Bone mineral density in coeliac disease.

Patients with coeliac disease may have osteomalacia or osteoporosis, even in the absence of abdominal symptoms. Little is known about the effects of a gluten-free diet and villous restitution on the bone mineral density in adult patients with coeliac disease. Of the 288 patients with coeliac disease in our unit, 13 (5%) had persistent villous atrophy of the small bowel despite dietary recommendations over at least the previous 4 years. For each of these 13 patients, 1 or 2 controls with coeliac disease, matched for age, gender, menopausal state, and dermatitis herpetiformis, whose intestinal m

A review of the morphology of Perthes' disease.

There are differences of opinion about the pathogenesis of Perthes' disease. All are agreed that it is due to ischaemia, but the cause of this and the size and number of infarctions are in dispute. Through the generosity of the contributors six whole femoral heads and core biopsies of five other cases have been studied radiographically and histologically. The findings ranged from an ischaemic arrest of ossification in the capital articular cartilage without infarction to multiple complete infarctions of the epiphysial bone. The ensuing reparative process contributes to the pathology, which is

[Creation and importance of the latent, ischaemic phase as an etiologic factor in Perthes' disease (author's transl)].

The article describes the latent ischaemic phase during proximal femoral epiphysis, basing on the variable development of the vessels with reduced number of afferent arterial vessels. The importance of this phenomenon for the development of the typical pattern of Perthes' disease (Legg-Calvé-Perthes' disease) is demonstrated.

Evaluation of femoral osteotomy in the treatment of Legg-Calvé-Perthes disease.

The optimal time and type of the varus-innerrotation femoral osteotomy in the treatment of Legg-Calvé-Perthes disease (LCPD) was evaluated on the basis of 53 hips having early operations and 14 hips having late operations and also on the basis of the previous observations on the disturbed venous drainage in various stages of LCPD. The operation should be done as early as possible in the cases in which it is indicated. Both the operative results in the present series and the theoretical background based on the severe disturbances of venous circulation in the initial phases justify an early inte

Indications for femoral osteotomy in Legg-Calvé-Perthes disease.

A group of children with Legg-Calvé-Perthes disease (LCPD) who underwent femoral osteotomy were analyzed to define further those most likely to respond favorably to this procedure. Treatment was directed to removal of the protruded, flattened, and damaged osteochondral head segment--as delineated by arthrography--from the damaging influence of the lateral acetabular lip and to expose the head to the molding influence of the acetabulum during weight-bearing and motion. This containment of the head was achieved by employing femoral varus-derotation (and sometimes extension) osteotomy. "Reversed"

Venous drainage of the femoral neck in various stages of activity in Perthes' disease.

The venous drainage of the femoral neck in 73 children with Perthes' disease was analyzed from intraosseous venography films. 55 contralater symptomless hips were used as controls. In active stages of Perthes' disease, i.e. initial and fragmentation stage, the venous drainage was pathological in 46/55 hips (82%). In the restitution stage the finding was pathological in 7/18 (39%) and in healed Perthes' the venous drainage was normal in all six cases. Two out of the 55 symptomless hips showed pathological venous drainage. The disturbances in the venous drainage of the femoral neck seem to corre

Morphology of osteochondrosis and sequelae in pigs.

The morphologic features of osteochondrosis were studied in skeletons of 176 pigs of different ages. Macroscopic and histologic as well as radiologic and microradiographic methods were used. Osteochondrosis was found to be a generalized disease, characterized by a disturbance of endochondral ossification. It was concluded that early changes occur at about 4 months of age in the growth cartilage. Both the metaphyseal growth cartilage (growth plates) and epiphyseal growth cartilage (joint cartilage) were affected. Abnormal hypertrophy and hyperplasia of cartilage as well as focal degeneration we

Slipped capital femoral epiphysis. A study of 68 cases in the eastern half area of Japan.

Slipped capital femoral epiphysis is an uncommon disease among children in Japan. Eighty-one hips in 68 patients collected in the eastern half area of Japan were examined and studies in reference to the incidence, clinical characteristics, and results of treatment. The estimated yearly incidence rate in boys was 0.3 to 0.5 per 100,000 in the population between 10 and 14 years old. The higher incidence of boys (a 5.8 to 1 predilection for males) was characteristic. The disease was extremely rare in girls. Although Japanese children are generally neither as tall nor as heavy as Americans or Euro

Deformity Correction in an Adult With Hypophosphatemic Rickets.

