Auditory brainstem responses (ABR) in children with neurological disorders.

Auditory brainstem responses (ABR) were studied in 35 children with neurological disorders and 24 controls. Abnormal results were obtained in 16 patients. All 5 of the patients with metachromatic leukodystrophy had evidence of peripheral and/or central delay in transmission in keeping with evidence of demyelination of both peripheral (i.e. auditory nerve) and central (i.e. brainstem) pathways as occurs in this disorder. Two children with lead poisoning had delayed conduction in the peripheral pathways only and in these there was good correlation between the degree of delay and the ulnar nerve

Neonatal adrenoleukodystrophy.

Nine cases of neonatal adrenoleukodystrophy are described. All patients had abnormal facial features, moderate to severe hypotonia, hepatomegaly, and retinitis pigmentosa. The clinical course was rapidly progressive in six cases and more protracted in three others. Biological signs of adrenal insufficiency were present in five cases. CT scan showed a demyelinating process in four patients. Trilamellar inclusions were found in the liver of four cases and dark and complex lipidic inclusions in three other cases. In the three necropsied patients there was severe alteration of the white matter inv

Spinal ganglia and peripheral nerves from a patent with Tay-Sachs disease. Morphological and ganglioside studies.

We examined the spinal ganglia and peripheral nerves from a patient with Tay-Sachs disease, comparing the other nervous tissues morphologically and lipid-biochemically. The spinal ganglia and peripheral nerves showed numerous membranous cytoplasmic inclusion bodies (MCBs), which are characteristic of GM2-ganglioside storage in the neuronal cell bodies of the patient brains. In spinal ganglia, all neurons and satellite cells around the neurons contained membrane-bound lipid materials. In peripheral nerves, Schwann cells and myelinated axons except for enlarged axons were devoid of MCBs. Major g

Triple Genetic Diagnosis in a Patient with Late-Onset Leukodystrophy and Mild Intellectual Disability.

We describe the complex case of a 44-year-old man with polycystic kidney disease, mild cognitive impairment, and tremors in the upper limbs. Brain MRI showed lesions compatible with leukodystrophy. The diagnostic process, which included clinical exome sequencing (CES) and chromosomal microarray analysis (CMA), revealed a triple diagnosis: autosomal dominant polycystic kidney disease (ADPKD) due to a pathogenic variant, c.2152C>T-p.(Gln718Ter), in the PKD1 gene; late-onset phenylketonuria due to the presence of two missense variants, c.842C>T-p.(Pro281Leu) and c.143T>C-p.(Leu48Ser) in the PAH g

Narrowing critical regions and determining penetrance for selected 18q- phenotypes.

One of our primary goals is to help families who have a child with an 18q deletion anticipate medical issues in order to optimize their child's medical care. To this end we have narrowed the critical regions for four phenotypic features and determined the penetrance for each of those phenotypes when the critical region for that feature is hemizygous. We completed molecular analysis using oligo-array CGH and clinical assessments on 151 individuals with deletions of 18q and made genotype-phenotype correlations defining or narrowing critical regions. These nested regions, all within 18q22.3 to q2

Hypothesis on the pathogenesis of vacuolar myelopathy, dementia, and peripheral neuropathy in AIDS.

Certain aspects of the clinical syndrome of dementia, cerebral atrophy, predominantly sensory neuropathy, and vacuolar myelopathy in AIDS resemble those seen in vitamin B12 deficiency. Pathologically, there are similarities not only in the changes in the spinal cord, but also in the brain and peripheral nerves. The pathogenesis of vacuolar myelopathy may be secondary to a combination of immune mediated myelin and oligodendrocyte injury, and simultaneous impairment of repair mechanisms due to a deficiency of S-adenosylmethionine (SAM). Products derived from macrophages may interfere directly wi

The firing probability of single motor units following transcranial magnetic stimulation in healthy subjects and patients with neurological disease.

Magnetic stimuli applied to the scalp can cause single motor units (MUs) to discharge in intrinsic hand or more proximal arm muscles. MUs are much more likely to be fired by stimuli if they are being activated voluntarily at the same time. The changes in firing probability induced by magnetic stimuli in such tonically active MUs have been studied in healthy subjects and patients using peri-stimulus time histograms (PeSTHs). The mean firing level in the 250 msec before the stimulus, was used to define 2 peaks of increased firing after the stimulus. The first, termed the primary peak (PP), was a

Guillain-Barré syndrome temporally associated with respiratory syncytial virus vaccination.

