ERCP in the diagnosis and management of complications after surgery for hepatic echinococcosis.

BACKGROUND: Surgery for liver hydatidosis can result in serious morbidity and mortality. The role and efficacy of ERCP in the management of these complications was reviewed. METHODS: Retrospective analysis of 79 patients with liver hydatidosis who underwent ERCP for postoperative complications. RESULTS: All patients with biliary fistulas (n = 50) were managed by endoscopic sphincterotomy, although 9 (18%) also required a biliary endoprosthesis. Surgical reintervention was avoided in 46 (92%) patients with subsequent surgery required in only in 4 (8%). The mean time to fistula closure was 13.

Distribution of hydatid cysts into the liver with reference to cystobiliary communications and cavity-related complications.

BACKGROUND: This study was designed to test a hypothesis that intrahepatic hydatid cyst location can effect the incidence of cystobiliary communications and the cavity-related complications. METHODS: A total of 121 cysts treated by conservative surgical methods in 113 patients were evaluated prospectively. Cysts were grouped as near to the liver hilum (segment I, III, IVb, V, and VI) and far from the hilum (segment II, IVa, VII, and VIII). RESULTS: There were 58 (48%) hilar and 63 (52%) peripheral cysts. We found more cystobiliary communications (48% versus 27%, P = 0.015), more biliary leak

Sonographic diagnosis of hydatidosis: the sign of the cyst wall.

The Authors evaluate the diagnostic accuracy of echography in hepatic echinococcosis, particularly in the completely liquid types of cysts without septa or hydatid sand and in the solidified forms where the differential diagnosis with other hepatic pathologies can be difficult. The study was carried out with a 7.5 MHz probe and the results were compared with those from the use of a 3.5 MHz probe and from CT, serology for hydatidosis and echoguided sampling. Seventy one patients, 36 with liquid hepatic lesions and 35 with solid hepatic lesions (31 hydatid and 40 non-parasitic), were evaluated e

Immunohistochemical observations on cellular response in unilocular hydatid lesions and lymph nodes of cattle.

As we believe the immunohistochemistry of the hydatid lesions and draining lymph nodes has never been studied, we collected them from the liver and lungs of cattle in Uruguay for such a study. Frozen sections of the tissues were immunohistochemically stained using monoclonal antibodies against surface markers CD2, CD4, CD5, CD8, B cell and granulocyte-monocyte/macrophage and antiserum against specific granules of bovine eosinophils. The adventitial layer of the cyst wall consists of a layer of epithelioid cells and connective tissue. The cells from the epithelioid cell layer were a kind of mac

Hydatid cyst of liver.

Hydatid disease of the liver is still endemic in certain parts of the world. The diagnosis of non-complicated hydatid cyst of the liver depends on clinical suspicion. Ultrasonography and computed tomography, the most important diagnostic tools, are helpful for determining the complications and planning treatment. The modern treatment of hydatid cyst of the liver varies from surgical intervention to percutaneous drainage or medical therapy. Surgery is still the treatment of choice and can be performed by the conventional or laparoscopic approach. Percutaneous drainage and treatment of the cyst

[Surgical treatment of hepatic hydatidosis. Analysis of results of conservative and radical surgery in our experience].

The authors review a series of 194 cases of hepatic hydatidosis submitted to surgery over the period from July 1973 to December 2001, comparing the results of conservative and radical operatins. In this period we performed 164 conservative operations (140 indirect marsupialisations, 5 delaminations with capitonnage of the residual cavity and 19 partial pericystectomies), 26 total pericystectomies and 4 liver resections. The main outcome measures analysed were postoperative hospital stay, mortality, morbidity and recurrence rates. The median postoperative hospital stay was 17.5 days for conserv

Management of hepatobiliary cystadenocarcinoma.

Primary biliary cystadenocarcinomas are rare malignant neoplasms of the liver. We report a case that was operated with a preoperative diagnosis of hydatid cyst of the liver. The patient underwent a wide surgical excision of the cyst. Histopathological examination revealed a hepatobiliary cystadenocarcinoma. The patient remains asymptomatic at 2 years following surgery. The importance of differentiating these rare tumors from hepatic hydatid cysts, especially in areas where the disease is endemic, is highlighted in this case report.

[Rationale for removal of fibrous capsule in echinococcectomy from the liver].

Experience of radical surgical treatment in 253 patients with hepatic echinococcosis is presented. Twenty-six patients underwent hepatic resection; 218, pericystectomy, 9 patients, combined interventions. Based on comparative analysis of the results of surgical treatment in 109 patients from the study and control groups, it is concluded that radical operations have significant advantages and show fever postoperative complications and recurrences. Removal of fibrous capsule is a token of successful treatment for hepatic echinococcosis.

