A sensitivity analysis approach to principal stratification with a continuous longitudinal intermediate outcome: applications to a cohort stepped wedge trial.

Causal inference in the presence of intermediate variables is a challenging problem in many applications. Principal stratification (PS) provides a framework to estimate principal causal effects (PCE) in such settings. However, existing PS methods primarily focus on settings with binary intermediate variables. We propose a novel approach to estimate PCE with continuous intermediate variables in the context of stepped wedge cluster randomized trials (SW-CRTs). Our method leverages the time-varying treatment assignment in SW-CRTs to calibrate sensitivity parameters and identify the PCE under real

A Mendelian randomization study of genetic liability to cutaneous melanoma and sunburns.

BACKGROUND: Some studies have reported that sunburns and cutaneous melanoma (CM) risk is increasing, but a clear causal link has yet to be established. METHODS: This current study conducted a two-sample Mendelian randomization (MR) approach to clarify the association and causality between sunburn history and CM using large-scale genome-wide association study data. RESULTS: The inverse-variance weighted method result showed that sunburn might be associated with the risk of CM increasing (p = 2.21 × 10-23, OR = 1.034, 95% CI= 1.027-1.041), causally. The MR-Egger regression, weighted median met

Key Questions About Familial Hypercholesterolemia: JACC Review Topic of the Week.

Familial hypercholesterolemia (FH) is characterized as a monogenic, autosomal dominant disorder, producing severe hypercholesterolemia within families due to causal variants within genes regulating the low-density lipoprotein receptor pathway. Demonstration of a causal variant is widely accepted as evidence of substantially higher cardiovascular risk. However, recent large-scale population studies challenge this characterization of FH, which appears to account for only a minor portion of those with severe hypercholesterolemia. Moreover, a substantial portion of FH variant positive patients do

The Home Observation Measure of the Environment is associated with symptoms of ADHD and oppositionality in a CAMHS sample.

BACKGROUND: Severe early deprivation has a causal role in attention-deficit/hyperactivity disorder (ADHD). Adversity in the home is associated with increased ADHD and oppositional symptoms in children with ADHD. We aimed to replicate this in an independent clinic sample. METHODS: A total of 247 sequential families with a child referred to child and adolescent mental health services (CAMHS) were invited to participate; 100 families completed the study. The Home Observation Measure of the Environment (HOME) assessment was completed in the family home; parents completed the Conners' Rating Scale

A three-phase model of the social emotional functioning in eating disorders.

BACKGROUND: Problems with social emotional functioning are an important part of eating disorder psychopathology. AIM: This study aimed to propose a model of social emotional functioning before and during the illness and to explain the consequences for those involved. METHOD: We propose a three-phase model of social and emotional processes as both causal and maintaining factors in anorexia nervosa. The predictions from this model are examined, and we consider the relevance for treatment. RESULTS: The evidence base for the theoretical model is presented: Phase 1 describes causal predispositio

Systolic and diastolic blood pressure, abdominal obesity, and alcohol consumption contribute to the risk of hypertrophic and dilated cardiomyopathies in European and East Asian populations.

BACKGROUND: Potentially modifiable cardiovascular risk factors, including hypertension, diabetes, obesity, alcohol consumption, and smoking initiation, are associated with hypertrophic (HCM) and dilated (DCM) cardiomyopathies, representing promising targets for interventions. However, the causality and cross-ancestry generalizability of these associations remain uncertain. METHODS: Collecting summary-level data from genome-wide association studies for systolic (SBP) and diastolic (DBP) blood pressure, hypertension, type 2 diabetes, fasting glucose, glycated hemoglobin, body mass index (BMI),

Inhibition of lactylation by LRP1 expression increases the risk of intervertebral disc degeneration: A multi-omics summary-based Mendelian randomization analysis.

Intervertebral disc degeneration (IVDD) causes neck, back and low back pain, and is a major global public health problem. Lactate, a metabolic product of disc cells, relates closely to degeneration. Lactylation, driven by lactic acid, associates with IVDD; targeting related genes may offer new therapies. This study aimed to identify causal lactylation-related genes in IVDD via multi-omics summary-based Mendelian randomization (SMR). In this study, data from a genome-wide association study (GWAS) were combined with methylation quantitative trait loci (mQTL) and expression quantitative trait loc

Inflammatory bowel disease and risk of autoimmune hepatitis: A univariable and multivariable Mendelian randomization study.

OBJECTIVE: This study aimed to use Mendelian randomization (MR) to investigate the potential causal association between inflammatory bowel disease (IBD) and autoimmune hepatitis (AIH). METHODS: Two-sample MR was performed to estimate the causal effect of IBD on AIH. The primary analysis employed the inverse variance weighted (IVW) method in univariable MR analysis, supplemented by additional methods including MR-Egger, weighted median, simple mode, and weighted mode. The p values were adjusted by FDR p-value adjustment. In the replication analysis, the primary IVW analysis was repeated and th

The Association between Rheumatic Diseases and the Risk of Polycystic Ovary Syndrome: A Two-Sample Mendelian Randomization Analysis.

