A causal role of the ventromedial prefrontal cortex in regulating anticipated regret and risk behaviors in trait anxiety.

BACKGROUND: Individuals with high trait anxiety (HA) exhibit maladaptive goal-directed behaviors, which are associated with dysfunctional counterfactual-thinking during decision-making. While lesion studies suggest the causal role of the ventromedial prefrontal cortex (vmPFC) in counterfactual-thinking, its modulatory role in anxiety-related counterfactual decision-making remains uncharacterized. Here, we bridge this gap by examining the characteristics of decision-making (forward counterfactual) and emotion responses (backward counterfactual) in trait anxiety, as well as its underlying modula

Pilot Study of a Patient Decision Aid for Valve Choices in Surgical Aortic Valve Replacement.

BACKGROUND: Objective superiority of tissue vs mechanical prostheses in surgical aortic valve replacement remains controversial, placing a greater emphasis on patients to consider personal lifestyle and risk preferences, including the burden of lifelong anticoagulation and the possible need for reoperation. A shared decision-making tool may therefore be of value in making this important choice. METHODS: A patient decision aid (PtDA) was developed using the International Patient Decision Aids Standards and used in a prospective pilot study. An intervention group received the PtDA and a survey.

Assessing infant and maternal readiness for newborn discharge.

PURPOSE OF REVIEW: The review highlights the shift from prescribed length of stay (LOS) to mother-infant dyad readiness as the basis for making discharge decisions for healthy term newborns. We describe the components of readiness that should be considered in making the decision, focusing on infant clinical readiness, and maternal and familial readiness. RECENT FINDINGS: Although the Newborns' and Mothers' Health Protection Act of 1996 aimed to protect infants and mothers by establishing a minimum LOS, the American Academy of Pediatrics 2015 policy on newborn discharge acknowledges the shift

How physicians approach prostate cancer screening before and after losing a lawsuit.

PURPOSE: In 2004, a commentary by Merenstein was published in JAMA describing how he was sued for engaging a patient in shared decision making for prostate cancer screening. The article sparked considerable debate on the impact of litigation on medical care. A natural experiment (a study assessing shared decision making under way at the practice that was sued) enabled us to evaluate whether physicians changed their prostate cancer screening behavior after the lawsuit. METHODS: As part of a randomized controlled trial conducted between January 2002 and November 2004, patients and physicians co

Patient suitability for free water protocols in acute stroke and general medicine: a qualitative study of clinician perceptions.

BACKGROUND: The free water protocol (FWP) is an alternate management strategy for patients with dysphagia, who would otherwise be nil by mouth or prescribed thickened fluids, allowing them to drink and potentially aspirate water under strict guidelines to minimize the risk of adverse consequences. The FWP is not widely implemented in acute settings, and it is unclear whether this is due to the complexity of patient presentations, clinician decision-making or barriers related to the setting. AIMS: To explore the perceptions and decision-making process of clinicians about using FWPs to manage d

Use of Computed Tomography in the Evaluation of Anterior Shoulder Instability: Possible Effect on Surgical Management.

BACKGROUND: Glenoid bone loss is a critical factor in the management of anterior shoulder instability (ASI). Computed tomography (CT) is often considered the gold standard to evaluate glenoid bone loss, but it is associated with negative factors such as radiation. Thus, interest exists as to when orthopaedic surgeons need a CT scan to guide decision-making when treating ASI. PURPOSE: To determine whether information gained from a shoulder CT scan alters orthopaedic surgeons' management plan for ASI and, secondarily, to determine whether surgeon- and patient-specific factors affect whether a C

Inverse Association Between Resting-State Putamen Activity and Iowa Gambling Task Performance in Patients With Obsessive-Compulsive Disorder and Control Subjects.

BACKGROUND: Symptoms of obsessive-compulsive disorder (OCD) have been conceptualized as manifestations of decision-making deficits. Patients with OCD exhibit impairment during the decision-making process, as assessed by the Iowa Gambling Task (IGT). This impairment is independent of clinical severity and disease progression. However, the association between the decision-making deficit and resting-state brain activity of patients with OCD has not been examined. METHODS: Fifty unmedicated patients with OCD and 55 matched control subjects completed IGT. Resting-state brain activity was examined

A bidirectional corticoamygdala circuit for the encoding and retrieval of detailed reward memories.

Adaptive reward-related decision making often requires accurate and detailed representation of potential available rewards. Environmental reward-predictive stimuli can facilitate these representations, allowing one to infer which specific rewards might be available and choose accordingly. This process relies on encoded relationships between the cues and the sensory-specific details of the rewards they predict. Here, we interrogated the function of the basolateral amygdala (BLA) and its interaction with the lateral orbitofrontal cortex (lOFC) in the ability to learn such stimulus-outcome associ

Decision Support Preferences Among Hispanic and Non-Hispanic White Older Adults With Chronic Musculoskeletal Pain.

