We describe the case of a 42-year old female patient with a carcinoma of the adrenal cortex. The primary tumor was resected without residual tumor tissue; only two weeks later there was a new large tumor formation in the adrenal gland's bed. PET-investigation showed the large local recurrency, multiple lung-, and liver-metastasis, so that no further operative therapy was performed. FDG-PET seems to be suitable for diagnosis and staging of adrenal cortex carcinoma in one single examination.

We evaluated the change of coronary flow reserve using a Doppler guidewire before and after coronary artery bypass grafting to assess the coronary hemodynamic effect of surgical revascularization in a 13-year-old boy with congenital atresia of the left coronary ostium, which is one of the rarest of the congenital coronary anomalies. Coronary flow reserve in the right coronary artery and left anterior descending artery increased significantly after coronary revascularization, and a microvascular bed developed in the left anterior descending artery.

In cats, distention of an isolated segment of the ileum with a saline injection elicited its contraction which could be abolished with blockade of muscarinic receptors. Serotonine acting from the ileum lumen enhanced the response, whereas acting from the vascular bed induced a contractile response which could be decreased with the beta-adrenoreceptor-blocking agent propranolol. Bile leakage from the gall-bladder bed following laparoscopic cholecystectomy is recognized as a potential cause of major morbidity. This problem is usually attributed to division of ducts draining a portion of liver directly into the gall-bladder. The authors' experience, however, and a review of the literature suggests that injury to a superficial duct draining into the major hepatic ducts rather than the gall-bladder is the principal cause of this problem. Three patients with bile leakage demonstrated on postoperative biliary tract imaging are presented with a discussion of the literature.

A 17 month-old girl was found dead in her bed. The findings of a juditial autopsy revealed that she had a cleft palate, low-set and accessory ears, and syndactyly of right and left second and third toes and right third and fourth toes. Her birth size was small and subsequently she failed to thrive. Organ anomaly, such as atriaol septal defect, accessory spleens, and uterus bicornis bicollis were found. From these findings, this case was diagnosed as a Smith-Lemli-Opitz syndrome.

While largely successful, the use of a conjunctival autograft in the exposed scleral bed following a pterygium excision can result in postoperative complications. We report two cases of a previously undescribed complication: subconjunctival fibrosis at the harvest site of the graft. In one case, the fibrosis caused an asymptomatic, purely cosmetic defect; however, in the other case, the scar tissue constricted the extraocular movement with resultant diplopia. We call attention to this complication as it can affect visual function; we discuss its management and prevention.

The effect of emoxypin on angiogenesis in rabbit cornea in aseptic inflammation induced by intracorneal implantation of a piece of quartz and on the development of the vessels of the chick embryo yolk sac was studied. 1% emoxypin pipetted thrice a day for 10-14 days inhibited corneal neovascularization and reduced the formation of new blood vessels. We observed an inhibitory effect on the development of vascular bed of the embryo yolk sac on incubation hour 64-72. The drug affected neither general growth of the embryos no the number of somites.

The authors report a case of total anomalous pulmonary venous return successfully treated by surgery in a 61 year-old man. This case is unusual because of the late discovery of this congenital malformation which is usually rapidly fatal in the absence of surgical correction. This prolonged survival can be explained by the large atrial septal defect and the absence of obstruction, to pulmonary venous return and associated malformations. Surgical repair is essential to prevent the development of irreversible lesions of the right side of the heat and the pulmonary arterial bed.

This report discusses the authors' technique in performing anterior cervical vertebrectomy and interbody fusion for multilevel cervical disease. The technique is performed with a high-speed drill and bone-bank fibular strut graft. After decompression of the cervical canal, ledges are made in the intact vertebral bodies to create a rectangular bed for safe seating of the bone graft. The bone-bank fibular strut graft is a feasible alternative to autograft. The simplified and safe nature of this procedure reduces postoperative morbidity as well as the length of hospital stay.