Vitamin D-resistant hypophosphatemic rickets is an X-linked genetic disorder that presents with varying degrees of bone deformities that usually involve the tibia and femur. The deformities affect the patient's quality of life and restrict their ability to ambulate, resulting in significant psychological distress. In this report, we present an adult case of hypophosphatemic rickets with severe bilateral tibial bowing, treated with multiple osteotomies and intramedullary nail fixation, resulting in marked improvement in ambulation.

Complications of Hip Hemiarthroplasty.

Hip hemiarthroplasty is a commonly performed orthopedic surgery, used to treat proximal femur fractures in the elderly population. Although hip hemiarthroplasty is frequently successful in addressing these injuries, complications can occur. Commonly seen complications include dislocation, periprosthetic fracture, acetabular erosion, and leg-length inequality. Less frequently seen complications include neurovascular injury and capsular interposition. This article presents a comprehensive review of the complications associated with the management of hip hemiarthroplasty. [Orthopedics. 2023;46(4)

Full Femoral Osteomyelitis Caused by Fusobacterium nucleatum in an Immunocompetent Adult: A Case Report.

CASE: We present a 46-year-old man who developed a full femoral osteomyelitis caused by Fusobacterium nucleatum. The subtle presentation of the infection and the late onset of appropriate antibiotic treatment caused a devastating bone quality of the full femur. CONCLUSIONS: A successful outcome was obtained with surgical debridement, antibiotics, and return to weight bearing guided by a laboratory and radiographic scale specially designed to avoid pathologic fractures toward his full functional recovery.

An unusual presentation of primary hyperparathyroidism: pathological fracture.

Primary hyperparathyroidism revealed by a pathological fracture is very uncommon. We present a case of a 54-year-old female patient who was admitted with fracture of her right femur. She underwent closed intramedullary reconstruction nailing with bipolar locking. The pathological findings confirmed the diagnosis of primary hyperparathyroidism with brown tumor. Further tests showed increased both calcium level and PTH level. A parathyroidectomy was performed. She made an uneventful recovery and was discharged to home.

[Treatment with bisphosphonates as a possible cause of bilateral spontaneous atypical femoral fracture].

In recent years, case reports have suggested an association between long-term bisphosphonate treatment and spontaneous atypical femoral fractures. We present a case with a 67 year-old female who was treated with bisphosphonate for eight years and within one year sustained bilateral spontaneous atypical femoral fractures preceded by uncharacteristic hip/thigh pain. X-ray of proximal femur is indicated in patients with long-term bisphosphonate use and non-traumatic hip/thigh pain.

Making a case for anterior inferior iliac spine/subspine hip impingement: three representative case reports and proposed concept.

Femoroacetabular impingement is typically described as occurring due to a conflict between the femoral head-neck junction and acetabular rim. A prior case report described an open decompression of the anterior inferior iliac spine (AIIS) due to impingement against the proximal femur. AIIS impingement may be developmental or the result of a prior AIIS avulsion or pelvic osteotomy. We describe 3 representative cases with minimum 1-year follow-up treated with an arthroscopic AIIS decompression.

Deep venous thrombosis revealed during ultrasound-guided femoral nerve block.

Ultrasound imaging used to facilitate performance of a femoral nerve block also affords imaging of adjacent anatomical structures. Following a fracture of the femur, an ultrasound guided femoral nerve block (UGFNB) was performed to provide analgesia; this led to the incidental finding of a previously undiagnosed femoral vein thrombosis (DVT), resulting in a change in patient management before surgery. An inferior vena cava (IVC) filter was placed before intramedullary nailing of the fracture.

Early detection of caudal regression syndrome: specific interest and findings in three cases.

Caudal regression syndrome (CRS) is a rare malformative syndrome seen mainly in cases of maternal diabetes with poor metabolic control. Early detection by vaginal ultrasound is possible. The authors describe three cases of CRS, relating the characteristic ultrasound findings which include abrupt interruption of the spine at the dorsal or lumbar level and abnormal position of the lower limbs. The femur bones are fixed in a 'V' pattern, giving a typical 'Buddha's poise'.

Osteosarcoma in a 16-year-old boy with Baller-Gerold syndrome.

We report a 16-year-old growth deficient boy with craniosynostosis, radial hypoplasia and hypoplastic thumbs. These findings are consistent with the autosomal recessively inherited Baller-Gerold syndrome (BGS) which furthermore shows a great variability of concomitant occasional anomalies. At the age of 16 years our patient suffered from an osteosarcoma of the left distal femur. The occurrence of malignancies has not yet been reported in the hitherto described 22 patients with BGS.
