Forty rejection episodes were noted in a series of 35 renal transplants. 72.5% were reversible with i.v. 1 g "boluses" of 6-methylprednisolone. Four subjects died during the post-operative period, 10 were subjected to explantation and 21 were discharged with a functioning kidney; of these, mean blood creatinine was 1.02 +/- 0.31 mg in 19. Factors other than rejection influence the results of transplantation. Gastrointestinal, metabolic and bone complications typical of protracted steroid management are examined.

Acute kidney injury (AKI) is common in children and is associated with significant morbidity and mortality. In the last decade our understanding of AKI has improved significantly, and it is now considered a systemic disorder that affects other organs including heart, lung, and brain. In spite of its limitations, serum creatinine remains the mainstay in the diagnosis of AKI. However, newer approaches such as urinary biomarkers, furosemide stress test, and clinical decision support are being increasingly used and have the potential to improve the accuracy and timeliness of AKI diagnosis.

Rhabdomyolysis is not a well-known complication in dengue fever and only a few cases are reported. We report a patient of dengue, with severe body ache and myalgia, passed reddish-brown urine with a very high creatine-phosphokinase level. He also developed oliguria, hypertension, and raised creatinine. This suggests rhabdomyolysis occurred which led to acute kidney injury. This can complicate otherwise stable patients with dengue and affect the outcome. This complication needs to be explored when a similar clinical picture is encountered.

The elevation of serum creatinine levels is a concern with telavancin therapy. We examined the onset of kidney injury associated with telavancin in an animal model. Urine samples were collected at baseline and daily to determine the concentrations of kidney injury molecule 1 (KIM-1), a marker for early kidney injury. When a clinically relevant exposure of telavancin was given daily to rats, some differences in kidney injury were attributed to the dosing regimen. Further investigations of alternative telavancin dosing regimens are warranted.

The hepatorenal syndrome is characterized by a combination of liver cirrhosis, ascites and renal impairment. It is divided into two typs, whereby typ I is defined as acute with a doubling of creatinine within 2 weeks and type II as a more modest decline of kidney function. The only definite therapy so far is transplantation. However, based on pathophysiological considerations a number of strategies have been applied. Aldosteronantagonists are the backbone of therapy even in cases of poor diuresis. Additional benefit is derived from a combination of vasopressors and albumine.

A 47-year-old man presented with subacute, low back pain and increased creatinine levels. A CT scan showed an extrinsic, bilateral compression of the ureters by a retroperitoneal mass with pronounced uptake of 18F-fluorodeoxyglucose in positron emission tomography. Pleural effusion caused by post-traumatic extravasation of urine from the abdominal cavity to the pleural cavity (urothorax) is an uncommon complication following traumatic injury. To the authors' knowledge, this is the first report of a case of traumatic urothorax in a dog presented with pleural and abdominal urine effusion. Combined urothorax and uroabdomen should be included in the differential-diagnosis list for dogs with recent trauma and a bicavitary effusion. The diagnosis can be confirmed by elevated creatinine concentrations in both effusates, compared to its serum concentration.

A 17-year-old castrated male domestic shorthair cat was diagnosed with bilateral perinephric pseudocysts (PNPs). Bilateral subtotal resection of the PNPs was performed via a laparoscopic approach. There was no regrowth or reformation of the PNPs after the procedure and the cat's serum concentration of creatinine remained stable for 6 months after surgery. Laparoscopic bilateral subtotal resection of PNPs appears to have been an effective treatment option in this cat with the advantages of being minimally invasive and requiring only a brief hospital stay.

A 38 year old patient presented with fever, myalgic abdominal pain, nose bleeding and acute renal failure since five days. A combination of thrombocytopenia, proteinuria, elevated CrP and creatinin is common in hemorrhagic fever with renal syndrome (HFRS) due to Hantavirus infection. The benigne form is called Nephropathia epidemica. Dialysis is infrequently required by patients with the Puumala virus. Other infection (e.g. malaria, leptospirosis, yellow fever) and systemic diseases (e.g. collagenosis or vasculitis) are considered.

