Understanding Physician Referral for Chimeric Antigen Receptor T-Cell (CAR-T) Therapy.

Chimeric antigen receptor t-cell (CAR T) therapy is a promising treatment option for patients with various types of leukemia and lymphoma, yet little is known about oncologists' attitudes, knowledge, and referral patterns for this therapy. Our goal was to identify potential gaps in Florida oncologists' attitudes, knowledge, and referral patterns for CAR T in patients with diffuse large b-cell lymphoma, transformed follicular lymphoma, primary mediastinal b-cell lymphoma, and acute lymphoblastic leukemia. Oncologists and Hematologists in Florida were identified from the American Medical Associa

Changes in resting-state functional connectivity linked to affective symptoms: insights from a population-based study of adolescents and young adults.

First episodes of affective disorders often emerge during adolescence and young adulthood. Alterations in resting-state functional connectivity (RSFC) have been reported in affective disorders, yet findings are heterogeneous and associations of RSFC with subclinical affective symptoms in community samples remain limited. A better understanding of these underlying neurobiological mechanisms may aid in identifying early vulnerability markers of affective disorders. We examined associations between affective symptom severity and both static and dynamic RSFC using resting-state fMRI data from 512

Explainable 3D CNNs link regional and network level disruption in early Parkinson's MRIs to symptom progression.

Parkinson's Disease (PD) is a progressive neurodegenerative disorder affecting approximately 1% of the population over 65. Clinical diagnosis typically depends on tracking gradually developing motor symptoms as the disease progresses, underscoring the need for early detection methods to aid intervention while symptoms are still minor. Inexpensive and widely available imaging modalities such as T1-weighted MRI (T1w MRI) have potential for early PD diagnosis but lack established systematic biomarkers of PD pathology. In this study, a 3D convolutional neural network (3D CNN) was trained on 100 pr

Identification of novel gammaherpesviruses in North American carnivores.

We investigated the diversity of gammaherpesviruses (GHVs) present in North American carnivorans using GHV-targeted consensus PCR to amplify viral glycoprotein B (gB) and DNA polymerase (Dpol) genes. For each unique GHV identified, we sequenced a 3.5 kb region spanning gB to Dpol. Screening of spleen DNA from 263 individual animals comprising 17 carnivoran species from 7 families resulted in the identification of 8 unique GHVs, each associated with a single host species. We identified five viruses from mustelid species (ermine, marten, mink, fisher, and river otter) and the first GHV from a me

Genetic risk factors for periodontitis: a genome-wide association study using UK Biobank data.

OBJECTIVES: Periodontitis is linked with many health conditions, but its genetic basis is not yet understood. This genome-wide association study (GWAS) aimed to investigate the genetic variants associated with periodontitis. MATERIALS AND METHODS: This study utilised UK Biobank participants of European descent. Individuals were categorised as "having periodontitis" if they self-reported having 'painful gums', 'bleeding gums' or 'loose teeth' (n = 68,482), or as "controls" for those without these symptoms (n = 307,342). We conducted GWAS of this binary periodontitis phenotype using logistic re

Neuroactive steroids and the pathophysiology of PTSD: Biomarkers for treatment targeting.

Posttraumatic stress disorder (PTSD) is a disabling psychiatric disorder that arises after acute or chronic exposure to threatened death, serious injury, or sexual violence. The pathophysiology of PTSD is complex and involves dysregulation of multiple interacting brain regions and neurobiological systems including the sympathetic nervous system, the hypothalamic-pituitary-adrenal (HPA) axis, and the immune system. Deficient biosynthesis of neurosteroids that positively modulate GABAA receptor function, including allopregnanolone (Allo) and its equipotent stereoisomer pregnanolone (PA), also af

Evolution of MRI Parameters from the Subacute to Chronic Phase After Human Traumatic Cervical Spinal Cord Injury: a Prospective, Observational Study. Part 2: Diffusion Tensor Imaging Characteristics.

PURPOSE: Imaging modalities, particularly magnetic resonance imaging (MRI), have become the gold standard for assessing lesion characteristics of traumatic spinal cord injuries (SCI). Diffusion tensor imaging (DTI), an advanced MRI technique, offers insights into microstructural changes in white matter tracts. While previous studies focused on either acute or chronic SCI, few have examined longitudinal changes during the transition from acute to chronic stages of injury. This study addresses this gap by analyzing the evolution of DTI metrics over the first year following cervical SCI. METHODS

NMR-Based Metabolomics Reveals Metabolic Pathway Disruptions and Potential Biomarkers for Fibrosis.

