Organisation and integrated healthcare approaches for people living with HIV, multimorbidity, or both: a systematic review.

BACKGROUND: Universal recommendation for antiretroviral drugs and their effectiveness has put forward the challenge of assuring a chronic and continued care approach to PLHIV (People Living with HIV), pressured by aging and multimorbidity. Integrated approaches are emerging which are more responsive to that reality. Studying those approaches, and their relation to the what of delivery arrangements and the how of implementation processes, may support future strategies to attain more effective organizational responses. METHODS: We reviewed empirical studies on either HIV, multimorbidity, or bot

[Plagiarism and duplicate publications in the medical literature: scope of the problem, risks, warning signs, and prevention strategies].

Plagiarism and duplicate publications pose a persistent threat to the integrity and credibility of the medical literature. Despite the widespread adoption of ethical rules, international guidelines, and detection software, these practices remain prevalent in medical research, and their scope continues to expand. Their consequences extend beyond damaging the reputation of researchers and journals. They can bias the scientific evidence base, distort the outcomes of systematic reviews and meta-analyses, artificially inflate researchers' academic achievements, waste editorial and peer-review resou

Occupational Therapy Referral Pathways for Upper Limb Conditions in Tygerberg, South Africa: Stakeholder Perspectives.

Effective referral pathways to occupational therapy are essential for continuity of care and recovery following upper limb injury or condition. Although challenges within South Africa's public healthcare referral systems are recognised, patients' experiences remain underexplored. This study explored patients' experiences of occupational therapy referral pathways within the Tygerberg substructure and identified factors influencing access to care. A qualitative descriptive design was used, with semistructured interviews conducted with seven purposively sampled participants. Data were analysed th

Cost-effective procurement and prescribing of continuous glucose monitors in the NHS.

As the global incidence of diabetes and its associated treatment costs rise, continuous glucose monitors (CGMs) constitute a valuable tool for patient self-monitoring. However, market dominance of a few well-known brands, predominantly in populations with type 1 diabetes, limits the adoption of more cost-effective alternatives that might offer improved clinical outcomes. To address this, an expert roundtable evaluated barriers to efficient CGM use, identifying challenges such as regional care board variations, lack of patient-centred prescribing and insufficient training for both patients and

Sleep and Cardiometabolic Health: Integration Into Internal Medicine Practice.

Sleep is a critical and often underestimated determinant of cardiometabolic health, influencing neuroendocrine, inflammatory, and metabolic pathways. Sleep disorders are associated with an increased risk of cardiovascular and metabolic diseases, including hypertension, type 2 diabetes, and obesity. This article aims to review the main pathophysiological mechanisms linking sleep to cardiometabolic disease, summarize the most recent clinical evidence, and propose intervention strategies applicable to Internal Medicine practice. We conducted a narrative review of literature published between 2015

Predictive factors for remission in post-stroke depression: A Markov model cohort study.

Post-stroke depression (PSD) follows a dynamic remission-relapse course, yet predictors of state transitions remain unclear. Here, we applied a continuous-time multi-state Markov model to 536 ischemic stroke patients with 1,441 longitudinal assessments. We quantified transition intensities between non-PSD, mild-PSD, and severe-PSD states and identified baseline predictors of deterioration and recovery. The transition intensity from mild to non-PSD was 5.5-fold higher than progression to severe-PSD. Education ≥10 years reduced deterioration risk (HR = 0.63), while functional independence (Barth

Developing bacterial resistance to antibiotics: a laboratory activity.

Bacterial resistance to antibiotics continues to be a major threat to human health. Agencies, such as the World Health Organization, have called for a multistep response, including increased educational training, both for professionals and the public at large, on this complex problem. Toward that end, we created a laboratory experience, ideally suited for undergraduates, where students observe the development of bacterial resistance over the course of a week. The procedure is conducted in a single container for simplicity and reliably generates resistant strains. Eightfold increases in resista

Issues impeding satisfactory outcomes with the competency-based medical education program for undergraduates, and some possible solutions.

The announcement of the competency-based medical education (CBME) curriculum for undergraduates by the National Medical Commission (NMC) in India in 2019, and its consequent implementation by all medical colleges has, by and large, been unsatisfactory because of many difficulties experienced by colleges in its implementation. The program has over 2800 listed competencies for the students to be trained in, over 800 listed skills, and over 100 skills for mandatory certification before the summative examination. The greatest reason for failure, however, is no major change in the assessment proces

Advances in End-of-Life Care in Canada: Implications for Oncology Nursing.

This paper explores recent advancements in end-of-life (EOL) care in Canada, focusing on palliative care (PC) in oncology, advance care planning (ACP), and medical assistance in dying (MAiD). Despite improvements in cancer treatment, cancer remains a leading cause of death in Canada, with patients facing significant physical, psychosocial, and emotional challenges throughout the illness trajectory. Over the past few decades, PC has evolved to address serious illness from diagnosis onward, enhancing symptom management, quality of life, and patient satisfaction, while reducing hospital admission

From exposure to embedding: how interconnected social determinants drive health inequalities-a complex adaptive system dynamics approach.

The social determinants of health (SDOH)-including socioeconomic position, education, housing, and social networks-are fundamental drivers of health inequalities worldwide. These inequalities persist because social determinants interact dynamically across time and context, with health emerging from the interplay of social, political, environmental, and biological processes in which determinants act simultaneously as causes and consequences. Early-life exposures and historical conditions produce enduring physiological vulnerability through biological embedding, encompassing stress dysregulation

Beyond the standard: targeted reapplication in palmoplantar scabies of infants.

Infantile scabies can be challenging to treat because of behavioural and anatomical factors that reduce the efficacy of topical therapies. In this small prospective cohort study, there were 22 neonates and infants with persistent palmoplantar scabies. Patients were treated exclusively with permethrin 5% cream. All patients initially failed the standard regimen consisting of one full-body application, followed by a second -application after 7 days. In 1 group, 11 infants received an intensified regimen with 3 additional nights of targeted application to the hands and feet (intervention group),

Perioperative prevention of euglycaemic diabetic ketoacidosis in people living with type 2 diabetes established on sodium-glucose transport-2 inhibitors - a cross-site multi-cycle audit.

Sodium-glucose transport-2 (SGLT2) inhibitors are commonly prescribed for the management of type 2 diabetes mellitus, chronic kidney disease and heart failure. However, their continuation in the perioperative setting in people with diabetes can precipitate euglycaemic diabetic ketoacidosis (EDKA), a potentially life-threatening complication. This multi-cycle audit evaluated adherence to perioperative guidelines regarding preoperative cessation and postoperative re-initiation of SGLT2 inhibitors. Electronic health records of consecutive surgical patients with type 2 diabetes were reviewed over
