The coming of age of self-mutilation.

Self-mutilation (SM), the deliberate, nonsuicidal destruction of one's own body tissue, occurs in such culturally sanctioned practices as tattooing; body piercing; and healing, spiritual, and order-preserving rituals. As a symptom, it has typically been regarded as a manifestation of borderline behavior and misidentified as a suicide attempt. It has begun to attract mainstream media attention, and many more who suffer from it are expected to seek treatment. This review suggests that SM can best be understood as a morbid self-help effort providing rapid but temporary relief from feelings of dep

Human heart rate variability relation is unchanged during motion sickness.

In a study of 18 human subjects, we applied a new technique, estimation of the transfer function between instantaneous lung volume (ILV) and instantaneous heart rate (HR), to assess autonomic activity during motion sickness. Two control recordings of ILV and electrocardiogram (ECG) were made prior to the development of motion sickness. During the first, subjects were seated motionless, and during the second they were seated rotating sinusoidally about an earth vertical axis. Subjects then wore prism goggles that reverse the left-right visual field and performed manual tasks until they develope

[Severe eye involvement in pemphigus vulgaris].

Pemphigus vulgaris (PV) is an immune-mediated vesiculobullous disease of the skin and mucous membranes. Generally, patients with PV present first with oral lesions, which may precede the cutaneous lesions, such as bullae and erosions, by several months. An ocular manifestation is unusual. The most common ophthalmologic involvement in pemphigus is conjunctivitis, but without progressive scarring such as occurs in ocular cictricial pemphigoid. Corneal involvement is very rare. CASE REPORT. We report on severe ocular involvement in a 56-year-old male Turkish patient with PV. Ophthalmologic findin

Clinical features, investigations and pathology in surgically treated thyrotoxicosis.

A retrospective study was conducted on 41 thyrotoxic patients operated from January 1986 to June 1992 in Alexandra Hospital. One had multinodular goitre, and the rest had autoimmune thyroid diseases. Analysis of the histopathological features of the 40 patients with autoimmune thyroid diseases revealed that 32 had Graves' disease, four had Hashimoto's thyroiditis and four had a mixed form of Graves' disease and Hashimoto's thyroiditis. Only 14 out of 32 patients had eye and/or cutaneous manifestation of Graves' disease. None of the patients with Graves' disease developed hypothyroidism over a

Quadruple therapy for ocular toxoplasmosis.

Between Dec. 1, 1973, and May 30, 1989, 36 patients (37 eyes) with ocular toxoplasmosis seen at a uveitis clinic received quadruple therapy (pyrimethamine, trisulfapyrimidines, clindamycin and prednisone). The criteria for quadruple therapy were active lesions involving or threatening the macula or the optic disc, or a visual acuity of 20/70 or worse due to vitreous opacification caused by active inflammation. All but four of the cases showed improved vision. A total of 54% of the cases responded favourably within 2 weeks after the start of treatment, and 81% responded within 3 weeks. Patients

Biological studies of dysthymia.

Dysthymic disorder (DD) is a chronic subsyndromal depressive condition that has generated increasing interest since its formal introduction into the psychiatric nomenclature in 1980. Although DD was included among the affective disorders in DSM-III, this classification was controversial. Some clinical and family studies support an association between DD and major depression disorder (MDD), but there has been little additional research firmly establishing the diagnostic validity of DD or clarifying its relation to MDD and to personality disorders. In this article, the literature on the biology

[Clinical aspects of osteoporosis].

The development of osteoporosis is a multifactorial process. Part of contributing factors are not to be influenced, others may be avoided or prevented. The achievement of an optimal peak bone mass as well as its maintenance require sufficient sex hormones in connection with optimal calcium supply with the nourishment and sufficient physical exercise. During the preclinical phase of osteoporosis only the analysis of risk factors is possible. There are no clinical symptoms of the condition itself. Manifestation of osteoporosis is documented by crush fractures or discontinuing fractures. In osteo

Clinical manifestations of iridocyclitis in leprosy.

Iridocyclitis--an inflammation of the iris and ciliary body occurs in lepromatous spectrum of leprosy of long duration. It is usually bilateral, has a very chronic course with acute exacerbations which are suppressed with the help of corticosteroids and other antiinflammatory drugs--Mydriatic to keep the Pupil dilated and also an antibiotic to take care of secondary infection which may be present. It is a major cause of blindness, due to its secondary damaging effects on the various ocular tissues. Medical and Paramedical Workers attending these patients must be made aware of the chronicity of

Necrotizing scleritis. A clinico-pathologic study of 41 cases.

Scleritis is not a single clinical or pathologic entity. It has several distinct forms. A clinico-pathologic study of 41 cases of necrotizing scleritis suggests that these different histopathologic forms of disease may reflect different mechanisms of immunopathogenesis. These cases were divided into three main groups: (1) scleral inflammations associated with various systemic autoimmune diseases, including 11 cases of rheumatoid arthritis, three cases of Wegener's granulomatosis, one case of polychondritis, and one case of Goodpasture's syndrome; (2) infectious scleritis, consisting of four ca

Takayasu's truncoarteritis. Pulseless disease or aortitis syndrome.

