized populations. We aimed to evaluate the association between AUD and clinical outcomes in aging hospitalized patients. METHODS: We analyzed 2017-2020 data from the Nationwide Inpatient Sample, including adults aged 18-80 with and without AUD based on ICD-10 codes (AUD: n = 523,464; general medical population [GMP]: n = 238,678). Vincamine attenuates alcoholic liver injury through modulation of a CDK1-glycolysis-NLRP3 immunometabolic axis.

BACKGROUND: Alcoholic liver disease (ALD) is the major immunometabolic disorder characterized by dysregulated lipid metabolism and persistent activation of hepatic innate immune responses. Vincamine (Vin), isolated from Vinca minor, has been reported to exhibit diverse pharmacological activities. Nevertheless, the immunomodulatory effects and underlying mechanisms of Vin on ALD remain poorly understood. PURPOSE: This study aimed to explore the protective effects of Vin against alcohol-induced hepatic injury, with a particular focus on immunometabolic regulation of inflammasome activation. ME

Carbohydrate-Deficient Transferrin Evaluation Over Time Has Usefulness for Confirming Sobriety of Alcohol Rehabilitation Program Patients.

BACKGROUND: For treating severe alcohol-related liver disease (ALD), abstinence from alcohol is essential. Because underreporting can occur, abstinence biomarkers are necessary during hepatology outpatient service. This study assesses whether serial changes of the serum carbohydrate-deficient transferrin to transferrin ratio (%CDT) reflect patient abstinence. METHODS: We retrospectively investigated serial %CDT measurements and laboratory data from two outpatient settings: patients in an alcohol rehabilitation program (A), and ALD patients at a hepatology clinic (H) who attempted abstinence w

Fenofibrate treatment during withdrawal reverses symptoms of ethanol-induced depression in male rats.

Alcohol use disorder (AUD) and major depression frequently co-occur, both involving significant neuroinflammatory components. Current treatments are often ineffective in addressing AUD-related depression, highlighting the need for novel therapeutic approaches. Previous studies showed that fenofibrate, a peroxisome proliferator-activated receptor alpha (PPAR-α) agonist, reduces voluntary alcohol intake and attenuates neuroinflammation and oxidative stress in alcohol-preferring rats. This study investigated whether fenofibrate administered during alcohol withdrawal could alleviate ethanol-induce

The Impact of Alcohol Use on Frontal White Matter in HIV.

BACKGROUND: Alcohol use disorder (AUD) is prevalent among individuals diagnosed with human immunodeficiency virus (HIV), and both HIV and alcohol use have been shown to negatively affect the integrity of white matter pathways in the brain. Behavioral, functional, and anatomical impairments have been linked independently to HIV and alcohol use, and these impairments have bases in specific frontally mediated pathways within the brain. METHODS: Magnetic resonance imaging data were acquired for 37 HIV+ participants without dementia or hepatitis C. Imaging data were processed through the FreeSurfe

Puerarin ameliorates experimental alcoholic liver injury by inhibition of endotoxin gut leakage, Kupffer cell activation, and endotoxin receptors expression.

Puerarin, an isoflavone component extracted from Kudzu (Pueraria lobata), has been demonstrated to alleviate alcohol-related disorders. Our study examined whether puerarin ameliorates chronic alcoholic liver injury through inhibition of endotoxin gut leakage, the subsequent Kupffer cell activation, and endotoxin receptors expression. Rats were provided with the Liber-DeCarli liquid diet for 8 weeks. Puerarin (90 mg/kg or 180 mg/kg daily) was orally administered from the beginning of the third week until the end of the experiment. Chronic alcohol intake caused increased serum alanine aminotrans