Backache, a common symptom, is rarely caused by infection of the lumbar discs. The authors present the case of a 60-year-old man with a Staphylococcus aureus septicemia and associated lumbar discitis in whom a pauci immune crescentic glomerulonephritis and renal failure developed. Treatment with antibiotics and not immunosuppressive agents resulted in improved renal function with a discharge creatinine level of 1.87 mg/dL (165 micromol/L). This case highlights an association of pauci-immune crescentic glomerulonephritis with discitis that responded to antibiotic therapy alone.

A guinea pig model was used to study correlations between cis-platinum dosage and level of hearing loss, hair cell loss in the cochlea. Other parameters measured included weight loss and serum urea, creatinine, and cis-platinum levels. The damage to the inner ear expressed as hearing loss and hair cell loss demonstrated a highly positive correlation with the dosage of cis-platinum, and only a moderately positive correlation with the cis-platinum serum levels. Weight loss correlated well with hearing loss.

The authors studied eight cases of IgA nephropathy presenting with nephrotic syndrome. Renal biopsy revealed only mild mesangial proliferation or minor glomerular changes on light microscopic examination but typical features of IgA nephropathy on immunofluorescent and electron microscopic examination. A satisfactory response characterized by correction of hypoalbuminemia, clearance of proteinuria, and an increase of endogenous creatinine clearance occurred with corticosteroid therapy. Results on haemoglobin, PCV, serum proteins, blood urea nitrogen, creatinine, cholesterol, bilirubin and some serum enzymes in bitches with clinical pyometra were compared with the degree of polydipsia, anorexia and dehydration in the same patients, and with their plasma progesterone levels. The findings indicate that the pyometra syndrome includes a liver derangement--i.e. cholestasis--in addition to changes in bone marrow activity and renal function as shown previously. Autopsic results also indicate the presence of an endotoxic influence upon the lung function.

Polymyositis (PM) has been in the diagnostic armamentarium in medicine since the mid-nineteenth century. Medicine training puts a heavy emphasis on this diagnosis, and anyone with proximal muscle weakness with elevated serum creatinine kinase is assumed to be PM and a number of patients were rendered this diagnosis without additional testing. We report a 58-year-old Asian woman who was diagnosed with systemic lupus erythematosus (SLE) and lupus nephritis, together with a mixed pulmonary bacterial and fungal infection including Aspergillus. The infection did not respond well to the routine administration of anti-bacterial and anti-fungal drugs, and the patient's creatinine levels continued to rise and protein remained in her urine. The patient's SLE persisted without going into remission. Finally, surgical resection of the pulmonary aspergilloma brought the SLE back under control.

A young woman with mild hypercalcaemia and an inappropriately normal serum parathyroid hormone had parathyroid scintigraphy suggestive of an active ectopic parathyroid tissue in the superior mediastinum. Urinary calcium to creatinine clearance ratio was low, and a subsequent genetic analysis confirmed a novel mutation (Q164K) in the calcium sensing receptor gene, consistent with familial hypocalciuric hypercalcaemia. We propose that this mutation accounts for her clinical and investigational findings, although a double pathology of Q164K and an ectopic parathyroid adenoma is also conceivable.

This article summarizes thirty years of intensive clinical metabolic and therapeutic studies of the consequences of severe protein deficiency relative to calories, which results in kwashiorkor, and of a balanced deficiency of protein and calories that results in marasmus. Evidence is provided that these are two different metabolic diseases, but kwashiorkor is usually superimposed on some degree of chronic marasmus and hence most cases studied were marasmic kwashiorkor. The value of the creatinine/height index to indicate the degree of lean body cell mass with any disease is demonstrated.

We report the case of a 71-year-old man with creatinine clearance of 41 mL/min and acute anterior ST-segment elevation who underwent urgent coronary artery bypass grafting. A continuous intraoperative veno-arterial hemofiltration with high volumes of exchange (35 mL/kg/h) was used in a series for a miniaturized extracorporeal bypass system to minimize the inflammatory response and to protect the kidneys of this patient who had preoperative renal dysfunction. The patient had an uneventful postoperative recovery.

We report a case of a 42 year old patient who collapsed at an airport departure lounge. The patient was investigated and found to have advanced renal failure with serum potassium (K+) of 10.3 mmol/L, creatinine of 3581 pmol/L and urea of 59.9 mmol/L secondary to obstructive uropathy due to advanced bladder cancer. Although these biochemical changes were thought to be incompatible with life, he made a slow recovery following haemodialysis and insertion of bilateral percutaneous nephrostomy tubes.