OBJECTIVES: Despite broad recognition that social networks play a key role in the management of chronic musculoskeletal pain (CMP), little is known about when and why older adults with CMP choose to involve others in treatment decisions. This study investigates the types (i.e., informational, emotional, and instrumental) and sources (i.e., formal and informal) of support Hispanic and non-Hispanic White CMP patients desire and receive when making decisions about their pain care. METHOD: Semi-structured interviews were conducted with Hispanic and non-Hispanic White older adults with CMP (N = 63

Neuropsychological function, anxiety, depression and pain impact in fibromyalgia patients.

Cognitive deficits have a significant impact on the daily performance of fibromyalgia patients. This paper analyzes executive functioning and decision-making performance, and the relationships between these functions and pain, anxiety, depression and medication in fibromyalgia patients. A group of fibromyalgia patients (FG) (n = 85) was compared with a healthy control group (CG) (n = 85) in their performance in the Wisconsin Card Sorting Test (WCST) and the Iowa Gambling Task (IGT). In the WCST, results showed a percentage of non-perseverative errors significantly higher in the CG than in the

A qualitative study of factors influencing the decision to have an elective amputation.

BACKGROUND: Some patients with a functionally impaired lower limb choose to have an elective amputation, whereas others do not. Functional outcomes do not favor either type of treatment, making this a complex decision. The experiences of patients who have chosen elective amputation were analyzed to identify the key factors in this decision-making process. METHODS: Patients from a tertiary care amputee clinic who had chosen to undergo elective amputation of a functionally impaired lower limb participated in the present study. A qualitative research design involved the use of one-on-one semi-st

Normal performance on a simulated gambling task in treatment-naive alcohol-dependent individuals.

BACKGROUND: Research suggests that substance abusers make more disadvantageous decisions on the simulated gambling task (SGT); such decisions are associated with deviance proneness and antisocial symptoms. This study examines decision making on the SGT in young adults with alcohol dependence who are treatment-naive (TxN). METHODS: A total of 116 subjects (58 controls, 58 TxNs) were tested on the SGT, where participants choose cards from 4 different decks that vary in terms of the magnitude of the immediate gain (large/small) and the magnitude of long-term loss (larger/smaller). Human factors and airway management: focus on the Swiss Airway Guidelines.

Although decision-making in airway management is crucial for performance and outcomes, it often deteriorates in crisis situations. Studies suggest that human decision-making can be unreliable in such situations and must be supported to reduce reliance on exceptional human performance to ensure safety. Airway guidelines provide essential evidence-based information intended to maximize the likelihood of successful airway management. However, the linear, textual and inflexible design of most algorithms does not align with the naturalistic decision-making processes involved in clinical activity. T

Me, my doctor and AI: a qualitative interview study on patients' requirements for trustworthy use of AI-CDSS in ophthalmology.

OBJECTIVE: Artificial intelligence-based clinical decision support systems (AI-CDSS) are increasingly entering care and redefining how medical decisions are made. As patients are directly affected by hybrid physician-AI decisions without interacting with the system itself, trust, transparency and involvement in decision-making become important concerns. This study explored patients' requirements for trustworthy use of AI-CDSS in ophthalmology, with particular attention to expectations regarding disclosure of AI use and informational needs. METHODS AND ANALYSIS: We conducted semi-structured in

Effect of the Head Computed Tomography Choice Decision Aid in Parents of Children With Minor Head Trauma: A Cluster Randomized Trial.

IMPORTANCE: The Pediatric Emergency Care Applied Research Network prediction rules for minor head trauma identify children at very low, intermediate, and high risk of clinically important traumatic brain injuries (ciTBIs) and recommend no computed tomography (CT) for those at very low risk. However, the prediction rules provide little guidance in the choice of home observation or CT in children at intermediate risk for ciTBI. OBJECTIVE: To compare a decision aid with usual care in parents of children at intermediate risk for ciTBI. DESIGN, SETTINGS, AND PARTICIPANTS: This cluster randomized

Chronic alcohol disrupts premotor cortex intrinsic excitability and transmission.

Alcohol dependence induced alterations to decision-making are hypothesized to occur in part through long-lasting changes to cortical function. Animal models of chronic alcohol exposure have been used to identify alcohol-related changes to the activity and intrinsic function of cortical neurons, but the extent of such changes across involved cortical circuits is unclear. The secondary or premotor cortex (M2), which is necessary for goal-directed decision-making, shows aberrant in vivo activity during decision-making following chronic alcohol exposure, and targeting M2 activity can restore appro

"We wish we had the option": a qualitative study of women's perspectives and experiences with contraception in a provincial prison in Ontario, Canada.

BACKGROUND: Evidence suggests that women who are incarcerated desire access to contraception while incarcerated, and that this need is not currently being met. Our objective in this study was to explore the perspectives and experiences of women in prisons regarding contraception and contraception access using data from focus groups with women in a provincial prison. We analyzed focus group data collected in a provincial prison in Ontario, Canada using content analysis and a constructivist epistemology. RESULTS: We conducted three focus groups, each approximately one hour in length. Discussion
