Experience with implanted electrodes.

Implant units, manufactured by Medtronic, Inc., have been implanted in 31 patients for treatment of drop-foot. The principal problem encountered in this series was inflammatory reaction. This was observed in seven patients, and was serious enough in five to require removal of the implant. On the positive side, there have been no failures of the implanted equipment for periods of up to 7.3 years, although there is currently a question on one patient. There has also been no documented damage to the nerve from chronic stimulation. Fine wire elctrodes have been implanted intraneurally in a series

Antibody-Drug Conjugate Manufacturing: Facility Design, Process Safety, and Regulatory Considerations.

This paper presents an in-depth examination of the manufacturing, formulation, and fill/finish processes for traditional antibody-drug conjugates (ADCs), with particular emphasis on cytotoxic ADCs and the unique risks associated with their production. ADCs inherent toxicity demands rigorous facility and equipment design, proactive risk assessment, and robust risk mitigation strategies. We describe the multiple processes and facility types involved in producing ADCs, with a more detailed focus on drug product facilities. In addition, we study the critical facility risk areas encountered and eff

A retrospective analysis of reasons for reoperation following initially successful peripheral nerve stimulation.

OBJECT: The authors investigated the causes for surgical reexploration in patients with complex regional pain syndrome Type II who received initial relief of pain from implantation of a peripheral nerve stimulator (PNS). METHODS: The authors reviewed the charts of 11 consecutive patients who underwent a total of 27 PNS-related operations at one institution. Duration of follow up ranged from 5 days to more than 24 months. Of 11 patients who received PNS implants, seven (64%) required one or more additional surgeries to relocate the PNS because initial pain relief following stimulation was lost

Long-term, tunneled, noncuffed central venous catheter in cancer patients (Vygon): safety, efficacy, and complications.

BACKGROUND: Totally implantable or partially cuffed central venous catheters (CVC) are commonly used in cancer patients, but they are often expensive and may produce complications. To minimize costs, we have been using a low-cost, partially tunneled, silicone elastomer catheter with no Dacron cuff or antireflux valve (Vygon) since 2001. This study is a retrospective investigation of our experience using the Vygon catheter as a long-term CVC in patients with malignancy. MATERIALS AND METHODS: A total of 458 Vygon catheters (Nutricath, Vygon) were percutaneously inserted by an interventional ra

A Virtual Reality Game to Change Sun Protection Behavior and Prevent Cancer: User-Centered Design Approach.

BACKGROUND: Public health sun safety campaigns introduced during the 1980s have successfully reduced skin cancer rates in Australia. Despite this success, high rates of sunburn continue to be reported by youth and young adults. As such, new strategies to reinforce sun protection approaches in this demographic are needed. OBJECTIVE: This study aims to develop a virtual reality (VR) game containing preventive skin cancer messaging and to assess the safety and satisfaction of the design based on end user feedback. METHODS: Using a two-phase design approach, we created a prototype VR game that i

Aberration-reduced spherical concave grating holographically recorded by a spherical wave and a toroidal wave for Rowland circle mounting.

We propose a new recording system that employs a spherical wave and a toroidal wave to holographically record an aberration-reduced Rowland-circle spherical grating. Two more variables in this recording system are available for the design compared with a conventional symmetric dual-plane-wave recording system, which allows both the astigmatism and sagittal coma of the grating to be corrected to minimum. We derive the aberration coefficients of such grating and give the universal design principles for the recording system. An aberration-reduced grating used in a spectrometer equipped with linea

An alternative linear impactor for impact research.

This paper introduces an affordable and easily replicable linear impactor designed for impact research. Traditional commercial systems are often expensive and complex, limiting their accessibility to many research and development teams. Our hardware addresses this by providing a reliable platform for conducting controlled impact experiments. The system uses a spring-driven ram to strike an object equipped with instrumentation for measuring impact responses. The design allows for customization and integration with various test objects and setups, making it adaptable for evaluating different imp

Design of a Patient-Specific 3D-Printed Splint for Surgical Correction of Pectus Excavatum in a Kitten.

Pectus excavatum is a rare congenital deformation of the chest wall. Associated clinical signs depend on the severity of the deformity of the sternum and costochondral cartilages. After thoracic radiographic studies, a patient-specific plastic external splint was designed based on a previously made plaster cast of the chest of the kitten. The splint was designed using 3D scan equipment and data processing to obtain the computer-aided model that was then used to manufacture the splint using 3D printing technology. The surgical application of the customized 3D-printed external splint for the cor

Engineering cells for solid tumor therapy.