An older woman developed acute progressive limb weakness associated with pain, paraesthesia and falls 2 weeks after receiving a respiratory syncytial virus (RSV) vaccine. Initial examination and imaging suggested alternative diagnoses, including cervical spondylotic disease and cerebrovascular pathology and early neurophysiology was non-diagnostic. During admission, she deteriorated with urinary retention, areflexia and type 2 respiratory failure requiring intubation and ventilation and was commenced on intravenous immunoglobulin (IVIG). Cerebrospinal fluid analysis later demonstrated albumin-

A Computational Framework for Myelin as a Regulator of Temporal Integration, Neuronal Excitability, and Synaptic Plasticity.

Myelin regulates neural signaling not only by enabling fast conduction but also by dynamically tuning axonal conduction velocity (CV) and thereby shaping spike timing. While activity-dependent myelination is thought to optimize neural communication, how changes in myelin translate into altered single-cell dynamics and plasticity remains unclear. Using a computational modeling approach, we show that disruptions of axonal CV reorganize presynaptic input correlations, leading to systematic changes in postsynaptic excitability, excitation-inhibition balance, and spike-timing-dependent plasticity.

Cranial Nerve Involvement With Diplopia as Presenting Feature of CMT1H Caused by Recurring FBLN5 Variant.

BACKGROUND AND AIMS: CMT1H is a rare, autosomal dominant, demyelinating subtype of CMT caused by variants in FBLN5. Symptomatic cranial nerve involvement has never been reported in patients with CMT1H. CASE REPORT: We report a 45-year-old woman with a history of long-standing diplopia. On examination, she had bilateral limitation of eye abduction, with double vision in all directions of gaze, particularly horizontally. She was unable to heel-walk, had mild lower limb distal weakness, decreased tendon reflexes with absent Achilles reflexes, reduced distal vibration sense, pes cavus, and hammer

Remyelination and neuroprotection translational trials: Lessons from optic neuritis.

Multiple sclerosis disease-modifying treatments have reduced inflammatory activity but have limited impact on progressive neurodegeneration. In this context, remyelination and neuroprotection appear as key therapeutic goals. The visual pathway offers an attractive translational model, as optic neuritis recapitulates the demyelinating and neurodegenerative features of multiple sclerosis while providing accessible functional and structural readouts through visual evoked potentials and optical coherence tomography. Over the past two decades, several clinical trials have evaluated remyelinating an

Sphingosine-1-Phosphate Is a Key Signaling Molecule in Normal Conditions and in Multiple Sclerosis.

Sphingosine-1-phosphate (S1P) is one of the most extensively studied bioactive signaling molecules of sphingolipid metabolism, which plays a pivotal role in regulating numerous processes in the central nervous system and immune system. Acting as an extracellular ligand for five subtypes of G-protein-coupled receptors (S1PR1-S1PR5) as well as an intracellular metabolic mediator, S1P controls lymphocyte migration, blood-brain barrier permeability, survival and differentiation of oligodendrocytes, reactivity of astrocytes and microglia, and balance between inflammation, neurodegeneration, and neu

Inflammatory CADASIL: the cross-link between CADASIL and multiple sclerosis: a report of two cases and systematic review of the literature.

BACKGROUND: Multiple Sclerosis (MS) is sometimes misdiagnosed in people with cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL). We aimed to discuss two CADASIL patients who were misdiagnosed with MS and review the literature for similar cases. METHODS: In addition to reporting our two cases, we searched the literature and all available databases for CADASIL cases with clinical or radiological features suggestive of MS. RESULTS: We reported two CADASIL cases misdiagnosed as MS. The review identified 12 reports describing 19 cases with CADASIL

Management of MOG antibody-associated disease by pembrolizumab and radiation myelopathy in an elderly patient: A case report and literature review.

Immunotherapy and radiotherapy are important means for treating solid tumors. Both treatment methods can potentially cause neurologic injuries. Timely identification, diagnosis, and early treatment play a crucial role in the recovery from neurologic damage. This case report describes a 69-year-old woman with esophageal and oropharyngeal cancer who developed neurological complications after radiotherapy and immunotherapy. She experienced radiation-related motor impairment and symptoms returned after treatment with bevacizumab and corticosteroids. Later, she developed a rare demyelinating neurop

A cellular assay to determine the fusion capacity of MFN2 variants linked to Charcot-Marie-Tooth disease of type 2 A.

Charcot-Marie-Tooth Disease (CMT) is an inherited peripheral neuropathy with two main forms: demyelinating CMT1 and axonal CMT2. The most frequent subtype of CMT2 (CMT2A) is linked to mutations of MFN2, encoding a ubiquitously expressed GTP-binding protein anchored to the mitochondrial outer membrane and essential for mitochondrial fusion. The use of Next-Generation Sequencing has led to the identification of increasing numbers of MFN2 variants, yet many of them remain of unknown significance, depriving patients of a clear diagnosis. In this work, we establish a cellular assay allowing to asse
