Parents' perceptions of pediatric day surgery risks: unforeseeable complications, or avoidable mistakes?

A decline in people's trust of the US health care system has been documented. But do parents of pediatric patients have specific safety worries? If so, what are they? And what cultural factors inform them? To help answer these questions, in San Diego 35 English- and Spanish-speaking parents' (or guardians') self-reports of perceptions of their child's risk for experiencing a medical error during day surgery were collected using open-ended rapid assessment interviews, and analyzed using qualitative content analysis methods. (The innovative data collection and analysis protocol is described so a

Economic burden of congenital athymia in the United States for patients receiving supportive care during the first 3 years of life.

AIMS: Congenital athymia is an ultra-rare pediatric condition characterized by the lack of thymus in utero and the naïve T cells critical for infection defense and immune regulation. Patients with congenital athymia receive supportive care to minimize and treat infections, autoimmune phenomena, and autologous graft-versus-host disease (aGVHD) manifestations, but historically, die within the first 3 years of life with supportive care only. We estimated the healthcare resource utilization and economic burden of supportive care over patients' first 3 years of life in the United States. METHODS:

Factors influencing the implementation of health care reform: an examination of the Concurrent Care for Children provision.

In the wake of the 2013 Presidential reelection, states now face the reality of implementing the healthcare reform, including ACA section 2302 (Concurrent Care for Children). The purpose of our study was to examine the influence of economic, political, and legal factors on state implementation of ACA 2302. Using data from 2010 to 2012, our analysis revealed that for early implementers economic, political or legal factors did not influence implementation of ACA 2302. In 2011, states that were engaged in Medicaid cost containment efforts were more likely to implement ACA 2302 and in 2012, states

Germ factories or immune boot camps? Infection and immunity in childcare settings.

SUMMARYChildcare outside the home is a common experience for many children in high-income countries. It is associated with an increase in the incidence of infectious diseases-not just for the child but also for their parents and other household members. In this review, we explore this phenomenon from multiple angles, combining age at first infection, maternal antibody dynamics, seroepidemiology, cohort studies, and outbreak reports to understand the relationship between the immune systems of children starting childcare and the pathogen milieu they encounter. We consider the interaction between

Sigmoid (S-shaped) cross-fused renal ectopia associated with calculus in a 3-year-old child: A case report and literature review.

Sigmoid (S-shaped) crossed-fused renal ectopia is a rare congenital anomaly, infrequently complicated by urolithiasis. We report a 3-year-old male child presenting with abdominal pain, diarrhea, and intermittent fever, previously diagnosed with a renal stone. Imaging revealed a 13-mm pelvic stone in the lower ectopic kidney with moderate hydronephrosis; laboratory tests were normal. The patient underwent open nephrolithotomy via a right modified Gibson incision, achieving complete stone removal through a small avascular parenchymal incision. Recovery was uneventful, and stone composition analy