Living cell therapy and nonliving drugs are both key approaches for treating solid tumors. Engineering living cells with nonliving drugs harnesses their complementary advantages to enhance therapeutic outcomes. However, inappropriate engineering methods or design strategies can compromise the synergy between cells and drugs. In this review, we provide a decision framework that matches engineering strategies to cell types and drug properties, yielding baseline-qualified engineered cells. To address barriers specific to solid tumors, we then discuss strategies for equipping cells with additional

Effect of safety harness design on the pressures exerted on the user's body in the state of its suspension.

The presented studies concern the pressure of safety harnesses on the user's body in a state of its suspension. An anthropomorphic dummy was used for simulation of human behaviour in suspension. The test objects included four models of harnesses of different designs, equipped with attachment elements placed at the back and front of the human body. Pressure mapping sensors of 56 × 56 mm2 surface area and two-dimensional (2D) pressure mapping apparatus were used for the measurements. The parameters characterizing the pressures of the harness belts on the dummy surface were defined. The obtained

A novel approach to an anatomical adapted stent design for the percutaneous therapy of tricuspid valve diseases: preliminary experiences from an engineering point of view.

Tricuspid valve regurgitation mostly occurs as result of dilation of the right ventricle, secondary to left heart valve diseases. Until recently, little attention has been given to the development of percutaneous therapeutic tools exclusively designed for tricuspid valve disease. A new approach to the interventional therapy of tricuspid regurgitation, in particular, the design of a conceptual new valve-bearing, self-expansible stent, is presented here. A three-dimensional computer model of a right porcine heart was developed to gain a realistic anatomical geometry. The new design consists of t

Systematic review of 3D-printed surgical devices for austere environments: an evaluation guideline.

BACKGROUND: Three-dimensional (3D) printing is a common manufacturing technique. It is being adopted to produce essential surgical equipment in austere settings around the world. However, the evaluation of these products remains inconsistent. This raises safety concerns and limits translation to clinical use. To address this, this systematic review aims to identify 3D-printed surgical devices intended for austere environments and examine their evaluation methodologies. The findings will inform the development of an evaluation framework that supports the design of patient-safe devices. METHODS

What influences stroke survivors with physical disabilities to be physically active? A qualitative study informed by the Theoretical Domains Framework.

BACKGROUND: Although the benefits of regular physical activity (PA) after stroke are well established, many stroke survivors do not achieve recommended PA levels. To date, studies exploring determinants to PA have not used a behaviour change theory and focused on stroke survivors with physical disabilities. As a precursor to an intervention development study, we aimed to use the Theoretical Domains Framework (TDF) to identify factors influencing PA in stroke survivors with physical disabilities in Singapore. METHODS: Between November 2021 and January 2022, we conducted interviews with 19 comm

Injuries and Use of Safety Equipment in River Surfing.

BACKGROUND: River surfing (also called "rapid surfing") involves surfing on stationary waves that are artificially created or placed in rivers and is gaining popularity, especially among surfers in landlocked areas but also among athletes without experience in ocean surfing. Different wave setups, types of boards, and types of fins, as well as the use of safety equipment, can lead to overuse and injuries. PURPOSE: To analyze the incidence, mechanisms, and risk factors of river surfing-related injuries for different types of waves and to evaluate the usage and appropriateness of safety equipme

Development and verification of a protocol to quantify hip joint kinematics: an evaluation of ice hockey goaltender pads on hip motion.

BACKGROUND: The butterfly save technique is commonly used by ice hockey goaltenders and has recently been identified as a potential mechanism for hip joint injuries due to the extreme body positions involved. Unfortunately, commonly used kinematic marker sets that determine these body positions are heavily influenced by skin motion artifact and are obscured by protective equipment, making it difficult to obtain reliable measures of hip motion. PURPOSE: To create a new kinematic protocol that could be used when protective equipment prevents typical marker placements and to use this protocol to

A "modified crossover technique" for vascular access management in high-risk patients undergoing transfemoral transcatheter aortic valve implantation.

OBJECTIVES: To describe results from our "modified crossover technique" for vascular access management during transcatheter aortic valve implantation (TAVI). BACKGROUND: Vascular access management remains a major cause of complications following TAVI due to the large bore sheaths required. METHODS: All suitable patients undergoing TAVI in our center, between June and August 2011, underwent our "modified crossover technique," which enables the passage of a balloon through left radial access and inflation in the proximal iliac to allow percutaneous closure in a clean field. RESULTS: In total,
