Urachal cancer: contemporary review of the pathological, surgical, and prognostic aspects of this rare disease.

Surgery is the mainstay of treatment for patients with urachal cancer (UraC). Still, one third of patients are un-resectable at presentation. Histology shows adenocarcinomas in the majority of cases with resemblance to enteric tumors. Standard chemotherapeutic regimens for bladder cancer have failed these patients. The role and efficacy of radiotherapy remains unclear due to its use in combination with chemotherapy or in a palliative setting. A lack of background knowledge about this disease leaves practitioners with unanswered questions when recommending (neo)adjuvant or palliative treatment

[Etiopathogenic factors in colorectal cancer. Genetic and clinical features (first of 2 parts)].

INTRODUCTION: Colorectal Cancer (CRC) is the second cause of death in Spain, reaching the first place in non-smoking population. It is also the tumour with the greatest one-year incidence. The last few years some reduction in its incidence along with a greater 5-year average survival has been observed. PATHOGENESIS: Most CRCs develop from benign polyps. Vogelstein's hypothesis suggests an orderly progression from normal mucosa, to a small polyp, to a large polyp and finally to a CRC. In this progression transforming cells will be increasingly charged with molecular alterations. GENETIC FACTO

Surgery for complete rectal prolapse in adults.

BACKGROUND: Complete rectal prolapse is a debilitating condition, which affects both the very young and the elderly and can cause faecal incontinence. The range of surgical methods available to correct the underlying anal sphincter or pelvic floor defects in complete rectal prolapse poses the question about the choice of the best operation. OBJECTIVES: To determine the effects of surgery on the treatment of rectal prolapse in adults. The following specific issues have been addressed: I. Whether surgical intervention is better than no treatment; II. Whether an abdominal approach to surgery is

Surgical navigation for targeted retroperitoneal lymph-node removal: a randomised, controlled, phase 3 trial.

BACKGROUND: Metastatic retroperitoneal lymph node dissection (LND) for nodal recurrence is applied for a variety of cancers, such as urological, gynaecological and rectal cancer. Precise localisation and resection of these lymph nodes (LNs) during surgery can be challenging, especially after previous radiotherapy or surgery. The objective of this study was to assess the added value of surgical navigation for targeted LND in the retroperitoneum. METHODS: We performed an open-label randomised, controlled, phase 3 trial at the Netherlands Cancer Institute, Amsterdam. Eligible participants were o

Treatment of oligometastatic disease in colorectal cancer: from molecular biology to multimodal strategies.

The management of oligometastatic colorectal cancer (OMD-CRC) has undergone a profound evolution, transforming what was once a uniformly fatal diagnosis into a clinical scenario in which long-term survival and even cure are attainable for a carefully selected subset of patients. This review provides a comprehensive synthesis of integrated, multidisciplinary strategies for OMD-CRC, with particular emphasis on hepatic and pulmonary metastases. We examine the biological foundations that differentiate oligometastatic from polymetastatic disease, including molecular subtypes that inform prognosis a

Preoperative tumor marker burden score predicts survival in colorectal cancer according to tumor size.

BACKGROUND: Reliable preoperative biomarkers are needed to improve risk stratification in colorectal cancer (CRC). The prognostic value of a combined score based on multiple serum tumor markers remains underexplored. METHODS: This retrospective study included 307 CRC patients undergoing curative surgery. A preoperative risk score was defined by the number of elevated markers (AFP, CA19-9, CA125, CA724, CA153) above institutional cut-offs. Patients were stratified into low-risk (≤ 1 elevated marker) and high-risk (> 1 elevated marker) groups. Associations with 5-year disease-free survival (DFS

Impact of Systematic Discontinuation of Mitomycin C HIPEC for Colorectal Peritoneal Metastases on Oncologic Outcomes: A Propensity Score-Matched Analysis.

BACKGROUND: The PRODIGE7 trial demonstrated a lack of effectiveness of oxaliplatin-based hyperthermic intraperitoneal chemotherapy (HIPEC) after cytoreductive surgery (CRS) for colorectal cancer peritoneal metastases (CRC-PM), prompting many centers to transition to mitomycin C (MMC)-based HIPEC. However, the oncologic benefit of MMC-HIPEC remains unclear. METHODS: In 2021, our  center systematically discontinued HIPEC for CRC-PM. We  retrospectively compared consecutive patients who underwent MMC-HIPEC after complete CRS (2009-2021) with those undergoing CRS alone (2021-2024). Outcomes inclu

An exploratory multimodal pipeline for recurrence prediction in CRC with XELOX therapy.

Colorectal cancer(CRC) recurrence remains a challenge despite curative surgery and adjuvant therapy. Integrating clinicopathological and liquid biopsy data may improve risk stratification, but evidence in small‑sample settings is lacking. This exploratory study assessed the feasibility of a multimodal machine learning pipeline for recurrence prediction in a well‑defined XELOX‑treated cohort. We analyzed 86 CRC patients(17 recurrences) receiving uniform XELOX adjuvant therapy. A three‑step pipeline was applied: (1) LASSO regression for coarse variable selection (10‑fold cross‑validation); (2) g
