[Bladder sphincter disorders in Parkinson's disease].

Parkinson's disease related to degeneration of the extrapyramidal structures is characterized in its typical form by the classic triad of tremors, rigidity and akinesia, constituting Parkinson's syndrome. These are combined with neurovegetative disorders, responsible for sexual and bladder sphincter dysfunction. The Latter occurs in from 30% to 90% of cases, depending on the stage of progression of the disease. The dopamine deficiency in the nigrostriataL tract leads to a lifting of the inhibition which is probably the cause of the bladder hyperreflexia. Irritative signs are most frequently ob

The role of ultrasonographic measurements of the median nerve in the diagnosis of carpal tunnel syndrome.

AIM: The aim of study was to assess the usefulness of ultrasonographic measurements of the median nerve in the diagnosis of carpal tunnel syndrome. MATERIALS AND METHODS: Eighty-six patients with carpal tunnel syndrome confirmed by electromyography and 45 asymptomatic controls were included in the study and underwent high-resolution ultrasonography of the wrists. The cross-sectional area and flattening ratio at the level of the pisiform bone of the proximal carpal tunnel were measured. Data from the patient group and control group were compared to determine the statistical significance. The a

Anorectal incontinence: aetiology, pathophysiology and evaluation.

Faecal incontinence is a complex problem, often of multifactorial origin. Although the condition is widely accepted as a problem in the elderly, it is now becoming apparent that much younger age groups are frequently affected. Its exact incidence is about 2% of the general population, while in other individuals, the prevalence has been reported to approach 60%. Despite the considerable advances that have been made the past decades in the evaluation of anorectal incontinence, our understanding of it remains limited. A thorough history, good physical examination, and detailed anorectal physiolog

Involuntary expiratory phonation as a dose-related consequence of L-dopa therapy in a patient with Parkinson's disease.

We report a case of involuntary phonation caused by abnormal vocal cord movements during expiration in a patient with Parkinson's disease. A 60-year-old woman had been treated for parkinsonism at the outpatient clinic of the Department of Neurology since August 1999. She began to groan involuntarily in the daytime in September 2001. She could not eat well while groaning. Stridor was not noted during sleep at night. Endoscopic examination of the larynx revealed insufficient abduction of the bilateral vocal cords, although the glottis was not so small as to cause stridor during inspiration. Duri

A supination splint worn distal to the elbow: a radiographic, electromyographic, and retrospective report.

The purposes of this study were to 1) describe a dynamic supination splint that does not cross the humeroulnar and humeroradial joints, allowing flexion and extension of the elbow; 2) provide retrospective data on effectiveness of this splint in patients with limited supination; and 3) provide data from radiography and electromyography (EMG) that document this splint's ability to provide a passive supination force. Eleven subjects treated for various elbow and/or wrist fractures leading to losses of forearm supination significantly increased their passive range of motion (PROM) from 34.0 degre

Shoulder muscle reflex latencies under various levels of muscle contraction.

Previous research in relaxed muscles shows that muscle reflex latencies are too slow to protect the shoulder. However, during athletic activity when injury occurs, some level of shoulder muscle contraction typically exists. The purpose of the current study was to assess shoulder muscle reflex latencies under various levels of muscle contraction. Seventeen healthy subjects participated. A perturbation consisting of an external rotation collision force to the anterior forearm in a position of apprehension under various levels of muscle contraction (0%, 20%, and 50% of a maximum voluntary muscle

[Swallowing disorders in Parkinson's disease].

Impairment of swallowing is a common symptom in advanced stage of Parkinson's disease and severe defect of this function may cause aspiration pneumonia, problems with food intake and cachexy. The aim of this study was to assess the reflex and oral, pharyngeal, oesophageal phase of swallowing. Eleven patients with Parkinson's disease and 9 healthy subjects were investigated by electromyography (EMG) and oesophageal scintigraphy. The study demonstrates delayed triggering of swallowing reflex (543 +/- 84 ms in patients with PD vs. 230 +/- 66 ms in controls, p < 0.05) and prolongation of laryngeal

[Spontaneous electromyographic activity. Practical importance].

Spontaneous activities are a major semeiologic sign in electromyography. The present article deals with the different aspects recorded in practice in normal and pathological cases. There are two types of spontaneous activities, those related to motor unit hyperactivity (fasciculations and myokymia) and those related to the hyperactivity of one or more muscle fibers: fibrillations, positive sharp waves, myotonic discharges and complex repetitive discharges. In the first case the lesion is located in the axone and in the second in the membrane of the muscle fibers; All theories related to the ce

Pathophysiological significance of fasciculations in the early diagnosis of ALS.

In amyotrophic lateral sclerosis (ALS), muscles with preserved strength can have fasciculation potentials (FPs) as the only abnormality. The FPs in strong muscles are predominantly simple and stable, and can often be recruited by a slight voluntary contraction. In weak and atrophic muscles, the FPs are generally complex and unstable, tend to have a slower firing rate and are not recruited by voluntary contraction. Macro-electromyography studies suggest that these FPs are part of a more complex motor unit, as opposed to simple FPs which can represent all the motor unit. FPs driven by transcrani

Alternative treatment of the carpal tunnel syndrome.

Carpal tunnel syndrome (CTS) is a painful disease of the wrist which is usually treated with corticoids locally or surgically. In this study combined infusion therapy with procain, pentoxyphyllin and magnesium sulphuricum in patients with CTS was evaluated retrospectively. 101 manually working patients (65 male, 36 female) aged 42.8 +/- 8.6 years, initially examined at our department to assess professional origin of the disease and who had abnormal conduction to the distal section of median nerve on electromyography (EMG), were entrolled. The group of patients was heterogenous etiologically. 4

Antroduodenal manometry findings in patients with slow-transit constipation.

BACKGROUND: The results of subtotal colectomy for slow-transit constipation are unpredictable. Abdominal pain, distension, and bloating often persist after operation. To ascertain whether patients with slow-transit constipation may have a generalized intestinal motor disorder, we studied the antroduodenal motor activity in 20 consecutive patients with slow-transit constipation. METHODS: All patients underwent symptom registration, whole-gut transit time, anorectal manometry, electromyography of the anal sphincter, the balloon expulsion test, and defecography to characterize their constipation
