Rapid/point-of-care syphilis testing in high-income countries: insights and implementation challenges from the authors' experience in Canada.

Rapid/point-of-care tests (RPOCTs) for syphilis can significantly contribute to addressing the ongoing syphilis epidemic in high-income countries (HICs) for individuals facing barriers to traditional healthcare access and who may be less likely to return for follow-up appointments. By providing immediate diagnosis and treatment, RPOCT helps reduce disease progression and transmission to sex partners and from pregnant women to their fetus. This review offers an overview of the resurgence of syphilis in HICs and discusses the advantages and limitations of existing RPOCTs. We also share our exper

Overview of Colistin Resistance in the Middle East and North Africa (MENA) Region.

Colistin (polymyxin E), a last-resort antibiotic for the treatment of multidrug-resistant Gram-negative bacterial infections, is becoming increasingly compromised by the rapid emergence and dissemination of resistance. The extensive use of colistin in agriculture and veterinary medicine has accelerated the spread of plasmid-mediated mobile colistin resistance (mcr) genes, facilitating transmission across human, animal, and environmental reservoirs. This review provides a comprehensive overview of the epidemiology, molecular mechanisms, and geographical distribution of colistin resistance in th

Invasive candidiasis in the intensive care unit: diagnostic stewardship, source control, and precision antifungal therapy.

INTRODUCTION: Invasive candidiasis remains one of the most challenging infections in the intensive care unit (ICU) because diagnosis is complicated by nonspecific clinical manifestations, frequent colonization, altered host immunity, complex anatomical sources and unstable pharmacokinetics. Distinguishing true invasive disease from colonization is fundamental to optimizing patient outcomes while avoiding unnecessary antifungal exposure. AREAS COVERED: This narrative review examines invasive candidiasis in critically ill adults from an ICU perspective, based on a structured PubMed/MEDLINE, Emb

Epidemiology and Clinical Impact of Pediatric Viral Gastroenteritis Before and After Rotavirus Vaccination in Sicily.

Acute gastroenteritis (AGE) remains a leading cause of pediatric mortality and morbidity, with rotavirus as the leading cause of severe disease. Post-vaccine surveillance is essential to monitor circulating pathogens and assess vaccination impact. Sicily was the first Italian region to implement universal rotavirus vaccination in 2012. We retrospectively studied 693 children hospitalized for suspected viral AGE at the Children's Hospital of Palermo (March 2017-February 2020), testing stool samples for viral and bacterial enteric pathogens. Rotavirus remained the most common agent (13.3%), foll

Antimicrobial resistance and genomic characteristics of avian Pasteurella multocida.

BACKGROUND: Pasteurella multocida, the cause of fowl cholera, poses a significant economic threat to poultry. The escalation of antimicrobial resistance (AMR) in this pathogen, driven by drug misuse, is a serious concern. Crucially, a comprehensive understanding of AMR patterns, molecular epidemiology, and virulence in Chinese avian strains is still lacking. This study aimed to elucidate the phenotypic and genomic characteristics of P. multocida strains isolated from avian in Zhejiang Province, China. RESULT: Twenty avian-origin P. multocida strains isolated in Zhejiang Province were subjecte

Prognostic Factors and Management Approaches for Small Nonfunctional Pancreatic Neuroendocrine Tumors: Insights from SEER Data.

OBJECTIVE: To explore the optimal management strategies for small-sized nonfunctional pancreatic neuroendocrine tumors (NF-PanNETs) and to assess the rationale for the 'wait and see' approach. METHODS: This retrospective study analyzed data from 2,052 patients with small-sized NF-PanNETs (tumor size ≤20 mm) diagnosed between 2004 and 2019. Patients were further divided into two groups: those with tumors ≤10 mm (n = 573) and those with tumors 11-20 mm (n = 1,479). Demographic information, clinical characteristics, and survival outcomes were obtained and compared using the Surveillance, Epidemi

Incidence, Disease Spectrum, and Outcomes of Tuberculous Meningitis in South African Children: The Initial Impact of COVID-19.

Tuberculous meningitis (TBM) is a very severe form of childhood tuberculosis (TB), requiring hospitalisation for diagnosis. We investigated trends in admission, disease spectrum, outcomes, and healthcare system factors in children with TBM managed at a tertiary referral hospital in Cape Town, South Africa. We conducted a retrospective cohort study of children (<13 years) with TBM admitted from 2017 to 2021. An innovative surveillance algorithm was used to identify all possible TBM episodes using integrated electronic health data. Episodes were clinically verified and data were extracted using

Recommendations for antibacterial prophylaxis in children receiving chemotherapy: a joint initiative of SITIP and infectious disease group of AIEOP.

BACKGROUND: Current guidelines for managing infections in pediatric patients with cancer do not recommend routine antibiotic prophylaxis (AP). However, several aspects of AP, including the role of diagnosis, the impact of neutropenia duration, screening for resistant bacterial colonization, and antibiotic stewardship, remain a matter of debate. METHODS: To address these issues, a panel of experts from the Italian Association of Pediatric Hematology and Oncology (AIEOP) and the Italian Society of Pediatric Infectious Diseases (SITIP) conducted a Delphi consensus. An African swine fever vaccine-like variant with multiple gene deletions caused reproductive failure in a Vietnamese breeding herd.

African swine fever (ASF), an economically damaging disease in domestic pigs, has emerged in Vietnam since 2019. Vietnam is the only country granted licenses for developing and commercializing modified live-attenuated vaccines (LAVs) against the highly pathogenic ASF virus (ASFV). The nationwide implementation of LAVs in Vietnam for prophylaxis has likely influenced the viral genetic pool among the swine population. This study highlighted the incursion of a novel ASF vaccine-like variant into a non-vaccinated breeding herd. Retrospective epidemiology suggested a high replacement rate and impro

Pathogenesis and Possible Consequences of Port-Site Metastasis in Gynecologic Oncology After Laparoscopic Procedures.

Port-site metastasis (PSM) is an infrequent and serious complication of laparoscopic treatment. The incidence of PSM is 0.16-2.3% of patients undergoing laparoscopic surgery for gynecologic malignancies. Depending on the place of origin of the malignant tumor, PSM appears on average the earliest after ovarian cancer treatment and the latest after vaginal cancer surgery. To date, several important risk factors have been identified. Hypotheses such as wound contamination, hematogenous and lymphatic spread, immune response and CO2 pneumoperitoneum have been formulated as possible agents influenci

Neonates at Risk: Understanding the Impact of High-Risk Pregnancies on Neonatal Health.

High-risk pregnancies (HRPs) constitute a significant global health issue due to their strong association with increased maternal and neonatal morbidity and mortality. Although pregnancy is generally characterized by positive expectations, the presence of maternal comorbidities, gestational complications, or adverse socioeconomic and environmental conditions can markedly elevate the probability of unfavorable outcomes. HRPs contribute disproportionately to complications such as preterm birth, fetal growth restriction, low birth weight, and congenital anomalies, which are key determinants of ne