The etiology of Takayasu's truncoarteritis is still unknown, although recent studies have suggested the participation of immunogenetic factors in the pathogenesis of this disease. Based on the summary of autopsy findings and taking clinical features into consideration, the current criteria for the pathological diagnosis of this condition might be summarized as follows. 1) Non-syphilitic truncoarteritis frequently occurring in young females. 2) Frequent manifestation of pulseless syndrome, and occasional presence of specific eye-ground findings or hypertension. 3) Common involvement of cranial

[Ocular localization appearing in a child with acute lymphoblastosis after hematological remission for 18 months].

Case report of a 4-year-old girl in whom a unilateral ocular localization appeared after 1 1/2 year of maintained remission of acute lymphoblastic leukaemia. The evolution of this localization can be divided in three phases: 1. During the first 17 months, an apparent simple relapsing conjunctivitis of the left eye. 2. During the following 5 months, repeated episodes of the left iritis with mydriasis and discolouration of the iris. 3. Finally, left uveitis involving the anterior chamber, with hypopion, hyphaema and glaucoma; the clinical presentation at this stage was suggestive of ocular lymph

Asymmetric Tear Secretion: Can This Disorder Help in Suspecting Gastroesophageal Reflux Disease and in Managing Sjögren's Disease? A Pilot Study.

Background and Objectives: Patients with Sjögren's disease (SjD) do not experience any improvement in gastroesophageal reflux disease (GERD) symptoms after SjD treatment, and in some patients, reflux even worsens. It is important to note that GERD manifests itself through typical and atypical symptoms, the latter of which may include eye damage, as evidenced by a growing body of research. When SjD patients were prescribed medication to treat GERD, their condition improved at the same time. Therefore, we aim to investigate whether there is a link between ocular dryness and gastroesophageal refl

Visual prognosis of Behçet's uveitis patients in China and its associated factors.

BACKGROUND: Behçet's disease (BD) is a chronic, relapsing, multisystem inflammatory disorder, with ocular involvement being a major cause of irreversible visual impairment. Behçet's uveitis (BU) often presents as bilateral, recurrent, non-granulomatous panuveitis with occlusive retinal vasculitis. Despite advances in immunosuppressive and biologic therapies, long-term visual outcomes remain variable, and factors associated with poor prognosis, particularly in Chinese populations, are not well defined. METHODS: We conducted a retrospective cohort study of 153 Chinese patients (268 eyes) with B

Mitochondrial function-associated genes underlie cortical atrophy in prodromal synucleinopathies.

Isolated rapid eye movement sleep behaviour disorder (iRBD) is a sleep disorder characterized by the loss of rapid eye movement sleep muscle atonia and the appearance of abnormal movements and vocalizations during rapid eye movement sleep. It is a strong marker of incipient synucleinopathy such as dementia with Lewy bodies and Parkinson's disease. Patients with iRBD already show brain changes that are reminiscent of manifest synucleinopathies including brain atrophy. However, the mechanisms underlying the development of this atrophy remain poorly understood. In this study, we performed cutting

Changes and significance of retinal blood oxygen saturation and oxidative stress indexes in patients with diabetic retinopathy.

BACKGROUND: Diabetic retinopathy (DR) is a diabetic complication that can severely affect the patients' vision, eventually leading to blindness. DR is the most important manifestation of diabetic micro-vasculopathy and is mainly related to the course of diabetes and the degree of blood glucose control, while the age of diabetes onset, sex, and type of diabetes have little influence on it. AIM: To explore the changes in blood oxygen saturation and oxidative stress indices of retinal vessels in patients with DR. METHODS: In total, 94 patients (94 eyes) with DR (DR group) diagnosed at Jianyang

Laser therapy for retinopathy in sickle cell disease.

BACKGROUND: Sickle cell disease (SCD) includes a group of inherited haemoglobinopathies affecting multiple organs including the eyes. Some people with SCD develop ocular manifestations. Vision-threatening complications are mainly due to proliferative sickle retinopathy, which is characterised by proliferation of new blood vessels. Laser photocoagulation is widely applicable in proliferative retinopathies. It is important to evaluate the efficacy and safety of laser photocoagulation in the treatment of proliferative sickle retinopathy (PSR) to prevent sight-threatening complications. OBJECTIVE

Multivariate Analysis of the Spatial Distance Between the Corneal Thinnest Point and Maximum Keratometry in Keratoconic Corneas: Association with Disease Severity.

Background: This study quantified the planar distance (D, mm) between the corneal thinnest point (TP) and maximum keratometry (Kmax) and examined its association with tomographic features and keratoconus severity. Methods: Tomographic data from 140 patients (280 eyes) with bilateral manifest keratoconus were obtained with a Scheimpflug camera (Pentacam HR; Oculus, Wetzlar, Germany). D was calculated from the Cartesian coordinates of TP and Kmax. A multiple linear regression model included simulated anterior average keratometry (SimKavg), anterior and posterior radii of curvature (ARC and PRC),