The landscape of chronic liver disease has changed significantly, with metabolic dysfunction-associated steatotic liver disease (MASLD) now emerging as the most widespread form worldwide. In Asia, particularly in India, the prevalence of MASLD is increasing, largely driven by poor dietary habits and a sedentary way of life. MASLD spans from fat deposition to inflammation and fibrosis. Fibrosis stands out as the most critical indicator of liver-related complications and overall risk of death in MASLD. Early identification of fibrosis is critical, but current tests are often invasive or unreliab

Cleavage-stage embryo segmentation using SAM-based dual branch pipeline: development and evaluation with the CleavageEmbryo dataset.

MOTIVATION: Embryo selection is one of the critical factors in determining the success of pregnancy in in vitro fertilization procedures. Using artificial intelligence to aid in embryo selection could effectively address the current time-consuming, expensive, subjectively influenced process of embryo assessment by trained embryologists. However, current deep learning-based methods often focus on blastocyst segmentation, grading, or predicting cell development via time-lapse videos, often overlooking morphokinetic parameters or lacking interpretability. Given the significance of both morphokine

Extracorporeal Membrane Oxygenation as a Bridge to Recovery in Children With Severe Necrotizing Pneumonia: Case Report.

Necrotizing pneumonia is a severe complication of bacterial or viral pneumonia that can lead to refractory respiratory failure in children. Extracorporeal membrane oxygenation (ECMO) has emerged as a vital rescue therapy when conventional treatments fail, primarily serving as a bridge to recovery. We describe the successful use of venovenous ECMO (VV-ECMO) in two pediatric patients diagnosed with severe necrotizing pneumonia. The first patient, a two-year-old boy, developed necrotizing pneumonia caused by parainfluenza 3 virus. He required seven days of VV-ECMO support for refractory hypoxic h

Increasing capacity for ethnically-based community leaders to engage in policy change: assessing the impact of a train-the-trainer approach.

BACKGROUND: The COVID-19 pandemic exacerbated existing disparities in healthcare access and outcomes, particularly among underserved communities. As one site participating in the NIH-funded Community Engagement Alliance Against COVID-19, our focus was to address COVID-19 disparities by training immigrant and refugee communities to advocate for their needs by increasing capacity to campaign for policy-level changes. OBJECTIVE: To evaluate the impact of a train-the-trainer policy advocacy program for ethnically-based community leaders within San Diego County using a mixed-methods evaluation. M

Impact of crisis intervention on mental health in the context of specific civilian emergencies.

BACKGROUND: The implementation of crisis response strategies, such as natural hazards, pandemics, and conflicts, is necessary during times of emergency. Despite the importance of these interventions, mental health outcomes in emergency situations remain poorly understood. There is a lack of research on the comparative effectiveness of different interventions. Therefore, this study addresses the following question: "How do crisis interventions affect mental health outcomes in emergencies?". METHODS: This study aims to conduct a scoping review of the impact mechanisms of crisis intervention on

Can hair concentrations of artemether-lumefantrine be used as a tool to retrospectively determine drug exposure during malaria treatment?

BACKGROUND: Artemether-lumefantrine (AL) is the first-line treatment for uncomplicated Plasmodium falciparum malaria, with cure rates exceeding 95 %. However, recrudescence occurs in 2-14 % of cases, often linked to inadequate lumefantrine exposure. Retrospective assessment of drug exposure in recrudescence cases is challenging, as lumefantrine levels are undetectable in blood after several weeks. Hair analysis may offer an alternative method to assess drug exposure over time. The objective of this proof-of-concept study was to assess whether artemether and lumefantrine, and their respective m

Creating a Superiorly Based Pharyngeal Flap: Is Prominence of C1 Superior Enough, and Will Adenoid Pad Be in the Way?

BACKGROUND: Positioning the pharyngeal flap base high along the posterior pharyngeal wall is essential for optimizing speech outcomes. Objective data on where to place the flap base are lacking. Further, adenoid tissue can restrict cephalad positioning of the flap. This study aimed to improve the design of the pharyngeal flap by measuring the distance from the first cervical vertebrae (C1) to the palatal plane, and the adenoid depth in children undergoing evaluation for velopharyngeal insufficiency. METHODS: This retrospective cross-sectional study analyzed magnetic resonance imaging (MRI) sc