Sexually transmitted diseases can be transmitted through nonsexual transmission. Neonatal contamination has been proven at parturition from mothers with anogenital diseases. Another example is transmission through exchange of underwear, towels, bed sharing, etc. It is possible that contamination may occur with use of the contaminated speculum and forceps. Use of laser may release viruses during treatment for viral diseases. Candidiasis, trichomoniasis and genitalis condilomatosis, which should not be considered an exclusively sexually transmitted diseases are considered.

It is shown that plasmapheresis (PA) as an adjuvant modality in combined treatment of idiopathic purpura resistant to glucocorticosteroids helps to partially overcome resistance and induces a positive trend in the clinical and hematological indices. Eighty-two patients with cervical incompetence were managed with MacDonald's cervical cerclage, bed rest, tocolytics and sedatives. 78 of the patients were followed upto termination of pregnancy and their pregnancy data recorded. 69.5% delivered at term. 78.1% were discharged from hospital with live babies. The incidence of operative delivery was encountered. MacDonald's cervical cerclage has a high success rate in a rural setting, well comparable to that in urban settings and prolonged in-patient treatment is not always necessary.

We describe a case of Budd-Chiari Syndrome in a 6-year-old boy secondary to Wilm's tumor. The patient had a right nephrectomy and mediastinotomy with removal of the tumor from the right atrium, inferior vena cava, and hepatic vein. Postoperatively, the patient had chemotherapy consisting of Actinomycin-D to be followed by radiation to the tumor bed to a total dose of 2000 rads in 10 fractions by using AP/PA field on 6 MeV Linear Accelerator. Currently, the child is receiving combination chemotherapy.

Life-threatening episodes of oronasal obstruction were observed in five bedridden, cachectic and dehydrated nursing-home patients with deviation of the nasal septum. During the episode they had swollen nasal mucosae and rhinorrhoea. The patients were found in bed with the nostrils sucked against the septum and the mouth tightly closed. They were cyanotic but conscious. After introduction of disposable catheters through the nose, dyspnoea and cyanosis were resolved in a few minutes and the patients were able to open the mouth spontaneously.

Chronic myeloid leukaemia was discovered in a female child aged 12 years, whose presenting signs and symptoms were those of Legg-Calve-Perthes' disease in the left hip. The chronic myeloid leukaemia responded dramatically to myeleran therapy, whilst the Legg-Calve-Perthes' disease was treated by bed rest in hospital followed by weight-relieving paten-ended calliper. A rare case of CML in a female child who has 'Legg-Calve-Perthes' disease of the left hip in association is described.

A middle-aged man presented with recurrent alveolar hemorrhage, rheumatoid arthritis, and crescentic immune complex-mediated glomerulonephritis. Nail bed hemorrhages and necrotizing vasculitis were documented concomitantly with the clinical picture of Goodpasture's syndrome. Perusal of the literature disclosed two additional similar cases. It is suggested that Goodpasture's syndrome may evolve in the framework of rheumatoid vasculitis. While immunosuppressive therapy resulted in short-term remission, two of the three patients involved developed late-stage renal failure.

Pterygium inversum unguis is a nail abnormality in which the distal aspect of the nail bed/hyponychium is adherent to the ventral surface of the nail plate, resulting in obliteration of the distal groove. We present a 50-year-old man who developed this abnormality on his right fingers and toes after a stroke that resulted in paresis of his right side. Unusual features of this case include the association with a neurologic disorder, assymetric distribution, involvement of toes as well as fingers, and occurrence in the male gender.

In glioblastoma multiforme and high-grade astrocytomas the following procedure has been implemented to try and prevent recurrence: after extirpation of the tumour its bed is lined with metal powder and the patient's head is exposed to a high-frequency magnetic field every 4 weeks. As a result the temperature is raised by induction to +45 degrees C at the boundary between metal and brain tissue. This ought to be sufficient to prevent tumour recurrence. Varied results were achieved using this technique in 103 patients.
