Interlayer-aware postoperative facial appearance prediction in orthognathic surgery with bio-geometric guidance.

Objective.Accurate prediction of postoperative facial appearance is essential for orthognathic surgical planning, yet remains challenging due to the nonlinear biomechanical coupling between bone and soft tissue. While deep learning methods offer a faster alternative to traditional biomechanical simulation, they typically map bony displacements directly to facial surface deformation, overlooking the intervening soft-tissue layers with distinct biomechanical properties through which displacement is progressively transmitted. Moreover, the many-to-one effect of regional bony movements on each sof

Analysis of the correlation between lower limb muscle synergy characteristics and landing load during single-leg landing in humans.

To explore the correlation between the characteristics of lower limb muscle synergies and landing loads during the single-leg landing process in humans. Lower limb electromyographic data were collected from 21 male athletes during single-leg landing. The non-negative matrix factorization algorithm was used to extract analytical indicators, including the number of participating muscle synergies, the time to peak activation of muscle synergies, and the relative muscle weights. Additionally, the associations between the above indicators and the loads during human single-leg landing were analyzed.

Ischemic preconditioning alters gait mechanics in older adults with knee osteoarthritis.

BACKGROUND: Individuals with knee osteoarthritis exhibit altered gait mechanics. While physical activity is recommended as a first-line treatment, few interventions outside of exercise have demonstrated significant gait changes. This study evaluated changes in gait mechanics after a single session of ischemic preconditioning in participants with knee osteoarthritis. METHODS: Thirty-one participants with knee osteoarthritis over the age of 50 completed gait analysis before and after a single session of ischemic preconditioning(n = 15) or sham(n = 16). The treatment consisted of 5-min of inflat

Body mass index moderates the effects of midsole hardness on metatarsophalangeal joint biomechanics during running in male recreational runners.

Whether body mass index (BMI) modifies biomechanical responses to midsole hardness remains unclear. This study tested whether BMI moderates the effects of midsole hardness on lower-limb biomechanics in male recreational runners, with particular focus on metatarsophalangeal (MTP) joint responses at the foot-shoe interface. Thirty male recreational runners were stratified into low-, medium-, and high-BMI groups and ran at 2.7 m/s in shoes with three midsole hardness levels (40C, 53C, and 65C). Joint kinematics, kinetics, stiffness, and ground reaction force variables were analyzed. BMI significa

Comparison of three fixation methods for posterolateral fragments in tibial plateau fractures: a biomechanical study.

BACKGROUND: Tibial plateau fractures involving posterolateral fragments are challenging to fix internally because of the small surface area and the complex regional anatomy. This study aimed to compare the biomechanical performance of three plate fixation methods and to identify the construct most resistant to fracture displacement under axial loading. METHODS: Fifteen synthetic tibia models were used to simulate posterolateral split fractures. The models were randomized into three groups (n = 5 per group): Group A received an R-LOCK locking plate with 3.5 mm locking screws; Group B received

In vivo quantification of knee-orthosis valgus-corrective moment during gait: Effects of medial strut length on external knee adduction moment in medial knee osteoarthritis.

Osteoarthritis (OA) is a progressive degenerative disease affecting articular surfaces, and knee orthoses are commonly employed to reduce the external knee adduction moment (EKAM). However, quantitative data on the corrective moments generated by orthoses during gait and their relationship with EKAM remain limited. This study quantified the corrective moments generated by a knee orthosis using a six-axis force sensor and evaluated the impacts of medial lower-leg strut length on these moments and EKAM, aiming to elucidate the biomechanical mechanisms by which orthotic design facilitates unloadi

Artificial intelligence predictions of knee kinematics, kinetics, and internal biomechanics during walking in people with knee osteoarthritis: A systematic review and meta-analysis.

This review synthesizes current literature on the use of artificial intelligence (AI) to predict knee biomechanics during walking in people with knee osteoarthritis (OA). Four databases were searched from inception to 22/01/2025. Risk of bias was assessed using a modified Newcastle-Ottawa Scale. Study quality was assessed using Grading of Recommendations, Assessment, Development, and Evaluations. Gait spatiotemporal parameters, knee kinematics, knee kinetics, and knee internal biomechanics calculated with both AI and physics-based methods were compared using root mean squared error (RMSE), nor

Mapping fusion zones to motion resistance: Finite element insights into return-to-activity after interbody fusion.

