Sacral neuromodulation: an effective treatment for lower urinary tract symptoms in multiple sclerosis.

INTRODUCTION AND HYPOTHESIS: Most subjects with multiple sclerosis (MS) suffer from lower urinary tract symptoms (LUTS). Detrusor overactivity, detrusor hypocontractility and detrusor-sphincter dyssynergia are the most common bladder dysfunctions. Management is not straightforward due to the progressive course of the disease. Sacral neuromodulation (SNM) has received increasing attention among new effective treatments for bladder disorders associated with MS. The aim of this study was to review the published literature on the role of SNM in the treatment of LUTS in patients with MS. METHODS:

[Comparative study of eyelid closure induced by electric exciting different sites on odoiculads oculi muscle].

OBJECTIVE: Directly excited the denervated orbicularis oculi muscle (OOM) by electric current on rabbits, to induce efficient eyelid closure, and seek the optimal sites for such excitation that can produce efficient eyelid closure with the minimal excitating current in the least channel. METHODS: Bilateral peripheral facial paralysis model on 20 healthy NewZealand rabbits (40 sides) were prepared. Exciting current was designed for two-way rectangular pulse, 35 Hz frequency and 0.2ms pulse width. The current intensity could be adjusted between 0 and 2.5 mA. Middle of upper-orbit (A), outer orb

Transcranial cerebellar direct current stimulation (tcDCS): motor control, cognition, learning and emotions.

The neurological manifestations of cerebellar diseases range from motor to cognitive or behavioral abnormalities. Experimental data in healthy subjects extend the cerebellar role to learning, emotional and mood control. The need for a non-invasive tool to influence cerebellar function in normal and pathological conditions led researchers to develop transcranial cerebellar direct current stimulation (tcDCS). tcDCS, like tDCS, depends on the principle that weak direct currents delivered at around 2mA for minutes over the cerebellum through surface electrodes induce prolonged changes in cerebella

Status of sacral neuromodulation for refractory constipation.

AIM: This review article aims to provide a brief update on the current data on and position of sacral neuromodulation (SNM) in the specialized management of refractory idiopathic constipation. METHOD: Published evidence from PubMed and our own unpublished data on SNM treatment for refractory idiopathic constipation were used for this evaluation. RESULTS: Seven studies were found in PubMed that covered this topic. The main focus was on the most recently published multicentre nonrandomized European trial. Summary data from our unpublished study on constipation in children are also included. C

Electrotherapeutic disputes: the 'Frankfurt Council' of 1891.

Since the 1980s and 1990s, vagus nerve and deep brain stimulation, transcranial magnetic stimulation and cranial electrotherapy stimulation have found their way into neurology as therapeutic approaches to epilepsy, Morbus Parkinson and other central nervous symptoms. Moreover, these methods have proven useful and provided hope in the therapy of other diseases, most of all in psychiatry. From a historic perspective, this new emphasis on somatic therapies in the case of transcranial magnetic stimulation and cranial electrotherapy stimulation represents the return of therapeutic methods widely us

Novel nonpharmacologic perspectives for the treatment of task-specific focal hand dystonia.

NARRATIVE REVIEW: The pathophysiology of focal hand dystonia (FHD) has not yet been completely clarified. Although there is a loss of inhibition at multiple levels of the central nervous system, maladaptive plasticity of the cerebral cortex as well as impairments in sensory and motor representations have also been reported. All of these abnormalities can be viewed as an epiphenomenon of the primary--still unknown--abnormality underlying focal dystonia. The purpose of this review is to describe the underlying constructs of novel nonpharmacologic approaches for the treatment of FHD. Alternative

Vagus nerve stimulation for epilepsy and depression.

Many patients with epilepsy suffer from persistent seizures despite maximal antiepileptic drug (AED) therapy. Chronic, intermittent vagus nerve stimulation (VNS) has proven to be a safe, effective option for patients suffering from refractory seizures who are not candidates for surgical resection. Although only a small minority of patients will be entirely seizure-free, VNS as an adjunct to medical therapy does appear to provide a significant amount of improvement in quality of life. Reports of antidepressant effects independent of seizure control, along with the use of multiple AEDs in the tr

Simultaneous intracochlear stimulation based on channel interaction compensation: analysis and first results.

A simultaneous paradigm for electric stimulation of the acoustic nerve based on a monopolar electrode configuration and sign-correlated pulses is presented. Simultaneous pulse amplitudes are determined by taking into account parameters of spatial channel interaction. The computation of simultaneous amplitudes requires the solution of linear systems of equations in an iterative procedure. The computation amount can be reduced significantly, if the spatial impulse responses in individual electrodes can be approximated by two exponentially decaying branches with decay constants alpha toward apex

Anatomical and physiological basis and mechanism of action of neurostimulation for epilepsy.

Neurostimulation is an emerging treatment for neurological diseases. Different types of neurostimulation exist mainly depending of the part of the nervous system that is being affected and the way this stimulation is being administered. Vagus nerve stimulation (VNS) is a neurophysiological treatment for patients with medically or surgically refractory epilepsy. Over 30,000 patients have been treated with VNS. No clear predictive factors for responders have been identified. To date, the precise mechanism of action remains to be elucidated. Better insight in the mechanism of action may identify

The elecrophysiological bulbocavernosus reflex test in female dogs: Its technique and applicability.

The intent of the study was to clarify the technique and the application of the electrophysiological bulbocavernosus reflex (EBCR) test in healthy female dogs. For this aim, 15 healthy female dogs were used in the study. The stimulations were made on the clitoris and the responses were recorded from the right side of the external anal sphincter muscle with a concentric needle recording electrode. The EBCR had response latencies between 18.99 and 25.69ms with the mean value of 22.26ms. The EBCR reflex test is not yet widely used for the evaluation of the functional integrity of sacral spinal co

Future directions in pelvic neuromodulation.

In view of the success of sacral nerve stimulation, new lines of research are enabling us to do more selective stimulation of the innervation of the bladder at more peripheral sites, such as pudendal nerve stimulation, and with the use of less invasive devices, such as Bion and other in situ implantable devices. The aim of this research is to determine ifa less invasive, more selective stimulation or an event-driven (detrusor overactivity ora desire for micturition) stimulus would allow for a more improved and durable response.

VNS Therapy versus the latest antiepileptic drug.

Pro AED: The central issue in medical decision-making is risk-benefit assessment. Surgery of any type is still considered to be a major undertaking. To warrant these risks, the patient has a right to expect that they have a greater chance of a good outcome with an invasive therapy than with a non-invasive one. The main question is when, if ever, this becomes the case when comparing implantation of a VNS Therapy System versus adding an antiepileptic drug (AED)? After the first drug? The second? After all AEDs have failed? To date, no randomized trial comparing the addition of an AED against vag
