Damage control strategy involving early strategic decision-making and staged surgical management in a hybrid emergency room system: A case report.

RATIONALE: The hybrid emergency room system (HERS) has gained recognition for enabling rapid diagnosis and intervention in trauma care. However, it remains unclear whether its benefits stem solely from the availability of integrated technology or from facilitating early strategic decision-making by the trauma team. PATIENT CONCERNS: A 58-year-old woman was involved in a motor vehicle collision and sustained severe injuries, including a diaphragmatic rupture, multiple left rib fractures, and thoracolumbar vertebral fractures. Despite initial resuscitation at a referring hospital, she remained

Ischiopubic synchondrosis osteomyelitis: a rare presentation of a limping child presenting to a paediatric emergency department.

We present a case of a middle childhood boy who presented to our paediatric emergency department with severe right upper thigh pain, radiating to inguinal/hip area. The pain was sharp in nature, described like a severe cramp which started 10 hours prior to presentation, progressively increasing over time. He denied any recent trauma or infective symptoms, and there were no other associated complaints. On physical examination, he was in tears due to excruciating pain which he could not locate precisely, had tense muscles at the right posterior upper thigh and lower gluteal area and had limping.

Text message incentives increased patient-reported outcomes survey response in emergency care: SWAT findings.

OBJECTIVES: To determine if text message-based behavioral interventions could increase response rates to a patient-reported outcomes survey in the emergency department (ED). STUDY DESIGN AND SETTING: We conducted a study within a trial (SWAT), within the NUDG-ED trial. The NUDG-ED trial aimed to reduce low-value care for patients with back pain presenting to eight EDs in Sydney, Australia. This SWAT was a 3-arm randomized controlled trial (RCT) nested within the NUDG-ED trial. After discharge from the ED, patients were randomized to receive one of the three text message invitations to complet

The epidemiology of osteopathic diagnoses and treatments in United States emergency departments from 2018 to 2021.

CONTEXT: Although a large proportion of US emergency physicians are osteopathic physicians (Doctor of Osteopathic Medicine [DOs]), the frequency of osteopathic manipulative medicine (OMM) use in emergency departments (EDs) is unknown. OBJECTIVES: We aimed to estimate the frequency of OMM in EDs across the US and describe the cohort who received OMM. METHODS: We performed a retrospective cohort study of ED patients from 2018 to 2021 in the Nationwide Emergency Department Sample (NEDS), Healthcare Cost and Utilization Project (HCUP), Agency for Healthcare Research and Quality. Encounters with

Using virtual reality to enhance procedure-related experiences in adult emergency departments: A qualitative study.

INTRODUCTION: Needle phobia is a common issue affecting many patients in emergency departments (EDs), including adults. Virtual reality (VR) has rapidly advanced in recent years and is increasingly used in patient care. However, there is limited research exploring the subjective experiences of VR users in this context. AIM: This study aims to explore the experiences of both patients and clinicians during painful needle-related procedures, as well as identify the barriers and facilitators associated with the use of VR to manage these procedures in ED. DESIGN: An exploratory qualitative study.

Diagnostic yield of an abbreviated MRI protocol in the evaluation of dizziness in the emergency department, a single institutional experience.

PURPOSE: MRI is the preferred imaging modality for patients with acute dizziness when a central etiology is possible. Abbreviated protocols may improve access in urgent settings. This study assesses the diagnostic yield and utility of an abbreviated MRI protocol for patients presenting with dizziness to the emergency department (ED). METHOD: This retrospective study included 613 adult patients presenting to the ED with dizziness from August 1, 2019 to August 31, 2023. The protocol included 3 mm coronal and axial DWI, axial FLAIR, and SWI sequences, with a duration of approximately 11 min. MRI

Elevated Blood Pressure Among Military Emergency Department Patients.

INTRODUCTION: Hypertension is a leading modifiable risk factor for atherosclerotic cardiovascular disease (ASCVD) resulting in a significant burden on public health. Although the relationship between elevated blood pressure (BP) and long-term ASCVD risk is well-established in older adults, data for younger populations, such as military service members and athletes, remain limited. To assess early detection of hypertension, this study describes the prevalence of elevated BP in emergency departments (EDs) and its relationship with subsequent diagnoses of hypertension. MATERIALS AND METHODS: A r

Sex disparities in chlamydia and gonorrhea treatment in U.S. adult emergency departments: A systematic review and meta-analysis.

BACKGROUND: Empiric antibiotic treatment for gonorrhea (GC) and chlamydia (CT) is common in U.S. emergency departments (EDs) due to the delayed availability of test results. Local studies have identified sex-based disparities in sexually transmitted infection (STI) care, with females potentially receiving less empiric treatment than males. This study evaluates how STI treatment aligns with laboratory-confirmed results to quantify potential overtreatment and undertreatment rates and investigates sex differences for these outcomes. METHODS: We conducted a preregistered (PROSPERO #241429) system

Gonorrhea and Chlamydia Screening for Adolescents and Young Adults in Emergency Departments.

IMPORTANCE: Adolescents account for almost half of the 2.5 million diagnosed sexually transmitted infections in the US annually, and the emergency department functions as the primary source of health care for many adolescents. No recommendations exist for emergency department gonorrhea and chlamydia screening. OBJECTIVE: To compare usual care against targeted and universally offered gonorrhea and chlamydia screening approaches. DESIGN, SETTING, AND PARTICIPANTS: This was a comparative effectiveness, multicenter, pragmatic trial with a type 3 hybrid stepped-wedge crossover design implemented

Universal opt-out syphilis screening in a UK emergency department.

OBJECTIVES: The incidence of syphilis in several areas of England has risen substantially. National programmes for blood-borne viruses (BBV) screening in emergency departments (ED) have been ongoing for several years however have not included syphilis. We hypothesised that screening for syphilis in an ED in a region of high prevalence might identify significant numbers of cases. METHODS: Universal opt-out syphilis screening was included with BBVs in a Northeast England ED, where a regional outbreak is ongoing. Those with reactive tests were invited to sexual health service (SHS) for further e

Derivation of the Difficult Airway Physiological Score (DAPS) in adults undergoing endotracheal intubation in the emergency department.

BACKGROUND: Prediction of serious outcomes among patients with physiological instability is crucial in airway management. In this study, we aim to develop a score to predict serious outcomes following intubation in critically ill adults with physiological instability by using clinical and laboratory parameters collected prior to intubation. METHOD: This single-center analytical cross-sectional study was conducted in the Emergency Department from 2016 to 2020. The airway score was derived using the transparent reporting of a multivariable prediction model for individual prognosis or diagnosis

Management of common pediatric ear complaints in the emergency department.

The clinical presentation for ear complaints can often overlap with other common pediatric conditions. As such, the differential diagnosis for ear complaints is broad, warranting a thorough history and targeted physical examination. Optimizing the approach for a definitive ear examination is essential when evaluating a pediatric patient with an ear concern. A systematic approach will improve the accuracy of diagnoses and the appropriateness of management. This issue provides an evidence-based approach to the diagnosis, workup, and management of common ear complaints in pediatric patients.