[Diagnostic and therapeutic approach in hepatic echinococcosis complicated by mechanical jaundice].

The article presents the results of the treatment of 43 patients with echinococcosis of the liver the course of which was complicated by mechanical jaundice (MJ). In 11 patients MJ arose as a result of cystic content break into the bile ducts, in 32 patients, due to cystic compression of the lobular bile ducts. An effective scheme of the patients' examination is proposed which involved present-day ultrasound, X-ray and endoscopic methods. The system of objective evaluation of the patients' condition is outlined as designed for planning surgical policy. In non-severe disease the patients are op

A new instrument, the "Perfore-Aspirator" for laparoscopic treatment of hydatid cysts of the liver.

The authors introduce a new instrument functioning as both perforator and aspirator in the laparoscopic management of hepatic hydatid cysts. Between January 1998 and January 2002, 11 laparoscopic cystotomy + partial cystectomy + drainage procedures were performed for eight consecutive patients. Eight of the cysts were located in the right lobe, and the remaining three in the left. The average diameter (+/-SD) of the cysts was 9.6 +/- 3.66 cm, and the mean age of the patients was 31.3 +/- 7.24 years. The diagnosis was confirmed by ultrasonography and/or computerized tomography. The procedure wa

Transthoracic versus transabdominal surgical approach for echinococcal cysts located over the superoposterior aspect of the right lobe of the liver.

A retrospective study of 80 patients operated on for hydatid cysts located on the superoposterior aspect of the right lobe of the liver (segments VI, VII, VIII) is presented. Right thoracotomy was performed in 30 patients, and 50 patients were operated on through bilateral subcostal incisions. The two approaches were compared in terms of radicality and morbidity. The transabdominal approach produced superior results and fewer postoperative complications, resulting in a shorter hospital stay (11 +/- 5 vs. 18 +/- 8 days). Total cystopericystectomy was feasible in 30% of patients operated on tran

Hydatid disease in a very unusual location: the adrenal gland. A case report.

Hydatid cysts are most often located in the liver and lungs (55-90%), while their location in the adrenal gland is very rare (0.5%). We observed a patient with a hydatid cyst in the adrenal gland, the diagnosis of which was incidental during ultrasonography and computed tomography. The experience acquired in Sicily where hydatid disease is endemic makes serological tests unnecessary, also because they often require a lengthy waiting period and are expensive. The patient had undergone surgery for the treatment of hepatic hydatid cysts. The adrenal localization may be explained as a consequence

A retrospective study comparing the different surgical procedures for the treatment of hydatid disease of the liver.

BACKGROUND/AIMS: The purpose of this study was to compare the security and effectiveness of the different surgical procedures used for treatment of the liver infestations with larvae of the dog tapeworm (Echinoocccus granulosus). METHODS: 55 patients with echinococcal cysts in the liver were treated surgically at our department between 1987 and 1999. The patients were grouped according to the mode of surgical technique and comprised 16 anatomical liver resections, 26 pericystectomies and 13 endocystectomies. The results of the different procedures have been analyzed statistically. RESULTS: T

Risk for laparoscopic fenestration of liver cysts.

BACKGROUND: Laparoscopic fenestration is considered the best treatment for symptomatic simple liver cysts. Conversely, the laparoscopic approach for the management of hydatid simple liver cysts is not widely accepted because of the risk for severe complications. Despite improvement in imaging techniques, the probability of preoperatively mistaking a hydatid liver cyst for a simple liver cyst remains about 5%. Therefore, laparoscopic fenestration, planned for a liver cyst could be performed unintentionally for an undiagnosed hydatid liver cyst. METHODS: From January 2000 to January 2001, 15 pa

Long term results after complete or incomplete surgical resection of liver hydatid disease.

BACKGROUND: The liver is the organ most frequently infected by hydatid disease and medical therapy alone is ineffective in eliminating the parasite. Surgical options vary from complete resection (e.g. total pericystectomy or hepatectomy) to limited procedures (e.g. percutaneous aspiration or unroofing of cysts). The aim of this study was to determine the long-term outcome after complete or partial resection of liver hydatid cysts. PATIENTS AND RESULTS: Between 1980 and 1996, 78 patients were operated upon at our institution for liver hydatid cysts. In Group 1, complete resection was achieved

[Echinococcosis - a rare differential diagnosis of a tumorous hepatic lesion].

We report the case of a 44-year-old woman who was admitted to our hospital because of a newly developed painless jaundice. Though she felt quite well sonographic and radiographic evaluation showed a tumor of the liver. An extensive diagnostic workup was performed but it was not until laparotomy that a malignant tumor could definitely be ruled out and the presumptive diagnosis of an echinococcosis was proven. A partial resection of the liver was performed.
