Toward patient-centered blood pressure monitoring: first evaluation of a low-pressure and rapid oscillometric method for reducing cuff-related measurement burden.

Out-of-office blood pressure (BP) monitoring is essential for hypertension management. Repeated cuff inflation may cause discomfort and reduce acceptance of out-of-office BP monitoring. To address this, we developed a novel low-pressure and rapid oscillometric method designed to reduce maximum cuff pressure (MaxCP) and measurement duration. In this participant-blinded crossover study, we evaluated cuff-related measurement burden using the Omron HEM-7290T, the first automated upper-arm BP monitor to use the novel method. The HEM-7290T was compared with an otherwise identical device using a conv

Premature atherosclerotic cardiovascular disease in the MENA region: economic impact, barriers to care, and policy recommendations for an escalating crisis.

PURPOSE OF THE REVIEW: Premature atherosclerotic cardiovascular disease (ASCVD) is increasingly recognized as a major public health challenge in the Middle East and North Africa (MENA) region, where cardiovascular events occur at younger ages than in Western populations. Despite ongoing policy reforms and improvements in healthcare infrastructure, premature ASCVD remains underrecognized and insufficiently addressed. This review summarizes the epidemiology, economic burden, policy initiatives, and current barriers related to premature ASCVD in MENA, while proposing targeted interventions to sup

Clinical, Biochemical, and Coronary Angiographic Profiling of Symptomatic Premature Coronary Artery Disease in a Gujarati Population: A Cross-Sectional Study.

Background With the prevalence of coronary artery diseases (CAD) on the rise, especially in the younger population, characterization of non-conventional risk factors remains essential, especially in the inherently predisposed Southeast Asian population. This study aimed at clinical and biochemical profiling in angiographically proven CAD in young Gujarati Indians without conventional risk factors such as tobacco/alcohol consumption. Methodology This single-center, descriptive, cross-sectional case series included consecutive Gujarati patients aged ≤45 years presenting with symptomatic, angiogr

Severe fat embolism syndrome following minor trauma in a child with Duchenne muscular dystrophy: A case report.

Background Fat embolism syndrome (FES) is a rare but potentially life-threatening complication classically associated with long-bone fractures and orthopedic procedures. Patients with Duchenne muscular dystrophy (DMD) are at increased risk because of severe osteoporosis, chronic corticosteroid therapy, loss of ambulation, and fatty bone marrow replacement. However, FES remains underrecognized in this population, particularly when triggered by minor trauma without radiographic evidence of fracture. Case Presentation We report the case of a 12-year-old boy with DMD receiving long-term corticoste

Experimental models of cerebral small vessel disease: Physiological constraints, translational challenges and future directions.

Cerebral small vessel disease (cSVD) is a chronic, progressive cerebrovascular disorder and the second most common cause of dementia after Alzheimer's disease. It accounts for approximately 20% of strokes, including a quarter of ischaemic strokes and nearly half of vascular dementias, representing a growing clinical and socio-economic burden in ageing populations. Despite its prevalence, mechanistic understanding remains limited and disease-modifying therapies are lacking. A major obstacle is the difficulty of interrogating disease progression in vivo, as the small calibre and deep location of

Suboptimal Selenium Intake Produces Sex-Specific Alterations in Metabolic Profiles in Western Diet-Fed Obese Mice.

Selenium (Se) is an essential micronutrient required for redox regulation and metabolic homeostasis. Altered biomarkers of Se status have been linked with obesity and metabolic syndrome, yet its role in these conditions, particularly in a sex-specific context, is not well defined. This study investigated the impact of suboptimal Se intake on metabolic risk profiles in male and female mice with pre-existing diet-induced obesity. C57BL/6N mice were fed either a standard diet with adequate Se (SD-ASe), a Western diet with adequate Se (WD-ASe), or WD-ASe followed by a Western diet containing subop

Intermittent fasting and cardiometabolic responses under cardiac stress: a systematic review of preclinical studies.

Intermittent fasting (IF) has emerged as a dietary strategy with benefits that extend beyond weight management and glycemic control. Although growing evidence supports its favorable effects on cardiometabolic health, the impact of IF under conditions of cardiac stress remains poorly characterized. Since clinical trials cannot ethically expose patients to acute cardiac stress during IF protocols, preclinical models are essential to elucidate its potential cardioprotective mechanisms. This systematic review aimed to synthesize the preclinical evidence on the effects of IF in models of cardiac st

Providing holistic care for patients with metabolic dysfunction-associated steatotic liver disease/metabolic dysfunction-associated steatohepatitis: Key aspects of clinical assessment and how to develop individualised care plans for surveillance and interventions.

Metabolic dysfunction-associated steatotic liver disease (MASLD), characterised by hepatic steatosis and metabolic dysfunction (i.e., obesity, type 2 diabetes, dyslipidaemia, and hypertension), is affecting over 30% of the adult population worldwide. It can progress to metabolic dysfunction-associated steatohepatitis (MASH) and fibrosis, cirrhosis and even hepatocellular carcinoma. However, most patients with non-cirrhotic MASLD die from extrahepatic causes, particularly cardiovascular disease and non-hepatic cancers, underscoring the need for a holistic approach to surveillance and treatment.

AI-driven saliency-guided retinal vessel segmentation framework for sustainable digital pathology.

INTRODUCTION: Accurate segmentation of retinal blood vessels is essential for the early diagnosis of ophthalmic and systemic diseases such as diabetes, hypertension, and cardiovascular disorders. However, challenges such as low contrast, complex vessel geometry, and the presence of pathological artifacts often degrade segmentation performance, particularly for thin vessels and boundary regions. METHODS: To address these challenges, this study proposes an AI-driven saliency-guided boundary refinement framework (SGB-Net). The model integrates a progressive boundary refinement (BR) module to enh