Lumbar interbody fusion outcomes depend on the spatial distribution and maturity of the fusion mass relative to direction-specific loading. Using finite element analysis of a pedicle screw-rod instrumented L4-L5 construct with a central PEEK cage, we quantified how localized fusion in anterior (A), posterior (P), and left/right lateral (L/R) zones affect stiffness under flexion-extension, lateral bending, and torsion. 11 fusion configurations (one 4-zone; four 3-zone; and six 2-zone) were tested using 20 Latin Hypercube-sampled fusion moduli from a trabecular bone distribution. Stiffness ratio

Thoracolumbar Instrumentation Options: Biomechanical Implications.

Pedicle screws and rods are the cornerstone of spine instrumentation. This article focuses on the practical usage of these implants in contemporary spine practice. Furthermore, we discuss strategies to optimize fixation in osteoporotic spine and implant-related considerations in preventing proximal junctional kyphosis. Cross-sectional imaging helps select the longest screws with maximum outer diameter that can be safely placed. Conical inner diameter with V-shaped thread provides best pullout strength. Dual-core, dual-thread screws are biomechanically superior to conventional screws. For pedic

Biomechanical comparison of oblique lateral interbody fusion with interspinous versus other fixation techniques: A finite element study.

BACKGROUND: Oblique lateral interbody fusion (OLIF) is a minimally invasive technique for treating degenerative lumbar diseases, yet the optimal supplemental fixation strategy remains debated. Interspinous fixation systems (ISFS) offer a less invasive alternative to conventional pedicle screw-based constructs, but their biomechanical performance when combined with OLIF is not well characterized. METHODS: A validated finite element model of the L3-S1 lumbar spine was used to simulate OLIF at L4-L5 with 5 fixation configurations: stand-alone, unilateral pedicle screw fixation, bilateral pedicle

Evaluation of a novel C2 pedicle screw insertion technique: a retrospective comparative clinical study and finite element analysis.

C2 pedicle screws (C2PS) are extensively used in the treatment of upper cervical spine disorders owing to their superior mechanical properties. However, incorrect screw insertion in the C2 pedicle can lead to catastrophic consequences due to the proximity to critical structures such as the vertebral artery groove and spinal cord. There is currently no universally accepted method for precise C2PS placement. In this study, we evaluated a novel free-hand C2PS insertion technique that uses the superior margin of the C2 lamina and the medial margin of the C2 pedicle as landmarks, thereby avoiding r

Biomechanical analysis of diamond-like carbon coated growth-guidance spinal system under cyclic axial compression.

PURPOSE: Growth-guidance spinal systems are used in children with progressive spinal deformity to reduce repeated surgical lengthening while allowing continued spinal growth. Their long-term biomechanical behaviour under cyclic loading remains insufficiently characterised. This study assessed whether diamond-like carbon (DLC) coating of rods and sliding screws affects axial stiffness and local kinematics in a growth-guidance spinal system. METHODS: Twelve porcine Th11-L7 spine specimens were assigned to three groups: non-instrumented control, non-coated titanium alloy instrumentation, and ins

Cervical implant fixation: a topical review of techniques and their importance.

Cervical implant fixation is a critical surgical intervention for stabilizing the cervical spine, often necessitated by trauma, degenerative diseases, or spinal deformities. While spinal disc disease has historically been treated with fusion-based procedures, there has been a recent surge of interest in motion-preserving disc arthroplasties. The present study provides a topical narrative review of selected and recent literature on cervical implant fixation techniques, including anterior and posterior approaches, implant materials, biomechanical considerations, and reported clinical outcomes. Progressive collapsing foot deformity: an instructional review.

Progressive collapsing foot deformity (PCFD) is an acquired condition characterised by collapse of the medial longitudinal arch, hindfoot valgus and forefoot abduction. This condition predominantly affects middle-aged and older patients and can lead to significant pain and functional impairment. Clinical assessment is key to identifying a flexible versus rigid foot deformity. The Johnson and Strom classification provides a framework for staging the disease process and guides management. Treatment can involve non-operative measures such as orthoses, or operative measures tailored to the compone

Three-dimensional motion analysis of a total wrist prosthesis during the dart-throwing motion: a cadaveric study.

Total wrist arthroplasty offers an alternative to wrist arthrodesis by preserving motion while relieving pain, and understanding its functional performance is clinically important. The DARTS wrist prosthesis, approved for clinical use in Japan in 2017, was designed to reproduce the physiological dart-throwing motion through its semi-constrained structure and ovoid carpal component. Because this motion is essential for wrist function, it is important to determine whether the prosthesis can adequately replicate it. This study analyzed the detailed kinematics of the DARTS wrist prosthesis during