Aims/Background The association between rheumatic immune diseases and polycystic ovary syndrome (PCOS) remains elusive. The purpose of this study was to investigate the causal relationship between rheumatic immune diseases and the risk of PCOS through a two-sample Mendelian randomization (MR) analysis. Methods In the assessment of exposure variables, we chose systemic lupus erythematosus (SLE), polymyositis (PM), and rheumatoid arthritis (RA) as representative rheumatic immune diseases, while PCOS was designated as the outcome of interest. All data utilized in this investigation were obtained

Hearing loss is not associated with risk of Parkinson's disease: A Mendelian randomization study.

PURPOSE: A few observational studies have indicated that Parkinson's disease (PD) risk may be higher in those with hearing loss, but the two's causal relationship is yet unknown. Using Mendelian randomization (MR) methods, this study sought to explore the causal link between hearing loss and the risk of PD. METHODS: We identified single nucleotide polymorphisms (SNPs) linked to hearing loss (P-value<5E-08) in a genome-wide association study (GWAS) included 323,978 people from the UK Biobank. The summary data for PD in the discovery group came from a GWAS meta-analysis of 33,647 cases and 449,

[The plague].

The word plague evokes associations with old times, but plague actually does still occur all over the world. The survival chances are at the moment much better (antibiotics) than formerly, when half of the patients died; now this figure is 15%. The causal organism is Yersinia pestis, a bacterium in fleas on rodents. This bug is a plausible candidate for attacks by bioterrosists. So, in this time, there is good reason to pay attention to it, as well as to to smallpox and anthrax and their causal organisms.

Discovery of critical thresholds in mixed exposures and estimation of policy intervention effects.

Regulations of chemical exposures often focus on individual substances, neglecting the amplified toxicity that can arise from multiple concurrent exposures. We propose a novel methodology to identify critical thresholds in multivariate exposure spaces and estimate the effects of policy interventions that limit exposures within these thresholds. Our approach employs a recursive partitioning algorithm integrated with targeted maximum likelihood estimation (TMLE) to discover regions in the exposure space where the expected outcome is minimized or maximized. To address potential overfitting bias f

Cross-trait analysis reveals shared genetic architecture between mental health and cardiovascular traits.

BACKGROUND: The association between cardiovascular disease (CVD) and mental health (MH) is well established. However, the shared genetic architecture and underlying causal relationships between these conditions remain insufficiently understood. MATERIAL AND METHODS: Summary statistics from Genome-wide association studies (GWAS) for 8 MH traits and 10 cardiovascular system (CVS) traits were used in the analyses. LDSC regression was applied to estimate heritability and genetic correlations. Bidirectional Mendelian randomization (MR) analysis was conducted to explore potential causal relationshi

Association between genetically predicted dietary patterns and atrial fibrillation: a two sample Mendelian randomization study.

BACKGROUND: The impact of dietary habits on atrial fibrillation (AF) remains unclear, and observational findings are often inconsistent. Clarifying the causal relationships between specific dietary habits and AF may provide insights into disease etiology and primary prevention. OBJECTIVES: This study aimed to investigate the causal effects of 20 dietary habits on AF using Mendelian randomization (MR) approaches. METHODS: Genetic instruments for 20 predefined dietary habits, including alcohol, processed meat, pork, poultry, beef, fish, bread, cheese, vegetables, fruits, coffee, water intake, an

Directional genetic relationships between obsessive-compulsive disorder and bipolar disorder and schizophrenia.

OBJECTIVE: Obsessive-compulsive disorder (OCD) frequently co-occurs with bipolar disorder (BD) or schizophrenia (SCZ), and, importantly, can often precede their onset. However, the genetic architecture and directionality underlying these relationships remain unclear. We leveraged large-scale genome-wide association study (GWAS) data to examine shared genetic architecture and directional relationships among OCD, BD and SCZ, and used major depressive disorder (MDD) as a comparator. Metabolites and Polycystic Ovarian Syndrome: A Mendelian Randomization Study.

INTRODUCTION: Polycystic ovarian syndrome (PCOS) is a common reproductive disorder that affects a considerable number of women worldwide. Nevertheless, the causal relationship between metabolites and PCOS remains undetermined. METHODS: We utilized a comprehensive two-sample Mendelian randomization (MR) analysis, a genetic epidemiological approach that uses genetic variants as instrumental variables to assess causal relationships between exposures and outcomes, to examine the causal link between 1352 metabolites and PCOS. We employed complementary MR methods, such as the inverse-variance weigh

Major depression and atherosclerotic disease: Linking shared genetics to pathways in blood, brain, heart, and atherosclerotic plaques.

The increased risk of atherosclerotic diseases (stroke, coronary artery disease [CAD]) observed in depression may stem from shared pathophysiology. We examined whether: 1) major depression (MD) and atherosclerotic traits share genetic risk, and 2) altered gene expression in various tissues linked to shared genetics has a potential causal role in depression etiology. Data from the largest genome-wide association studies of MD (N = 3,887,532) and 8 atherosclerotic traits (N = 26,909-1,308,460) were used in Two-Sample Mendelian randomization and colocalization to detect cross-trait causal associa
